The following have been modified:

ADDENDUM TO 52.212-1

ADDENDUM TO FAR PROVISION 52.212-1, INSTRUCTIONS TO OFFERORS--COMMERCIAL ITEMS

(INCORPORATED BY REFERENCE ON STANDARD FORM 1449)

I. PROPOSAL PREPARATION INSTRUCTIONS - Addendums made to FAR 52.212-1:

A. FAR 52.212-1, paragraph (b)(8), is changed to read:

(8) A completed copy of the representations and certifications at FAR 52.212-3 and DFARS 252.212-

7000.

B. FAR 52.212-1, paragraph (b)(10), is changed to read:

(10) Past performance information, to include recent and relevant contracts for the same or similar items within the past five years but limited to the most recent 3 to be filled in on Attachment 3 -

Responsibility Questionnaire (including contract numbers, points of contact with telephone numbers and other relevant information).

C. FAR 52.212-1, paragraph (c), is changed to read:

(c) Period for acceptance of offers. The offeror agrees to hold the prices in its offer firm through 30

Sep 2013.

II. SPECIFIC INSTRUCTIONS:

A. TECHNICAL PROPOSAL: All proposal preparation costs will be the sole responsibility of the

Offeror. The Government will not reimburse any firm for their proposal preparation costs.

1. In order to be considered technically acceptable, the offeror must provide the following documentation:

i. Personnel Certification: The offeror shall submit personnel training/certification documents for prospective employees, which validate personnel qualifications to support all requirements outlined in the Performance Work Statement (PWS). Some of these documents may include:

personal training records which indicate tasks qualification/certification or an individual

Federal Aviation Administration Airframe and Power Plant License.

ii. Quality Control Plan: The offeror shall submit a quality control plan that addresses all requirements of the PWS which includes foreign object damage (FOD) preventative methods and a tool accountability control system. The plan must also address methods to be used for identifying and preventing defects before the performance becomes unacceptable. The plan shall also include procedures to identify unfavorable performance trends, problem areas, and the corrective actions that would be taken.

iii. Management/Manpower: The offeror shall provide a management staffing approach that adequately provides personnel qualified to perform the services as required by the PWS. This should include retention of personnel, organization structure, and related commercial and/or government experience supporting transient aircraft maintenance services. The offeror shall submit a realistic manpower schedule that adequately depicts personnel by labor/skills to include a hiring and retention objective enabling contract performance at Minot AFB, ND.

iv. Technical Proposal required information is limited to ten (10) pages. A page shall be considered an 8-1/2” x 11” sheet of paper. Two pages may be printed on one sheet of paper, but will count as two (2) pages. Any information / pages / documents / certifications / attachments / plans beyond the ten page maximum will not be considered (excluding cover page/table of contents). The type size will not be smaller than Microsoft Word Times New

Roman 10 point font, normal proportional spacing. Text lines will be single-spaced.

Company name shall be indicated on the upper right hand corner of each page.

B. PAST PERFORMANCE: As part of the evaluation process, the Government will assess each offeror’s past performance deemed relevant to the requirements of this solicitation. The offeror shall present factual material dealing with contracts held with other Government agencies or with private sector businesses on which the same or similar services were provided as the offeror will reflect in

Attachment 3. The completed Attachment 3 must be submitted with the proposal no later than the offer due date and time indicated on the Standard Form 1449, Block 8. Offerors are cautioned that the

Government will use information provided by the offerors and information obtained from other sources in the development of the performance assessments.

1. For past performance, the offeror must have a record showing they are both recent and relevant:

i. Recency: Have successfully completed transient aircraft services for a minimum of three (3) years. Recent past performance is defined as occurring within the last five (5) years. Offeror shall complete Attachment 3 and submit with offer.

ii. Relevancy: Having successfully completed transient aircraft services. Relevant services are defined as the offeror being actively engaged as a contractor, either prime or subcontractor, performing transient aircraft services as defined by the North American Industry

Classification System Code (NAICS Code) 488190 with a minimum value of $125K per year.

C. PRICE PROPOSAL: Complete the “Signature of Offeror/Contractor” part of the Standard Form 1449 in Blocks 30a, 30b, and 30c. An authorized official of the firm must sign the Standard Form 1449. In doing so, the Offeror accedes to the contract terms and conditions as written in the RFP.

1. Provide complete pricing for all items in Section B of the SF 1449– the Schedule of

Supplies/Services by entering in prices for each Contract Line Item (CLIN), including all option years.

2. Acknowledge and sign all amendments.

III. GENERAL INFORMATION:

A. This solicitation is set-aside for service-disabled veteran-owned small businesses only.

B. INFORMATION REGARDING SUBMISSION OF PROPOSALS: Hand carried or mailed proposals

(USPS, FedEx, UPS, DHL, etc.) must be delivered to 5 CONS/LGCS Mon-Fri between the hours of

0730-1630 local time, Attn: Scott Stauffer, 165 Missile Ave, Minot AFB, ND 58705. E-mailed proposals can be sent to scott.stauffer@us.af.mil. The sealed envelope, e-mail, or package used to submit your proposal must show the time and date for receipt, solicitation number, and name and address of the offeror and must be received no later than the offeror due date.

ADDENDUM TO 52.212-2

ADDENDUM TO FAR PROVISION 52.212-2 EVALUATION--COMMERCIAL ITEMS (JAN 1999)

I. Basis For Contract Award: The Government will award a contract resulting from this solicitation to the responsible service-disabled veteran-owned small business offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:

A. The award will be made to the Lowest Priced Technically Acceptable Offeror meeting the acceptability standards for non-cost factors list below in section II and III. Award is expected to result from selection of the technically acceptable proposal with the lowest evaluated price. Technical and past performance factors will be judged acceptable or unacceptable. Award will be made on the basis of the lowest evaluated price of proposals meeting the acceptability standards for non-cost factors.

B. The Government intends to award one contract to the offeror deemed responsible in accordance with

Federal Acquisition Regulation (FAR), as supplemented, whose proposal conforms to the Request for

Proposals (RFP) requirements including all stated terms, conditions, representations, certifications, and all other information required by the RFP and is judged, based on the evaluation criteria.

II. Technical Acceptance Criteria - Technical acceptance will be evaluated on an acceptable/unacceptable basis.

One of the following ratings will be assigned:

Acceptable Proposal clearly meets the minimum requirements of the solicitation

Unacceptable Proposal does not clearly meet the minimum requirements of the solicitation.

A. Only proposals rated technically acceptable, either initially or after exchanges with offerors (see FAR

15.305) will be considered for further evaluation.

B. A proposal found deficient will be rated unacceptable. Reasons for unacceptable include: Proposal fails to adequately respond to or satisfy the requirements stated in the RFP, proposal presents an unacceptable risk to the Government, etc.

C. In order to be considered technically acceptable, the offeror must provide the following documentation:

1. III. Past Performance Criteria – An offeror’s past performance must be both recent and relevant as described below. Past performance will be rated according to the following table:

Acceptable Based on the offeror’s performance record, the Government has a reasonable expectation that the offeror will successfully perform the required effort, or the offeror’s performance record is unknown.

Unacceptable Based on the offeror’s performance record, the Government has no reasonable expectation that the offeror will be able to successfully perform the required effort.

A. For past performance, the offeror must have a record showing they are both recent and relevant:

1. Recency: Have successfully completed transient aircraft services for a minimum of three (3) years.

Recent past performance is defined as occurring within the last five (5) years. Offeror shall complete

Attachment 3 and submit with offer.

2. Relevancy: Having successfully completed transient aircraft services. Relevant services are defined as the offeror being actively engaged as a contractor, either prime or subcontractor, performing transient aircraft services as defined by the North American Industry Classification System Code (NAICS Code)

488190 with a minimum value of $125K per year.

B. In the case of an offeror without a record of relevant past performance or for whom information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the offeror may not be evaluated favorably or unfavorably on past performance (see FAR 15.305

(a)(2)(iv)). Therefore, the offeror shall be determined to have unknown past performance. In the context of acceptability/unacceptability, “unknown” shall be considered “acceptable”.

IV. Price: The Offeror’s price proposal will be as follows:

A. An Offeror’s proposed prices will be determined by multiplying the quantities in the SF 1449 line items by the proposed unit price for each Contract Line Item Number to confirm the extended amount for each. Unit prices will prevail. The price evaluation will document the completeness and reasonableness of the proposed total price for the offers evaluated. Offerors are specifically advised that under this evaluation method, the Government may determine an offer unacceptable if the prices proposed are materially unbalanced between line items and sub-line items. Prices are materially unbalanced if they are significantly overstated or understated in relation to the actual cost of that work. Refer to “Local Area” factors when formulating overall pricing as it relates to local economic conditions. The price will also be evaluated to determine the Offeror’s understanding of the contract requirements as expressed by the solicitation.

B. The Government will evaluate offers for award purposes by adding the total price for all options and option years to the total price for the basic requirement. The Option to Extend Services Clause 52.217-8 will also be evaluated for the Offerors proposed prices. The 52.217-8 prices will be calculated at the final option prices. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced or unrealistic. Evaluation of options shall not obligate the Government to exercise the option(s).

C. The Government will use various price analysis techniques and procedures to ensure a fair and reasonable price. It is expected that the competitive nature of this acquisition will drive price reasonableness and fairness; however, the Government reserves the right, if it deems appropriate, to make these determinations by means of one or more price analysis techniques.

D. The Contracting Officer has determined there is a probability of adequate price competition in this acquisition. Upon examination of the initial offers, the Contracting Officer will review this determination and if, in the Contracting Officer’s opinion, adequate price competition exists, no additional cost/price information will be requested. However, if at any time during this competition the Contracting Officer determines that adequate price competition no longer exists, offerors may be required to submit information to the extent necessary for the Contracting Officer to determine the reasonableness of price.

E. The Government may evaluate proposals and make award without discussions (except clarifications as described in FAR 15.306(a)). Therefore, the offeror’s initial proposal should contain the offeror’s best terms from a technical, performance, and price standpoint.

V. Late Submissions, Debriefings, and Notice of Award:

A. Hand carried or mailed proposals (USPS, FedEx, UPS, DHL, etc.) must be delivered to 5 CONS/LGCS

B. Debriefings:

1. Pre-award Debriefing of Offerors:

i. Offerors excluded from the competition before award may request a debriefing, before award (10

U.S.C. 253(b)(6)(A) and 41 U.S.C. 253(b)(f-h)). The Offeror may request a pre-award debriefing by submitting a written request for debriefing to the Contracting Officer within 3 days after receipt of the notice of exclusion from the competition.

2. Post-award Debriefing of Offerors:

i. An Offeror, upon its written request received by the agency within 3 days after the date on which that Offeror has received notification of contract award in accordance with 15.503(b), shall be debriefed and furnished the basis for the selection decision and contract award.

C. Notice of Award:

1. A written notice of award or acceptance of any offer (a copy of the Standard Form 1449 signed by the

Contracting Officer), mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either mailto:scott.stauffer@us.af.mil party. Before the offer’s specified expiration time, the Government may accept an offer, whether or not there are negotiations after its receipt, unless written notice of withdrawal is received before award.
