1) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in the Federal Acquisition Regulation (FAR), subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; this is a Request for Quotation (RFQ) and a written solicitation will not be issued.

2) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-99, Defense Change Notice 20180629, and Air Force Acquisition Circular 2018-0525.

3) The NAICS for this solicitation is 238220 Plumbing, Heating, and Air-Conditioning Contractors. The Small Business Size Standard for this NAICS code is $15M. This acquisition is set-aside 100% for Small Business.

Minot AFB, ND 58705 The Cross Connection Survey is to be completed No Later Than 90 Days after Contract Award

8) Site Visit: A site visit will be performed on 24 August 2018 at 9:00AM CST (all personnel will meet at the 5th Contracting Squadron Building 165) at the Alpha-01 Missile Alert Facility (MAF) site and buildings 1134, 688, and 455. Due to MAF sites being restricted areas all personnel planning on attending the site visit must be added to an Entry Authorization List (EAL). This is accomplished utilizing Attachment 4 – Missile Field EAL Submittal Form. This form must be filled out for each employee which will be in attendance at the site visit. Legible photocopies of the front and back of the employee’s ID must fit onto page two. Once the form is filled out save the document as a PDF and send all employee EAL forms through the AMRDEC website (https://safe.amrdec.army.mil/safe/Default.aspx). All EAL forms must be submitted no later than 14 August 2018 to Mr. Daniel Betz – daniel.betz.2@us.af.mil and 1st Lt Trenton High – trenton.high.1@us.af.mil. Attendance must be confirmed with 1st Lt Trenton High & SSgt William Tucker no later than 22 August 2018.

Attachment 4 – Missile Field EAL Submittal Form Instructions:

A. The contractor's summarized list of employees’ information must show each employee's (use of the attached Missile Field EAL submittal form for each employee/xls document can be used instead for each employee):

1) Full name exactly as it appears on the credentialed photo identification (ID) submitted for used in conjunction with the contractor's EAL.

2) Date of birth (DOB).

3) Full Social Security Account Number (SSAN).

4) Citizenship.

5) The government issuing agency name and the control number (exactly as it appears on the credentialed photo ID) of the government issued credentialed photo ID submitted for used in conjunction with the contractor's EAL.

6) A legible photocopy of the front and back of an employee’s U.S. government issued credential photo ID must be provided.. The employee’s U.S. government issued credential photo ID must have a picture of the individual, individual's DOB and a control number. The following are acceptable government issued credentialed photo IDs: U.S. Federal (i.e. U.S. passport or U.S. passport card), or state, commonwealth, territory, protectorate, the District of Columbia, US sovereign insular area or a tribal organization recognized by the US Department of the Interior (i.e. state issued driver's license, state issued photo identification, or Tribal ID card). This ID must not be expired.

B. The contractor's summarized list of vehicle information must include:

1) Vehicle Make

2) Vehicle Model

3) Vehicle Year

4) Vehicle Primary Color

5) Vehicle License Plate # (State of Issue and Number). (If no license plate, provide the vehicle identification number (VIN) or Serial Number).

6) Vehicles include: mission essential company owned/leased/rented vehicles driven out to the MAFs or LFs and brought onsite for use (i.e. pick-up trucks, cars, vans, lineman's trucks, dump trucks (vehicles driven to and onto the site). Let us know if owned, leased, and rented.

7) All equipment (i.e. skid-steers, bobcats, forklifts, loaders/backhoes, lifts, trenchers, air compressors, welders, etc.), materials, trailers, and hand-carried items (i.e. power tools and hand tools) brought out to the sites on a trailer are not required to be listed on the EAL. But if you drive out a motor driven piece of equipment (i.e. a loader or forklift) out to the site by itself (not on a trailer), then this equipment will be considered a vehicle and information on it will need to be listed on the EAL.

8) No privately owned vehicles (POVs) will be permitted on MAFs or LFs.

9) Quotes must be valid through 30 September 2018 and signed/dated using this attachment, and submitted by 31 August, 10:00 A.M. Central Time. Quotes may be electronically submitted via email to the following points of contact. Failure to send to both points of contract will preclude the chances of receiving the award.

10) Questions may be submitted no later than 28 August 2018, 10:00 A.M. CST. Questions submitted after the specified date may not be answered.

13) Instructions to Offerors, emailed proposals will be accepted. Note email size must not exceed 10 MB. If the proposal is more than 10 MB, please submit as email 1 of 2, etc. Format must be a Word Document, PDF, or Excel sheet. Attachment 4 - Missile Field EAL Submittal Form must be submitted to Mr. Daniel Betz and 1Lt Trenton High no later than 10 August 2018. Attachment 5 – PPIF must be sent to a Government official in which backflow prevention or similar work has been completed within the last three years. The Government official is responsible for filling out the form and submitting it to contracting (see POCs above) by the RFQ due date. Any proposals submitted directly by the contractor will not be considered. The offeror must request and obtain an acknowledgement of receipt for electronic submission.

Minot AFB Identification Requirements. For either the site visit, delivery of commodities, or submission of proposals, all personnel requesting access to Minot Air Force Base must possess a valid State or Government picture identification card. Furthermore, individuals presenting identification cards from a state that is non-compliant with the REAL ID Act will require additional documentation to gain unescorted base access. Additional documentation includes a valid U.S. or foreign government issued passport, an employment authorization document that contains a photograph, or identification cards issued by federal state or local government agencies that include a photo and biographic information. Additional information to include a full list of REAL ID Act compliant and non-complaint states can be found at https://www.dhs.gov/current-status-states-territories. Personnel requesting vehicle access to Minot Air Force Base must provide a valid driver's license (compliant with REAL ID Act), current vehicle registration, and insurance.

The following Clauses and provisions applies to this acquisition:

This provision is incorporated by reference, and paragraph (a) is modified to read as follows:

(a) This is a competitive best value acquisition in which the Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, considering price and other factors. The Government intends to make award to the offeror with the best value to the Government. The following listed evaluation factor shall be used in the evaluation process:

- Price: Pricing will be evaluated for reasonableness and realism.

- Technical: Technical Acceptability is achieved when the offeror provides proof that the services will meet the standards established in Attachment 1 – Cross Connection Survey SOW and the delivery date listed above. All offerors shall submit technical narrative of the services being offered in sufficient detail to demonstrate understanding of the services being requested. This should include such information such as how the work is to be performed, equipment usage, time lines, manpower, etc. This shall not exceed 1 page in length. Additionally the contractor must provide copies of all required certifications in accordance with Attachment 1 – Cross Connection Survey SOW.

- Past Performance: A PPIF must be provided for similar work completed within the last three years to include backflow prevention.
