FA4625-16-Q-0019
This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR 12.6 and FAR 13.5, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; Quotations are being requested and a written solicitation will not be issued. This solicitation is issued as a Request for Quotation (RFQ), solicitation number FA4625-16-Q-0019. This procurement is a 100% small business set-aside. The North American Industry Classification System (NAICS) code for this acquisition is 811310. For the purpose of this procurement the small business size standard is $7.5M. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-86, Defense Acquisition Circular 20151230, and Air Force Acquisition Circular 2015- 1001. Award will be a firm fixed price contract.
Paper copies of the Request for Quotation will not be made available. Quotations can be e-mailed to the following e-mail address: 509cons.sollgca@us.af.mil or by FAX to (660) 687-4822 to the attention of SSgt Theodore Church-Reeves. NOTE: Quotations must be received by 14 March 2016, 2:00 PM CST; late quotations will not be considered for award. Prospective Offeror must be registered and have an active status in the System for Award Management (SAM) (www.sam.gov) in order to be eligible for award. Responsible sources may submit a quotation which, if timely received, shall be considered by this agency. Evaluation for this acquisition shall be a best value determination using a lowest price technically acceptable determination, with past performance consideration, IAW FAR 15.101. All quotes shall be valid for 60 days. Your quote shall be firm fixed price and consist of a Bid Schedule, Mission Essential Contractor Services Plan, Quality Control Plan, and Past Performance References.

Contract effective date is 1 Apr 2016 or date of signed award, whichever is later.

a. Provide a Quality Control Plan and identify key personnel. At a minimum, key personnel to be identified are the quality control technician and lead technician. Quality Control technician and lead technician shall have a minimum of 2 years experience in their trade, and provide in resume format.
b. Past performance shall be related to electrical and hydraulic work, with at least one reference for vehicle work. Past performance is limited to the last 5 years from 10 Feb 2016.
Provide at least 2 past performance questionnaires and no more than 10.
