Statement of Work (SOW)
Repair and Recondition Well #5, Pump #4 Motor and Pump
8 July 2014
1. DESCRIPTION OF SERVICES. The contractor shall remove pump and motor #4 at Well #5, visually inspecting pump and motor for damage. See attached Well #5, Pump #4 data sheet (ATCH 4).
This purchase and installation shall comply with federal, state, local laws and regulations, commercial standards, manufacturer’s specifications, National Electric Code (NEC), and this statement of work. All work accomplished shall be performed by appropriately qualified, certified personnel.
1.1 Contractor shall take appropriate measures/precautions as to not contaminate Aquifer. Follow
American Water Works Association (AWWA) disinfection procedures.
1.2 Motor and pump shall be evaluated as to cause of failure, cause of excessive pump/motor vibration and evaluated for loss of pumping capabilities. Provide Government a report on findings.
1.3 Contractor shall inspect all electrical connections and connections between 100 HP motor and pump, then perform diagnostic test on motor and pump. A report on the pump and motor defects shall be provided to the Government for evaluation.
1.4 Contractor overhaul pump in accordance with manufacturer’s standard overhaul instructions, then check pump for correct tolerances. A written report shall be provided to the Government denoting defects and the parts needed to facilitate repairs. The report is to identify those parts requiring replacement that is outside those required by the manufacturer’s standard overhaul instructions.
1.5 The Government shall approve replacement parts based on the list of defective parts as identified in the report. The contractor shall not replace any parts or incur any additional expense without Government approval.
1.6 Contractor shall provide a cost estimate to provide and install the replacement parts for the pump and/or motor, and a cost estimate to replace the pump and/or motor.
1.7 Government shall make the decision at that time to either repair or replace the equipment prior to reassembly and reinstallation of pump and motor by the contractor.
1.8 If the Government decision is to repair the pump and/or motor, contractor shall complete repairs, reassemble and reinstall pump and motor in Well #5 and shall demonstrate satisfactory pumping capabilities to the Government representative. Pump and motor shall produce
400GPM measured at the wellhouse and measured against the normal head for this pump.
1.9 If the Government decision is to install a new pump and/or motor, the contractor shall analyze all aspects of the well to determine its characteristics and propose a pump/motor that makes maximum use of those characteristics, subject to approval by the Government.
1.10 Contractor shall have a vibration test performed during startup. The vibration test shall be performed by a Level 2 certified vibration analyst as a minimum. Contractor shall correct any and all problems identified during vibration analysis.
2. GENERAL INFORMATION.
2.1 Period of Performance: Period of performance for removal, disassembly, overhaul and submission of report to Government shall be 14 days after receipt of purchase order.
2.2 Place of Performance: Services are to be performed at Fort George Wright Annex Building
#1002.
2.3 Equipment and Personnel: Contractor shall provide all equipment and personnel necessary to complete this task. This task will require the use of a crane, accessing the equipment through a removable roof hatch.
2.4 Hours of Operation: Fairchild AFB normal hours of operation is Monday‐Friday, 7:00 AM to 4:00
PM, excluding federal holidays. The following federal holidays are observed:

3. Continuation of Mission Essential Functions: IAW DoDI 1100.22 (Policy and Procedures for Determining Workforce Mix), and any applicable Air Force implementation, unless otherwise directed by an authorized Government representative, it is determined that services provided under this contract are not mission essential for performance during a crisis.
