DESIGN/BUILD

STATEMENT OF

WORK FOR

CONSTRUCT 5 ISU STORAGE PAD

PROJECT: VLSB 17-0100 24 Aug 17

The Contractor shall provide all design, labor, material, equipment and supervision to complete the project to construct 5 concrete pads.

The Contractor shall take all precautions to ensure that their operations are conducted in a manner so as not to interfere with the normal operations of the specified facilities nor the safety and convenience of the base personnel. Any after normal hour working time will be approved by the Contracting Officer.

The Project Inspector will serve as a point of contact for all demolition, construction and any required power outages throughout the repair process. Contractor shall have an approved AF Form103 prior to beginning any on site work activities.

In addition to DD1354 requirements already identified in the Shaw Design Standards, all Contractors shall also be responsible for submitting a completed DD1354 (AUG 2013 edition, PDF file) with each progress payment request (beginning at 50% construction completion). This form shall be marked "Draft" until the Contractor has reached approximately 90% completion, or when it has been determined that funding will not be adjusted due to modifications. The Contractor shall have a reviewed and accepted DD1354 submitted to the Government Inspector two (2) weeks prior to the scheduled final inspection date (i.e. Placed in Service Date). This reviewed and accepted DD1354 shall be marked "Interim" until the final inspection occurs.

All work shall be done in accordance with the International Building Code (IBC), National Electrical Code (NEC), National Electrical Safety Code (NESC), National Fire Protection Association (NFPA), United Facilities Criteria (UFC), and Shaw Air Force Base Design Standards, latest editions.

SPECIFICS:

1) Place 5 each 20’ x 40’ pads, 6 inch thick with a 12 inch thickened edge with a 3 foot transition to the 12 inch thickened edge with saw cuts every 10’. The concrete pads as a minimum shall meet this SOW, the attached slab detail and UFC 3-320- 06a (Slab on Grade).

2) The concrete pad shall be a minimum of 2 inches above surrounding grades with #4 rebar and aggregate/sand base compacted 95 % of Modified Proctor which the maximum compacted laboratory density as determined by an accredited soils laboratory with broom finish.

3) Concrete pads shall be constructed so as not to dam or puddle water on the site.

The Contractor shall furnish and install all necessary drainage controls to keep the site free of standing water.

4) All pad sites have been chosen and approved for this function and grading for each pad location is different. Site photo attach for all pads locations.

GENERAL CONSTRUCTION NOTES

a. Construction shall be in strict accordance with the Shaw AFB Design Standards. The information provided in this SOW, the provided drawings, and the requirements of the Shaw AFB Design Standards shall be determined to be the minimum standards required in this project. Where the information provided and/or Shaw AFB Design Standards conflict, the most stringent shall be used. This includes all standards, codes, regulations and requirements provided by reference.

b. As built drawings, shall be sealed, signed and dated by a Professional Engineer (PE) who is a design professional in the area covered by the sheet(s) of the submittal they seal.

c. Contractor are require to take a 4 hour Flight line training at Base OPS before work began.
