Attachment I
STATEMENT OF WORK (SOW) FLOORING
REPLACEMENT/INSTALLATION
Buildings 120, 130, 131, 167, 221, 505, 523, 718, 970
1. DESCRIPTION OF SERVICES: The Contractor shall provide all management, tools, supplies, equipment, and labor necessary to perform all work in accordance with these specifications. Work includes installing and or replacing: carpet roll, carpet tile, cove base, patching floor, stair tread, rubber tile and providing labor to remedy unusual requirements related to the installation or replacement of flooring materials. The carpet tile and carpet roll will be government furnished.
1.1 Work Included.
1.1.1 Prepare and measure all surfaces to receive new flooring.
1.1.2 Install government furnished carpet tile and carpet roll as specified at locations identified in attached drawings.
1.1.3 Install vinyl cove base, adhesives, rubber tile, stair treads, and flooring material as identified in paragraph 2.
1.1.4 Install contractor furnished executive carpet tiles. Provide carpet samples at time of quote.
Executive Carpet Tile Technical Specifications are in Appendix A.
1.1.5 Install edge molding/transition strips where flooring joins at other floor finishes.
1.1.6 Install used wood base boards as identified in the attached floor plans.
1.1.7 The Contractor shall dispose of all scraps and debris after installation at an off base location and vacuum the entire area where flooring was installed at no additional cost to the Government. Disposal shall be in accordance with existing local, state, and federal regulations and shall not be placed in base refuse dumpsters.
1.1.8 Upon completion of installation the contractor shall promptly notify the Civil Engineering Inspector (CEI) for final inspection and acceptance of service.
1.1.9 Carpet Tile/Roll. Contract includes the removal of carpet and rubber flooring and installation of carpet tile in accordance with (IAW) manufactures instructions (https://www.tandus-centiva.com/technical/installation) Modular ER3 & Flex-Aire RS.
1.1.10 Rubber Flooring/Stair Tread With Riser. Contract includes the removal of existing flooring and installation of rubber flooring IAW manufacturer’s instructions. Manufacture products may consist of Roppe, Flexco, Endura, Musson, etc.
1.2 Work Excluded.
1.2.1 All furniture and equipment will be moved by the using agency, facility occupants. Where the contractor elects to perform movement of furniture or equipment to gain access to additional work areas, work will be done at no cost to the Government. Any furnishings moved by the contractor shall be returned to their original location, and the contractor is liable for any damages caused by the movement of furnishings.
1.3 Reference Standards. All work shall be accomplished in accordance with CRI-2011 Carpet and Rug Institute Standards, the Manufacturers Specifications, Engineering Technical Letter (ETL) 07-4: Air Force Carpet Standard, and The Air Force Carpet Selection Handbook.
2. PRODUCTS:
2.1 Adhesives and primers shall be consistent with the type recommended by the manufacturer for the floor covering being installed. Adhesives shall be used in strict accordance with manufacturer's printed instructions.
2.2 Edge molding shall be heavy duty vinyl designed for the type of flooring being installed.
Floor flange shall be a minimum 2" wide. Color shall match the cove base as specified in Color Selection Table at the end of this document or as specified during the site visit.
2.3 After flooring is installed, the Contractor shall install set-on type, rolled, rubber cove base 1/8 inch thick and in height (either 4 inches or 6 inches) as indicated during the site visit. An adequate coved backing strip to prevent "poke-thru" shall be provided. Color is specified in color selection table on page 6.
2.4 Stair Tread: Roppe, #98 Vantage, square nose raised round pattern with black abrasive strip insert or approved equal. If providing an equal, please provide when submitting the quote.
Acceptability will be determined at time of award. Color is specified in color selection table on page 6.
2.5 Rubber Tile: Roppe, #996, raised round pattern rubber tile or approved equal. If providing an equal, please provide when submitting the quote. Acceptability will be determined at time of award. Color is specified in color selection table on page 6.
3. EXECUTION:
3.1 The CEI shall work in cooperation with the Contractor and Contracting Officer (CO) in order to schedule and coordinate the installation of flooring with the facility where the work is to be performed.
3.2 The possibility of Asbestos Containing Material (ACM) may exist in the area to be worked.
If there is ACM in the area, the Contractor shall not use any mechanical means to remove the ACM. In the event during carpet removal, it is discovered that the ACM must be removed, the Contractor must stop immediately and notify the CEI and the CO.
3.3 Removal
3.3.1 The most suitable removal method of floor coverings and associated items will be determined by the contractor.
3.3.2 Work shall be performed in a manner which creates minimum disturbance to the occupants.
3.3.3 Remove any residual glue, oil, grease, paint, plaster, and other substances detrimental to proper performance of adhesive. Allow floors to thoroughly dry before laying floor covering.
Floors shall be free of any foreign materials that would affect the proper installation of new floor covering.
3.4 Patching and Preparation of Surfaces. Inspect all areas before installation. Repair holes, cracks, depressions or rough areas using polymer fortified patching compounds recommended by the flooring or adhesive manufacturer compatible with existing surfaces to ensure a smooth, finished appearance. Minimum floor temperature before, during and after installation and requirements for conditioning adhesive, shall comply with the floor covering and adhesive manufacturer's instructions. Floor temperature shall be a minimum of 60° Fahrenheit for at least 24 hours prior to, during, and after installation.
3.5 Installation
3.5.1 Vacuum/Sweep clean subfloor. Apply skim coat to entire floor surface. Roll installed carpet tile with a weighted roller per manufacturer’s instructions (https://www.tandus-centiva.com/technical/installation) Modular ER3 & Flex-Aire RS.
3.5.2 Contractor shall be responsible for cutting and transporting carpets to the job site from Building 521. The cost for cutting, handling, transporting, and disposing shall be included in the overall quote.
3.5.3 Cut and fit floor covering neatly around projections through floor and to walls and other vertical surfaces.
3.5.4 Install molding/transition strips where installed flooring joins at other floor coverings. Use full-length transition pieces only. Butt tight to vertical surfaces. Where splicing cannot be avoided, butt ends tight and flush.
3.5.5 Install cove base where removed. Score interior corners and heat mold exterior corners for proper installation.
3.6 Check matching of carpet before cutting and ensure there is no visible variation between dye lots. Check for any other manufacturing defects prior to installation and notify the CEI. Do not install defective carpet.
3.6.1 Cut carpet, where required, in a manner to allow proper seam and pattern match. Ensure cuts are straight, true and un-frayed.
3.6.2 Where possible and practical, locate seams in areas of the least amount of traffic. Seams shall occur only where necessary due to carpet width. No cross seams will be accepted.
3.6.3 Join seams so as not to detract from the appearance of the floor covering installation and decrease its life expectancy. Ensure seams are straight, not overlapped or peaked, and free of gaps.
3.6.4 Lay floor covering with the run of the pile in the same direction of anticipated traffic. Do not change run of pile in any one room or from one room to next where continuous through a wall opening.
3.6.5 Do not place objects on floor coverings for a minimum of 8 hours or until adhesive is set.
4. GOVERNMENT FURNISHED PROPERTY AND SERVICES:
4.1 Utilities. All reasonable quantities of utilities shall be made available at no cost to the Contractor.
4.2 Storage. Material and equipment storage will be approved by CEI. If approved, materials shall be placed in a neat appearance, and in a manner that allows safe continued facility use.
4.3 Damage to Government Property. The Contractor shall notify the Government when damage to government property is discovered. Any damage to government property caused by the Contractor shall be repaired or replaced by the Contractor at the Contractor’s expense. The Contractor shall annotate on the inspection report, the damage, the cause of damage (if known) and a projected proposal for repair.
5. GENERAL INFORMATION:
5.1 Hours of Operation. 7:30 AM to 4:00 PM, Monday through Friday, except Federal Holidays listed in paragraph 5.2.
5.2 Federal Holidays.

5.3 Base Closures. Work scheduled, but not accomplished because of base closure due to weather, exercises, or actual alerts, shall commence as soon as possible after reopening the base, unless otherwise directed by the CO through coordination of the CEI.
5.4 Security Requirements. Contractor personnel will be limited to work areas. The CEI will identify facilities requiring additional escorts upon receipt of contract award.
5.4.1 The Contractor shall be responsible for obtaining all necessary passes, decals or other items required for access to the areas in which work will be performed. The Contractor shall request contractor passes through the Contracting Officer by providing the name, social security number, date of birth, work phone, and home phone of employees requiring passes. Personnel must adhere to the REAL ID Act of 2005 to ensure access to base is granted. Pass requests will be forwarded by the Contracting Officer to the 5th Security Forces Squadron Visitor’s Control Center for approval and issuance. Upon completion or termination of the contract, or an individual’s release from employment, the Contractor shall retrieve the identification issued above and surrender it to the 5th Security Forces Squadron Pass and Registration Section located in the Visitor Control Center. Template will be provided at contract award and due NLT 10 days after award.
5.4.2 Reporting Requirements. Contractor personnel shall report, to an appropriate authority, any information or circumstances of which they are aware may pose a threat to the security of DoD personnel, contractor personnel, resources, and classified or unclassified defense information. Contractor’s employees shall be briefed by their immediate supervisor prior to entering the base.
5.5 Notification of Environmental Spills. The Contractor or their authorized representative shall report any spills immediately by calling 723-2461 or 911 from a standard phone. If the Contractor spills or releases any substance listed in 40 CFR 302, Designation, Reportable Quantities, and Notification or general fluids into the environment. In these cases, the type of material and quantity spilled shall be reported. The Contractor shall be held liable for the spill or release of such substances, and is expected to clean up all spills of less than 25 gallons. The Contractor shall be liable for the cleanup and cost of any spill greater than 25 gallons that occurs and is determined to be contractor negligence.
5.5.1 The Contractor shall be familiar with, and follow the most current "Full Spectrum Threat Response (FSTR) Plan 10-2". The Contractor is required to have and maintain Safety Data Sheets (SDS) on all Hazardous Material (HM). The Contractor shall provide a list of all HM to the CO at least 30 days prior to entering the base. The CO will distribute the list to all appropriate agencies for approval. The list shall contain the following information: the expected to be used including a copy of the SDS, an estimate on the quantity of the HM expected to be used during the performance of the contract, and anticipated storage location for each HM intended for use.
5.6 Compliance with Laws and Regulations. The Contractor shall be knowledgeable of and comply with all applicable federal, state, and local laws, regulations, and requirements regarding environmental protection to include AFI 32-7080, Pollution Prevention Program, AFI 32-7042, Solid and Hazardous Waste Compliance, ACC Manual 32-7051, Environmental Quality Manual, 40 CFR 273, 40 CFR 133, North Dakota Administrative Code 33-24-02-06, and base Hazardous Waste Management Plan (September 2005) 5 BW-32-7043. In the event of environmental laws/regulations/requirements change during the term of the contract, the Contractor shall comply with such laws/regulations as changed. The Contractor shall comply with the base Hazardous Waste Management Plan (September 2005) when turning in wastes generated from spill cleanup.
5.7 Traffic Laws. The Contractor and its employees shall comply with base traffic regulations.
5.8 Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their contractor-owned or privately owned vehicle while on Minot Air Force Base.
5.9 For Official Use Only (FOUO). The Contractor shall comply with DoD 5400-7-R, Chapter 4, DoD Freedom of Information Act (FOIA) Program requirements. This regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding FOUO material.
5.10 Personnel. Contractor personnel shall present a neat appearance. Contractor personnel shall be easily recognizable while on the installation in conjunction with this contract. This shall be accomplished through the wear of distinctive clothing, overcoats, or hats, bearing the company name or logo. The coloring or design of the items selected should be such that identifies personnel easily and quickly for reasons of safety and personal protection.
The Government is authorized to restrict the employment under the contract of any contractor employee or prospective contractor employee, who is identified as a potential threat to the health, safety, security, general well-being, or operational mission of the installation and its population.
5.11 Fire and Emergency Reporting. Call 723-2461 or 911 from a standard phone.
6. MEASUREMENT:
6.1 Measurement. Measurements provided in Attachment II, Pricing and Attachment III, Floor Plans are estimates only. Offerors are encouraged to attend site visits for their own measurements. The Contractor is responsible for determining actual material quantities for the required work.

APPENDIX A
Executive Carpet Tile Technical Specifications:
1. Minimum Warranties:
a. Fifteen-year warranty against 10 percent loss of face fiber.
b. Fifteen-year warranty against edge raveling, snags, picks, runs, and delamination.
2. Construction:
a. Broadloom carpet will be tufted, woven, needle bonded or fusion-bonded.
3. Surface Fibers:
a. Nylon: Fiber manufacturer continuous filament nylon 6, 6 or nylon 6 with a cross-section modification ratio no greater than 2.5.
4. Pile Height:
a. No higher than ½ inch, measured to the backing
5. Average Pile Yarn Density (APYD):
a. Must exceed 4,000
6. Tuft Bind:
a. Must exceed 10 pounds
7. Backing:
a. For tufted carpet, primary and secondary backings (or unitary backing) must be synthetic material. Jute backings are not authorized.
b. Use of attached cushion back is optional. Open-cell urethane cushion or closed-cell vinyl backings are authorized.
c. Carpet tile shall be peel and stick.
8. Colorfastness:
a. Colorfastness to light (fading) will comply with AATCC TM 16, Colorfastness to
Light, and will have a minimum of 4 after 40 AATCC fading units using the AATCC gray scale for color change.
9. Color
a. Carpet tile that contains a pattern or bold, multicolored tweed, except as noted in paragraphs 9.3.6.1 and 9.3.6.2 of the AF ETL 07-04. Tone-on-tone coloration does not meet criteria as a multi-colored carpet - the pattern acts as a camouflage to mask spots and spills until they can be cleaned. A random multicolored pattern performs better than geometric patterns or tweed. The pattern must cover a majority of the carpet surface. Avoid patterns with definite lines or a design that repeats in a distinctively linear pattern (vertical or horizontal) in corridors, small rooms, or areas with angled walls. These designs are difficult to install parallel to walls.
10. Electrostatic Propensity (Maximum):
a. 2.0 kilovolts or less
11. Fire Resistance:
a. Carpet must conform to CFR Part 1630, Standard for the Surface Flammability of Carpets and Rugs (FF 1-70) (Pill Test).
b. Minimum average critical radiant flux of 0.22 watt per square centimeter or greater.
c. Smoke density, when necessary to meet specific application, shall be tested in accordance with ASTM E662 and shall have a maximum specific optical density not exceeding 450 (flaming mode exposure).
12. Appearance Retention Rating (ARR):
a. Severe: Minimum of 3.5 – 4.0 ARR
13. Stain Resistance:
a. Minimum rating of 8 using the AATCC red 40 stain scale after 2 HWE (hot water extraction) using the AATCC 171, Carpets: Cleaning of; Hot Water Extraction Method, test method followed by the AATCC 175, Stain Resistance: Pike Floor Coverings, stain test.
14. Indoor Air Quality:
a. Certified as CRI “Green Label” or “Green Label Plus.”
