STATEMENT OF WORK

Design-Build (D-B)

Repair HVAC, Bldg 9190 Project Location: Vandenberg AFB, CA Base Project Number: XUMU 12-1008B

5 May 2015

1.0 INTRODUCTION

This work will be conducted under a Design-Build (DB) delivery method.

2.0 SCOPE OF WORK

2.1 SCOPE & APPLICABLE DOCUMENTS

This Statement of Work (SOW) sets forth the requirements for a design-build construction effort at Vandenberg AFB, CA. The Contractor is to provide the Government with a complete engineering design, construction, and warranty for the renovation of Bldg 9190s HVAV systems needed to meet user and Air Force Standards herein.

2.2 DESCRIPTION OF WORK

The heating ventilation and air conditioning system at building 9190 has reached the end of its expected service life and will need to be replaced. The majority of this the renovation work is a repair by replacement, but some new HVAC DDC control systems will need to be designed and installed. The contractor will be provided a 35% design and will be required to complete a 100% design. The design process must follow the rules outlined in the SOW and be approved by the government. After the design is approved the contractor will construct the approved 100% design. As part of the proposed effort on this contract, the Contractor shall implement the following tasks for the completion of this project. The Contractor’s proposal shall include information on each type of equipment they are bidding to install (i.e. cut sheets from the manufacture) along with a basic sketch of planned improvements to what is required as part of this SOW.

2.2.1 General Description of Project:

The Contractor shall design and construct HVAC renovations at building 9190 in accordance with the provided design documents. The contractor shall use the government provided 35% Design (SOW Attachment 1) and finish the design in accordance with industry standards and the DOD’s Uniform Facility Codes. The contractor is required to complete all of the work identified in the government provided 35% design and the contractor’s approved 100% design. Some of the critical tasks are as follows:

a. The contractor shall take the 35% design and perform required calculations and design work needed to provide a full and complete design that follows the processes outlined in this SOW.

b. The contractor shall demolish four existing roof mounted Air Handling Units (AHUs), and related equipment, including controls, panels, dampers and appurtances.

c. The contractor shall demolish existing roof top refrigeration unit and related equipment.

d. The contractor shall demolish nine existing roof top exhaust fans.

e. The contractor shall install four new roof mounted AHUs, and related equipment, including controls, panels, dampers and appurtances.

f. The contractor shall install new roof top refrigeration unit and related equipment deemed required.

g. The contractor shall install nine new roof top exhaust fans.

h. The contractor shall demolish existing hot water pumps and install new hot water pumps in mechanical room.

i. The contractor shall design and install new Building Management System (BMS) in accordance with the 35% design.

j. The contractor shall demolish electric hot water booster for dish washer in mechanical room.

k. The contractor shall design and install new gas hot water booster in mechanical room. The new gas hot water booster shall at a minimum meet the requirements of the demolished electric hot water booster.

l. The contractor shall perform duct cleaning of existing ducts in accordance with industry standards.

NOTES:

The Contractor shall ensure users/maintainers are trained on all operations and maintenance of all devices.

The Contractor shall locate and avoid all utility lines.

In the event that an existing utility line must be modified to meet the final design, this must be coordinated through the Project Manager (PM) and shall be included in the contract cost.

If required, it shall be the Contractor’s responsibility to develop a traffic management plan to minimize project impact on VAFB traffic. The traffic management plan shall be coordinated through the 30CES PM and 30CES Traffic Engineering POC.

Any and all monitoring is the responsibility of the Contractor this includes, but is not limited to, those required by the AF Forms 103, 332, 813; the VAFB Fire Department; and DLA (formerly DRMO).

The Contractor shall prepare and fill out required forms and permits: AF Form 103, Base Civil Engineering Work Clearance Request, and HAZMART.

The Contractor shall coordinate schedule of work around mission and user requirements. Coordination with the CE PM is required.

ENVIRONMENTAL REQUIREMENTS:

The Contractor shall comply with all requirements identified in the AF 813, and all federal, state and local laws and regulations.

The contractor shall apply Low Impact Development (LID) standards.

DESIGN REQUIREMENTS:

The Contractor shall ensure both the design and construction complies with DoD Unified Facility Criteria

(UFC).

The Contractor shall ensure all design/construction complies with local, State and Federal standards.

2.2.2 Equipment

All environmental requirements shall be adhered to as described in SOW Attachment 4, VAFB Environmental Specifications.

The Contractor shall implement this contract in two phases: the design phase and construction phase.

2.2.6 Applicable Documents

The Contractor shall identify and comply with all applicable federal, state, and local statutes; Air Force/ Military instructions, manuals, handbooks, regulations, guidance, and policy letters; Executive Orders ; American Petroleum Institute Codes; National Association of Corrosion Engineers; National Fire Protection; Steel Structures and

Painting Council; National Electrical Code; Uniform Fire Code; Occupational Safety and Health Administration (OSHA) regulations; Environmental Protection Agency regulations; and International Building Code including all changes and amendments in effect on the date of issuance of the contract. It is the Contractor’s responsibility to identify and comply with all applicable and relevant requirements.

2.3 DESIGN PHASE

The Contractor shall complete development of plans and specifications in accordance with defined levels of effort necessary to construct the project and provide as-built drawings (both electronically and in hard copy) for retention by the Government in accordance with VAFB CAD standards. The Contractor shall be responsible for implementation, coordination, and execution of all regulatory reviews, ensure technical adequacy of the final design, and provide quality control of all phases of the contract.

2.3.1 Project Phasing

The Contractor shall accomplish this project in phases as agreed upon by the Base civil engineers (CE) and production staff. The Contractor shall coordinate all telecoms and meetings with the CE PM. The Government shall have up to 45 calendar days to review each design deliverable. The Contractor shall submit meeting minutes within 7 calendar days after each teleconference or meeting. The Contractor shall complete the work as described below.

2.3.2 Site Survey / Charrette

The Contractor shall avail themselves for a design “Charrette” on the day of or the day after the Kickoff Meeting / Site Survey. The design “Charrette” will allow the Government and/or Contractor to discuss any specific concerns not previously addressed. The Charrette shall not be concluded until the Contractor has a complete, workable design concept.

2.3.3 35% Design The Government provided design will count as the 35% for this project. This, combined with the existing design and any documentation from the Charrette will be the 35% design. No submittal is required.

2.3.4 65% Design The Contractor shall within 60 calendar days from the design charrette meeting provide 65% design for review and comment. The 65% design package shall include drawings 65% complete showing all major details and design analysis. The 65% design package documents should be representative of final design. The Government shall have up to 45 calendar days to provide review comments. The Contractor shall schedule and attend a review conference at Vandenberg AFB, CA within 7 calendar days of receipt of Government comments.

(1) Design Analysis: The Contractor shall update the Preliminary (35%) Design Submittal by documenting the decision-making process. The Design Analysis shall be bound or in notebook form and include the following:

a. Basis of Design Submittal: The Contractor shall include the approved Basis of Design as a reference for design review.

b. List of Section Titles and Numbers: The Contractor shall prepare a List of Section Titles and Numbers to be used in accordance with the Master Format - Master List of Section Titles and Numbers from the Construction Specifications Institute (CSI) Manual of Practice.

c. Environmental Surveys: The Contractor shall provide the environmental surveys and include abatement specifications if asbestos, lead-based paint, PCBs, or underground storage tanks (USTs) are affected by work pertaining to any project.

d. Recycled Materials List: The Contractor shall provide a preliminary list of recycled materials that can be used in each project. The Air Force Affirmative Procurement Program (APP) mandates use of recycled materials found in the Comprehensive Procurement Guideline (CPG). The Environmental Protection Agency (EPA) lists the types of products to be included in the APP available on-line at www.epa.gov/cpg/products.htm.

e. Written Explanation of Major Design Decisions: The Contractor shall provide a written narrative describing key design decisions and discussing the engineering reasons behind these.

f. Executive Summary: The Contractor shall provide a written narrative overviewing the project scope and key project information. This shall be at the front of the Design Analysis.

g. Interviews: The Contractor shall document all discussions with the Air Force Project Manager, using agency, and other base officials including names, position titles, and telephone numbers.

h. Engineering Data: The Contractor shall provide calculations and other supporting engineering data or analyses for equipment or system sizing and shall justify any special requirements that require deviation from normally accepted commercial or Government standards.

i. Manufacturer Data: The Contractor shall provide catalog cuts or other manufacturer data of major equipment items.

j. Annotated Review Comments: The Contractor shall annotate the design review comment sheets received previously by indicating the full disposition of each comment. Provide the reference location where any changes were made to the construction contract documents.

(2) Drawings: The Contractor shall make corrections from the Preliminary (35%) Design Review Conference and submit two half size (14” x 20”) copies of updated Drawings plotted on bond.

(3) Specifications: The Contractor shall prepare the Specifications in order for a construction contractor to determine the type and quality of materials and methods required for installation of these materials.

The Specifications shall be bound or in notebook form and shall follow these guidelines:

a. The Contractor may use any Government or commercial guide specification service or their own specifications; however, the specification format shall strictly follow the Construction Specifications Institute (CSI) Manual of Practice chapters on Master Format - Master List of Section Titles and Numbers, 3-part Section Format, and Page Format.

b. The Contractor shall specify generic characteristics of construction materials and shall avoid the use of proprietary “brand name” specifications, unless Vandenberg AFB requires a specific manufacturer for base-wide compatibility.

c. The Specifications shall be clearly written and shall not conflict with the drawings. The

Specifications shall be thoroughly edited so that they do not contain provisions that do not pertain to any project in this contract.

d. The following Vandenberg AFB standard specification sections shall be inserted into the

Specifications for all projects:

e. Sections 01000 and 015720 require editing and coordination with the Air Force Project Manager for specific base requirements and restrictions. Section 01000 provides a project description and other conditions such as occupancy, work schedule, work stoppages, security, utilities, safety, construction and welding permits, and submittals. Section 015720 specifies environmental compliance and monitoring and disposal of removed materials. Special requirements may be added.

h. The Contractor shall use one or more of the following Vandenberg AFB standard specification sections if asbestos, lead-based paint, PCBs, or USTs exist in any project:

- Section 02075 Removal and Disposal of Asbestos Materials

- Section 02081 Asbestos Abatement – Small Scale

- Section 02082 Asbestos Abatement – Large Scale

- Section 02084 Lead Based Paint (LBP) Abatement and Disposal

k. The Contractor shall use the Environmental Surveys to prepare the scope of asbestos, lead-based paint, PCB, and UST abatement and identify the locations of abatement requirements on the Drawings. The cost of all abatement shall be itemized separately in the abatement cost estimate.

l. Additional specifications have been provided as part of the RFP package and are required to be included if applicable to the project.

j. The Contractor shall become familiar with the Vandenberg AFB standard specification sections to ensure that the remaining specification sections do not conflict with or duplicate these provisions.

(4) Schedule of Material Submittals: The Contractor shall complete an AF Form 66, Schedule of Material Submittals. The AF Form 66 indicates all material submittal requirements throughout the specifications listed by description of Item, Contract Reference, and Type of Submittal. The AF Form is available in MS Excel.

(5) Recycled Materials List: The Contractor shall provide a final list of recycled materials that can be used in each project. The Contractor shall verify the classification of each recycled material by completing a Recovered Material Determination Form available on-line at http://www.afcee.brooks.af.mil/eq/programs/progpage.asp?PID=1 in the “AF Guide to Green Purchasing” Appendix C.

2.3.5 95% Design The Contractor shall within 42 calendar days from 65% review meeting provide 95% design for review and comment. Included shall be all drawings, specifications, and design analysis. This package shall be in a 100% design state with the exception of Government corrections/modifications. Provide design to the individuals list below. Government shall have up to 45 calendar days to provide review comments. The Contractor shall schedule and attend final review conference meeting at Vandenberg AFB, CA within 7 days of receipt of Government comments.

(1) Design Analysis: The Contractor shall make final corrections from the Pre-Final (65%) Design Review Conference and submit an updated Design Analysis. The Design Analysis shall be bound or in notebook form. In addition to Pre-Final (65%) Design Submittal requirements, the Design Analysis shall include the following:

a. Annotated Review Comments: The Contractor shall annotate the design review comment sheets received at the Pre-Final (65%) Design Review Conference indicating the full disposition of each comment. Provide the reference location where any changes were made to the contract documents.

(2) Drawings: The Contractor shall make corrections from the Pre-Final (65%) Design Review

Conference and submit two half size (14” x 20”) copies of updated Drawings plotted on bond.

(3) Specifications: The Contractor shall make final corrections from the Pre-Final (65%) Design Review Conference and submit updated specifications. The Specifications shall be bound or in notebook form.

(4) CAD Project File List and Computer Files: The Contractor shall make final corrections from the

Pre-Final (65%) Design Review Conference and submit an updated CAD Project File List and Computer Files, two copies on CD.

(5) Resubmittals: The Contractor may be required to resubmit the Backcheck (95%) Design Submittal if all changes from previous submittals have not been incorporated.

2.3.6 100% Design Within 14 days from this review meeting, the Contractor shall provide the 100% Final Design package. The Government shall have up to 21 days to approve the 100% design. After final acceptance of the Backcheck (95%) Design Submittal by the Air Force Project Manager, the Contractor shall submit the required originals and two sets of all corresponding documents plus all Computer Files.

2.3.7 Verification of Existing Conditions and Site Survey

Use of existing condition data provided by the Air Force conveys acceptance and as such does not relieve the Contractor of liability associated with performance as the Engineer of Record. The Contractor shall be solely responsible for verification and validation of existing conditions, coordination of existing conditions in parallel with proposed requirements, and above and below-grade condition assessment.

2.3.8 Floor Plans/Elevations Design Requirements

The subject project shall be compatible with all United States Air Force and Vandenberg AFB, CA Guidelines provided in the attached Vandenberg specifications and UFC requirements.

2.3.9 Architect of Record

The term “Architect of Record” in the context of this contract shall mean the principal professional Architect or Engineer in the State of California. The Contractor shall serve as the “Architect of Record” and, as such, will ensure that all completed designs and as-builts are signed and sealed by registered professionals. The Contractor is accountable for all aspects of the completed design and as-built drawings under all applicable Federal, State and local laws, regulations and base-specific standards.

2.3.10 DESIGN SUBMITTALS

The Contractor shall submit the following sets of Design Submittals to the Air Force Project Manager and Air Force Contract Specialist by whatever means necessary to meet the submittal schedule:

2.3.10.1 PRE-FINAL (65%) DESIGN SUBMITTAL:

(1) Air Force Project Manager

- 2 sets of Design Analysis, Drawings, and Specifications. Hard copy and CD.

- 2 sets of CAD Project File List, Computer Files, and Drawings on CD.

(2) Air Force Contract Specialist

- 1 set of Design Analysis, Drawings, and Specifications on CD

(3) Customer

- 1 set of Design Analysis, Drawings and Specifications on CD

2.3.10.3 BACKCHECK (95%) DESIGN SUBMITTAL:

- 2 sets of CAD Project File List, Computer Files, and Drawings on CD.

- 1 set of Design Analysis, Drawings, and Specifications on CD

2.3.10.4 FINAL (100%) DESIGN SUBMITTAL:

- 2 sets of unbound single-sided original Specifications on high quality bond paper with the cover sheet stamped by the A-E California Registered Professional Engineer or Architect.

- 1 set of original Drawings on Mylar (28” x 40”) with each drawing sheet stamped by the A-E

California Registered Professional Engineer or Architect.

- 2 sets of CAD Project File List and Computer Files on CD.

- 1 set of Design Analysis, Drawings, and Specifications on CD

2.4 CONSTRUCTION PHASE

Upon the approval of the Contracting Officer, the Contractor may commence on-site construction activities. The Contractor shall complete the work as described below:

2.4.1 On-Site Field Oversight

The Contractor shall provide the manpower, equipment, material, services, and transportation necessary to ensure oversight services throughout accomplishment of this design/build contract. It is the responsibility of the Contractor to establish and maintain open communications between salient authorities associated with accomplishment of this contract.

2.4.2 Delivery/Warranty

The Contractor shall complete all inspection and commissioning requirements prior to final inspection. Following final inspection, the Contractor shall deliver to the Government the signed and sealed as-built drawings in both hard copy and reproducible format. The Contractor shall issue a warranty in accordance with FAR 52.246-21.

3.0 GENERAL REQUIREMENTS

3.1 QUALIFIED PERSONNEL

The Contractor shall ensure that only qualified, competent personnel carry out the tasks outlined in this SOW.

Competent is defined as registered professional or, where registration is not applicable, trained, and/or certified in their respective field.

3.2 AIR FORCE PROPERTY

All materials gathered and/or developed in the performance of these tasks listed shall be returned to and become property of the Air Force; and shall not be used and/or distributed by the Contractor without specific written permission of the CE PM.

Material resulting from demolition activities shall be disposed of by the Contractor through the 30CES Qualified Recycling Program, the Defense Logistics Agency (DLA) Disposition Services or off base landfill, as appropriate.

The VAFB Base Landfill is no longer available for projects at Vandenberg AFB.

3.3 DESIGN AND CONSTRUCTION REQUIREMENTS

The project shall conform to all applicable Federal, State and local laws, regulations and base-specific standards and applicable Unified Facilities Criteria (UFC). Where there is a conflict between the referenced standards, the most stringent shall take precedence. All work accomplished by the Contractor shall comply with these criteria unless a waiver is obtained from the Air Force when conditions warrant such consideration.

4.0 GOVERNMENT FURNISHED ITEMS

The following is a list of specific items of support that the Base CE will provide:

None

4.1 AF FORM 103

The Base CE will assist the Contractor with efforts to fully complete the Base Civil Engineering Work Clearance Request (AF Form 103). The Base CE will locate underground utilities and issue the AF Form 103 to the Contractor. It is the Contractor’s responsibility to have a completed and approved AF Form 103 prior to beginning any construction activities including clearing, grubbing, exploratory geophysical work, etc.

4.2 STAGING AREA

The Base CE will identify space on the jobsite or in a base contractor staging area where the Contractor may deliver non-hazardous materials within the installation. An approval letter from Base planning is required for laydown areas.

4.3 EXISTING ENGINEERING PLANS, ETC.

The Base CE will provide the Contractor access to existing engineering plans, drawings, diagrams, aerial photographs, digitized map files, etc., to facilitate evaluation of the site(s), as needed or requested.

4.4 PRECONSTRUCTION MEETING

The Base CE will host a pre-construction (i.e., Kick-Off) meeting to advise and coordinate on local procedures, safety and security requirements, and project coordination activities. The CE PM will arrange for a suitable Kick- Off meeting location with teleconferencing capabilities to ensure contracting officer (CO) participation. The CE PM will ensure the appropriate base agencies attend the Kick-Off meeting to provide necessary information or briefings to facilitate the success of the contract effort.

4.5 BASE REQUIREMENTS

The Base CE will advise the Contractor of the location and processes to obtain the following as needed:

a. Personnel identification badges, vehicle passes, and/or entry permits.

b. Information on base fire prevention/security practices and procedures.

c. Access to utilities (electrical, water, sewer, phone, etc.) This includes a supply (e.g., fire hydrant or stand pipe) of large quantities of potable water.

d. Security escorts for work in restricted areas.

e. Safety training classes for the purpose of allowing access within secure or hazardous areas.

f. A paved area where equipment can be cleaned.

g. A set of keys to locks as necessary.

4.6 CIVILIAN ENGINEERING PROGRAM MANAGER (CE PM)

The Base CE will provide a CE PM who will expeditiously review and certify the Contractor’s progress by signing AF Form(s) 3064 and 3065 and monthly Status Reports for the Contractor. (Note: If the CE PM does not agree that the progress is as represented by the Contractor, he/she shall return the documents to the Contractor for correction.)

5.1 PROJECT MANAGEMENT PLAN

The Project Management Plan shall include a description and flowchart of personnel involved with the project and a thorough description of construction management. The construction management narrative shall include timing of permitting, lead time for material, use date for user, etc.

5.2 CONTRACT PROGRESS SCHEDULE (AF 3064)

The Contractor shall submit a signed AF 3064 Contract Progress Schedule, depicting an overall contract progress schedule for the main elements of work for the period of performance as described in the Table of Deliverables above. The Contractor shall also provide a line graph depicting actual construction progress (solid line) versus scheduled construction progress (dotted line) throughout the period of performance.

The CE PM and CO shall approve the first (baseline) progress schedule and it shall not be altered without the approval of the CO.

The CE PM and CO shall sign subsequent and periodic updates to the progress schedule.

5.3 CONTRACT PROGRESS REPORT (AF 3065)

The Contractor shall submit AF 3065 as described in the Table of Deliverables above. The AF 3065 shall be signed by the Contractor’s on-site representative in the Remarks section of the AF 3065. The work elements and percentages of the total job identified on the AF 3065 shall be identical to the work elements and percentages on the AF 3064. If the Contractor is more than 5% behind schedule, the Contractor shall note in the Remarks section the reason why and what is being done to get back on schedule. The CE PM will sign the AF 3065 and send to the Contract Specialist for coordination and approval by the CO. If the Contractor requires reimbursement of a high dollar value item, the item shall be identified on the AF 3065, an appropriate percentage applied, and the item must be received and stored on site.

5.4 Monthly Status Report (MSR).

The Monthly Status Reports shall communicate activities accomplished during the previous period, document discussions, identification of problems, proposed solutions, corrective actions taken, outstanding issues, and record issues or concerns. The Monthly Status Report shall include a financial tracking table containing columns with the following headings:

a. Task Breakout

b. Current Project Status (summary of work previously completed and completed during the current reporting period).

c. Task Value

d. Effort % Complete

e. Dollars % Expensed To Date

f. Dollars Earned This Month

g. Dollars Earned To Date

h. Expected work activities for the following month

i. Problems and/or concerns

j. RFI’s and Statuses

k. Updated Submittal Register

l. Safety

m. Photographic Log of Pervious Period Operations

An approved AF 3064, AF 3065 and a Monthly Status Report shall accompany each invoice. The AF 3064, AF 3065, Monthly Status Report and invoice shall all cover the same period of time.

5.5 KICK-OFF MEETING

It is the responsibility of the Contractor to coordinate the date and time of pre-construction (i.e., Kick-Off) meeting with the Base Project Manager (CE PM, who will arrange for Contracting Officer participation), reference Para 4.4.

During the meeting, the Contractor shall present a complete overview of the project and review the schedule, highlighting any issues (review periods, material submittals, etc) which are key to maintaining the schedule. The Contractor shall take meeting minutes to include the recording of the attendance list.

The Contractor shall perform Progress Meetings as described in the Table of Deliverables. The meetings will include all relevant base POC’s and Contracting Personnel on an as needed basis. The meetings will cover the progress made in the past period and will include any pertinent issues which could affect future contract progress.

The following items shall be covered:

a. Meeting Attendees and Contact Information

b. Project Percent Complete (Actual and Scheduled)

c. RFI’s and Statuses

d. Material Submittals and Statuses

e. Three Week Look Ahead

f. Schedule

g. Safety

h. Resolution of Existing Issues

i. New Business

5.7 FORMAT OF DELIVERABLES

An electronic copy of all deliverables shall be provided, formatted for use on the latest version and compatible with the Government’s version of Microsoft Word, Excel, Adobe Acrobat, and AutoCAD. All deliverables shall also be dated. The Contractor will be provided samples of the deliverables, upon request.

5.8 DESIGN SUBMITTAL REQUIREMENTS

In addition to the requirements of applicable UFC’s, each design package shall include the following:

a. Executive summary with statement of work and all modifications.

b. Table of Contents

c. Minutes and sign-in sheets of all meetings pertaining to the development of the design.

d. Previous government review comments and contractor responses.

e. All appropriate Engineering Calculations.

f. Design Drawings.

5.9 PRE-FINAL INSPECTION

The Contractor shall conduct a pre-final walk through inspection with Base personnel and publish the pre-final inspection findings in a pre-final inspection (punch list) report. The Contractor shall include a draft DD Form 1354, Transfer and Acceptance of Real Property for the CE PM to review. The initial draft DD Form 1354 should be submitted to the CE PM at minimum 28 days prior to expected project completion.

5.10 FINAL INSPECTION

The Contractor shall conduct a final inspection with Base personnel and publish the findings in a final inspection report. The inspection shall concentrate on the items identified at the pre-final inspection and recorded in the pre-final inspection (punch list) report. A final inspection shall not be performed until the pre-final inspection (punchlist) report has been resolved. At the final inspection, the Contractor shall present a completed DD Form 1354, Transfer and Acceptance of Real Property to the Base Civil Engineer (BCE) or other appropriate organization for signature and acceptance, if required.
