PERFORMANCE WORK

STATEMENT

FOR

CUSTODIAL S E R V I C E S

At

87TH MDG Treatment Facilities

Revision 3 14 April 2016

1. DESCRIPTION OF SERVICES. Contractor shall provide custodial services for the 87th Medical Group at Joint Base McGuire-Dix-Lakehurst (JB MDL). Contractor shall perform the services in accordance with the terms and requirements specified in this Performance Work Statement (PWS) and the professional standards of the Accreditation Association for Ambulatory Health Care (AAAHC) and American Hospital Association Standards (AHAS).

1.1 BASIC CLEANING SERVICES. Contractor shall accomplish all cleaning tasks specified below and those necessary to be in accordance with the American Hospital Association Standards. Contractor will be responsible for fulfilling the services in the following type of areas:

1.2. MAINTAIN FLOORS. All floors, except carpeted areas, shall have a uniform glossy appearance. All floor maintenance solutions shall be removed from baseboards, furniture, trash receptacles, and any other areas. Chairs, trash receptacles, and other moveable items shall be moved while floors are being cleaned to ensure all areas of the floor are being reached. Following the completion of cleaning the floor, all items must be returned to their original and proper position. Carpeted areas, including any throw rugs and floor mats, shall be vacuumed ensuring any carpets are free of all visible dirt, debris, litter and other foreign matter. All tears, burns, and raveling shall be brought to the attention of the Facilities Manager. Floor mats must also be cleaned as necessary to ensure they are free of soil and returned to their original location following any such cleaning.

1.3 REMOVE TRASH. All trash containers shall be emptied/cleaned and returned to their initial location daily (Monday through Friday). Trash includes all solid waste to be discarded except for recyclable materials. If Contractor generates recyclable materials during the course of performance, i.e. cardboard, Contractor shall use the appropriate recycling containers for disposal. Contractor shall replace any obviously soiled or torn plastic trash receptacle liners when emptying a receptacle. Dumpsters are located near the l o a d i n g dock area of each building for discarding of trash.

1.3.1 POLICING. Contractor will police for trash fifteen (15) feet in each direction of the bulk refuse containers, entrances, smoking area, pavilion, walkways and loading dock areas as these areas are to be free of trash daily. Contractor shall empty and clean all smoking receptacles located outside.

1.4 CLEAN DRINKING FOUNTAINS. Contractor shall clean and disinfect all porcelain

Level I Clinical Areas: Areas where patient care is delivered or patient treatment is delivered and require the removal of all dirt of any kind, including, bacteria and viruses in accordance with Association for Professionals in Infection Control and Epidemiology (APIC) specifications and standards for low level disinfection.

Level II High Use: These areas are administrative in nature and require regularly scheduled cleaning of surfaces regardless of whether dirt is visible. These areas are equivalent to Building Owners and Managers Associations (BOMA) identification of a combination of Executive and Basic Services.

1.5 CLEAN STAIRWAYS. All stairway surfaces shall be cleaned as specified in the paragraph 1.2 above relating to the cleaning and maintaining of floors. Grease and grime shall be removed from stair guards, handrails and baseboards. Contractor shall remove all marks, dirt, smudges, scuffs, and other foreign matter from adjoining stairwell walls to provide a clean uniform appearance at all times.

1.6 GENERAL SPOT CLEANING. Contractor will also perform spot cleaning on a continual basis. Spot Cleaning includes, but is not limited to, removing or cleaning smudges, fingerprints, marks, streaks, spills, or other similar non-clean appearances from all washable surfaces of all walls, partitions, vents, grillwork, doors, door guards, door handles, push-bars, kick-plates, light switches, temperature controls, and fixtures. After spot cleaning, the surfaces shall have a clean, uniform appearance, free of streaks, and other evidence of soil.

1.7 GENERAL DUSTING. Contractor shall perform general dusting in all administrative and common areas, meaning all horizontal surfaces must be dusted or cleaned to eliminate dust collection. General dusting shall be performed as often as needed to maintain a clean, uniform, and reasonably dust free environment.

1.7.1 Patient Care Areas shall be dusted daily.

1.8 BULB/TUBE REPLACEMENT. Contractor shall replace all standard fluorescent light tubes when burned out, flickering, improperly working, or when notified by Facility Manager or designee. Color rendering index (CRI) bulbs will be used only in dental Clinic treatment rooms and dental laboratory rooms unless otherwise instructed by the government.

Immediately upon removal, all fluorescent tubes shall be placed into a lidded cardboard box for disposal. The box must be labeled “Used Lamp(s)” and the date the first bulb was placed into the box must be clearly marked on the outside of the box. New bulbs/tubes shall not be stored together in the same box. The insertion pins may be bent or broken off in order to distinguish the old lamps from the new lamps. Used lamps must be brought to the Recycling Center, 2321 Radin Road, for disposal as needed, but no less than quarterly.

1.9 BASIC RESTROOM/LOCKER ROOM CLEANING SERVICES. Contractor

shall perform all cleaning tasks, as specified in this PWS and in the American Hospital Association Standards, in any restrooms or locker rooms in the Buildings specified herein.

1.10 CLEAN AND DISINFECT. Contractor shall completely clean and disinfect all surfaces of sinks, toilet bowls, urinals, lavatories, showers, shower mats, dispensers, plumbing fixtures, saunas, partitions, doors, walls and other such surfaces, using a germicidal detergent.

Receptacles shall be free of deposits, dirt streaks and odors following cleaning. Contractor will also disinfect all surfaces of partitions, stalls, stall doors, entry doors, (including handles, kick plates, ventilation grates, metal guards, etc.) and wall areas adjacent to wall mounted lavatories, urinals and toilets.

1.11 SWEEP AND MOP BATHROOM/LOCKER ROOM FLOORS. Contractor will ensure, after sweeping and mopping, the entire floor surface, including grout, shall be free from litter, dirt, dust and debris. Contractor will assure all grout on wall and floor tiles is free of dirt, scum, mildew, residue, etc. Floors shall have a uniform appearance without streaks, swirl marks, detergent residue, or any evidence of soil, stain, and film or standing water.

1.12 STOCK RESTROOM SUPPLIES. Contractor shall ensure restrooms are stocked sufficiently such that supplies do not run out prior to the next provision of service.

1.13 LINEN SERVICE. Contractor shall provide linen distribution service. Contractor will submit, in writing, a monthly linen inventory report. Contractor will ensure the completion of the following with respect to linens:

1.13.1 Clean linen shall be received directly from the laundry contractor by the housekeeping staff. After government acceptance, the housekeeping staff will weigh, log and store the linen in the central clean linen area in accordance with the facilities infection control guidelines.

1.13.2 Sufficient linen (including a small reserve) shall be maintained in the using areas to support normal usage until next scheduled distribution. Depleted linen levels shall be replenished on an on-call basis due to a medical emergency.

1.13.3 Housekeeping personnel shall verify the quantities of items received from the Linen contractor to be consistent with delivery tickets. Delivery tickets shall be forwarded to MTF Linen Contract Facility Manager or designee for acceptance, and shall include a separate list of all discrepancies noted. Housekeeping staff will then store the accepted linen in the designated location for clean linen.

1.13.4 All new linen purchased by the government shall be immediately marked with “87th

Medical Group” stamps provided by the government. Linen items with illegible identifying marks shall be remarked before placing back in use.

1.14 Curtains will be removed, laundered, and replaced semi-annually. Laundering will occur in April and October unless a change in schedule is agreed upon by the Facility Manager or designee and contractor.

1.15 PERIODIC CLEANING SERVICES. Contractor will perform these services in accordance with the AHA standards and current Association for Professionals in Infection Control and Epidemiology (APIC). www.apic.org / www.aha.org http://www.apic.org/

1.16 STRIP, SCRUB, SEAL, AND WAX FLOORS. Contractor will strip, scrub, seal and wax floors as necessary to maintain a uniform glossy appearance. Contractor will utilize only a non-skid wax. A uniform glossy appearance means the floor is free of heel marks, scuff marks, wax build up, and other stains and discoloration. The services explained in this paragraph shall be accomplished as often as necessary to maintain sanitary conditions and a clean, uniform appearance. The blue rubber flooring in the pharmacy in bldg. 3458 and the brown flooring in the dental clinic bldg. 2417 are non-wax floors.

1.17 CLEAN INTERIOR & EXTERIOR WINDOWS. Window screens in the buildings shall be removed, cleaned, and replaced annually. Contractor will assure that after cleaning windows that all frames, casings, sills, and glass are free of all traces of film, dirt, smudges, water and other foreign matter.

1.18 CLEAN/SHAMPOO CARPETS. Contractor will spot clean all carpets as needed and shampoo all carpets at least once per year. Large spots (defined as any spot larger than one (1) square foot) on any carpets, throw rugs, or floor mats shall be cleaned and removed immediately. Vacuum cleaners should be maintained to minimize dust dispersal in general, and be equipped with high-efficiency particulate air filters. All carpets shall be cleaned in accordance with health-care facility practices. A heavy-duty spot remover may be required in heavily soiled areas. After shampooing, the carpeted area will be uniform in appearance and free of stains and discoloration. All cleaning solutions shall be removed from baseboards, furniture, trash receptacles, chairs and other similar items. All carpeted areas underneath chairs, trash receptacles, and other items shall be cleaned, and all items shall be returned to their original location after carpeted areas are cleaned.

1.19 LIGHTS. Contractor shall clean all ceiling lights, emergency exit lights, patio lights, outside entrance and loading dock lights, interior lighted signs, canopy lights, and all other lights (except desk and table lights). All fixtures shall be free of bugs, dirt, dust, grease, and other matter.

1.20 INTERIOR DUCT CLEANING. Contractor will remove, clean, disinfect, and replace the removable portions of registers, diffusers, and grills. Duct interiors shall be cleaned up to two feet inside the register, and sprayed with a phenolic disinfectant.

1.21 REGULATED MEDICAL WASTE. All medical waste shall be handled and properly documented in accordance with all applicable Federal and State Laws including, but not limited to, the New Jersey Comprehensive Regulated Medical Waste Management Act (CRMWMA, N.J.S.A. 13:48.1 to 48.28) and any regulations adopted pursuant to CRMAWMA, SOLID WASTE REGULATIONS N.J.A.C. 7:26 Subchapter 3A. Regulated Medical Wastes. Contractor shall be responsible and liable for all penalties and/or fines that may be required or assessed by any State, Federal or Local Governments resulting from Contractor’s acts or omissions.

1.21.1 All regulated medical waste shall be placed in appropriate containers by MTF personnel. Regulated medical waste shall be collected daily by Contractor personnel in a transport cart separate from general refuse. Waste holding areas must be kept secure.

1.21.2 Contractor personnel shall package regulated medical waste in provided medical waste containers. Containers shall be sealed to prevent leakage and handled in a manner that will prevent bursting or tearing during transport. Sharp containers shall also be placed in similar containers. Liquid containers shall be placed in break-resistant and tightly sealed packages.

All exterior containers used shall have the words "Medical Waste" or "Infectious Waste" displayed or display the biohazard symbol. Bags used as container liners need not be labeled.

All exterior containers shall be marked with the generator's name, permit number, and transporter's address. NOTE: Transportation and final disposition of medical waste will be performed under a separate contract.

1.22 CLEANING PRODUCTS AND HAZARDOUS MATERIALS. Any hazardous material or chemical to be used or stored by Contractor must be approved, in writing, by the Infection Control Officer prior to Contractor bringing the material on JB MDL.

1.23 . Cleaning products are considered to be hazardous materials. Contractor must utilize an AF Form 3000, Material Approval Submittal, in order to gain approval to bring hazardous material on the base, which will list each container of hazardous material stored, kept, or utilized, including applicable Safety Data Sheets (SDS). SDS sheets must also be displayed on-site for access by Contractor employees and the Government, in accordance with Occupational Health Administration (OSHA requirements). Contractor must maintain in a binder at each building to include, current copies of SDSs for every cleaning product utilized or stored at each facility. Contractor is required to track and record the quantities of every product used on a monthly basis and provide a monthly report to the Functional Requirements Evaluator Designee. (FRED) Excess materials must be properly handled and disposed of by the Contractor at Contractor’s expense.

1.23.1 ENVIRONMENTAL COMPLIANCE: Contractor is required to comply with all applicable State, Federal, and local laws and regulations, including any Air Force Instructions, applicable environmental laws and occupational health and safety regulations. Contractor shall be wholly responsible for any penalties, fines, or claims of any kind assessed by any federal, state or local government entity resulting from the acts or omissions of the Contractor. The Government may inspect for environmental compliance at their discretion, and will notify the CO of any discrepancies found. In addition, the Contractor may be subject to contractual sanctions for non-compliance with law including, but not limited to, withholding, offset, termination, suspension, debarment and adverse past performance assessment.

1.23.2 NOTIFICATION OF ENVIRONMENTAL SPILLS: In the event Contractor spills or releases any unpermitted substance, hazardous substance or hazardous waste into the environment, Contractor shall immediately report the incident to the Fire Department, CO, and the designated Government representative. Contractor shall be liable for any penalties, fines, or claims of any kind, including but not limited to the costs of clean up and remediation, resulting from any spills or releases of any substances into the environment during the course of performance under this agreement.

3. GOVERNMENT FURNISHED FACILITIES, AND SERVICES:

3.1 GENERAL. The Government will provide the following equipment, supplies, and services listed below:

3.2 EQUIPMENT. Equipment provided by the government will include linen marking equipment, linen carts, and protective equipment/clothing, including resistant gowns, masks, eye protection, shoe/hair covers and gloves worn by Contractor employees as required by the Medical Treatment Facility (MTF). Government will also provide light bulbs/tubes and antiseptic soap for soap dispensers.

3.3 FORMS. Any forms required are publicly available at e-pubs (http://www.e-publishing.af.mil/).

3.4 UTILITIES. The Government will provide reasonable use of electricity, water, sewage, air conditioning, and heating for office operations.

3.5 TELEPHONE. The Government will provide telephone service to be used to perform contract duties only.

3.6 INSTALLATION DISTRIBUTION. The Government will provide a designated distribution box for Contractor to receive mail.

3.7 OFFICE SPACE. Contractor shall be provided with office space in the 87th Medical Group, building 3458, located on JB MDL, New Jersey. The total office space is 118 square feet. The total amount of square footage (includes office, lounge, and equipment storage) for use by the contractor is 349. Furnishings will be provided in the office space to include computers, furnishing, and associated office equipment.

4. CONTRACTOR FURNISHED ITEMS AND SERVICES

4.1. GENERAL. Except for those items or services specifically stated in the PWS, the Contractor must provide all cleaning supplies, equipment, materials and labor required to perform this contract in accordance with all of its terms.

5. GENERAL INFORMATION

5.1 POINT OF CONTACT. Contractor shall provide a point of contact responsible for the performance of the work. The point of contact shall have full authority to act for the Contractor on all matters relating to the daily operation of this contract. Contractor shall designate this individual, in writing, to the Contracting Officer (CO) before the contract start date. An alternate may be designated, but the Contractor shall identify those times when the alternate shall be the primary point of contact.

5.2 ARRANGE FOR REPLACEMENT PERSONNEL. The point of contact shall arrange for personnel coverage to ensure uninterrupted service.

5.3 STAFFING. Trained staff shall be required to meet the MTF performance standards and coverage for all buildings.

5.4 CONTINUITY OF SERVICES. If routine services are disrupted for more than three (3) consecutive scheduled shifts, and not result of a Government work stoppage, the Government reserves the right to procure such services from another source until routine services are restored by the Contractor. When the Government exercises its right to procure these services from another source, the Government reserves the right to reduce the Contractor's invoice consistent with any amounts paid for third party services as a result of Contractor’s failure to perform. A copy of the third party supplier’s service ticket will be used as the basis for any reduction.

5.5 PERSONNEL REQUIREMENTS. Contractor personnel shall present a neat, clean, and professional appearance and be easily recognized as Contractor employees. Identification badges will be supplied by the MTF and are to be visibly displayed by Contractor employees at all times within the MTF. Shorts, halter tops, swim wear, clothing with discriminatory or sexually suggestive words, phrases, or images, see-through or excessively revealing clothing, and clothing in obvious disrepair are not considered appropriate attire. Hair shall be neat and well groomed. Contractor employees shall have no visible piercings, other than those in ears, unless said piercing is part of the practice of an established religion. Personnel in supervisory position will have computer access in order to maintain and distribute electronic reports required by MTF functions and committees. Supervisory personnel must be on duty from 0700-1600 with a 60 minute lunch break to be taken approximately during the middle of the shift.

5.6 ENGLISH LANGUAGE REQUIREMENT. Since communication with the MTF personnel is necessary for determining the work to be performed and specifically how the work needs to be completed, personnel shall be able to understand English well enough to effectively communicate.

5.7 CONFLICT OF INTEREST. Contractor shall not employ any person who is an employee of the United States Government if the employment of that person would create a conflict of interest. Contractor shall not employ any person who is an employee of the Department of the Air Force unless such person seeks and receives approval in accordance with DoD Directive 5500.7 and Air Force policies.

5.8 COMMUNICATION. Contractor shall ensure its personnel maintain open and professional communication with members of the MTF. Complaints must be validated by the Contracting Officer Representative (COR) or designee. Failure of the Contractor to correct validated complaints raised by the MTF staff and the CO will be considered a failure to perform.

5.9 EDUCATION AND TRAINING REQUIREMENTS. Contractor shall provide an on-going employee developmental training program to ensure that all employees perform to the standards set forth in the PWS, the American Hospital Association Standards, OSHA, HIPPA and the contractor's quality assurance plan. Contractor must review with the FRED, training documentation on an annual basis for each personnel employed at the MTF.

5.10 SECURITY REQUIREMENTS

5.10.1 See Security Forces Appendix for requirements for obtaining access to Joint Base McGuire-Dix-Lakehurst. All costs incurred for the security and background checks in appendix E will be the responsibility of the contractor.

5.10.2 Each employee shall wear appropriate identification badges (provided by the MTF) on the outer clothing on the front of the body between the neck and waist so that the badge is visible at all times.

5.10.3 Contractor shall establish and implement methods of ensuring that all keys/access controls issued to Contractor by the Government are not lost or misplaced and are not used by unauthorized persons. Contractor shall not duplicate any Government keys/access controls for any reason. Contractor shall notify the FACILITY MANAGER OR DESIGNEE immediately in the event of a lost key/access control.

5.11 LOSS OF GOVERNMENT FURNISHED KEY/ACCESS CONTROL: In the event Contractor loses or misplaces any Government key/access control, the Government reserves the option to have Contractor pay to have the affected lock or locks re-keyed at Contractor's expense.

5.11.1 Contractor shall limit access to locked areas to only those employees performing in those specific areas.

5.12 SECURITY OF PROTECTED HEALTH INFORMATION

5.12.1 Contractor agrees to implement administrative, physical, and technical safeguards that reasonably and appropriately protect the confidentiality, integrity and availability of the Electronic Protected Health Information that it comes in contact with.

5.12.2 Contractor agrees to ensure that any agent, including a subcontractor, to whom it provides Electronic Protected Health Information that it creates, receives, maintains, or transmits on behalf of the Government, agrees to the same restrictions and conditions that apply through this Contract to the Contractor with respect to such information.

5.13 HEALTH REQUIREMENTS.

5.13.1. Contract personnel providing services under this contract shall receive a pre-employment physical examination prior to commencement of work and annually thereafter.

Contract employees shall report to the Contractor’s physician to receive a pre-employment examination and immunizations/shots prescribed by the MTF.

5.13.2 Not later than five (5) working days prior to commencement of work, certification shall be provided to the contracting officer’s representative that contract employees have completed medical evaluation required above. This certification shall state the date on which the examination was completed, the doctor’s name who performed the examination, and a statement concerning the physical health of the individual. The certification shall also contain the following statement: “(name of contract employee) is suffering from no contagious diseases to include but not limited to Tuberculosis, and Hepatitis.”

5.13.3 Also, as a condition of employment, OSHA requires that all contract personnel who will have occupational exposure to blood or body fluids, or other potentially infectious materials, shall receive Hepatitis B vaccine, sign a voluntary declination, or have documented proof of immunity to Hepatitis B infection. Personnel who sign declinations may change their minds at any time and receive the Hepatitis B vaccine without penalty. In addition, contract personnel must provide proof of MMR immunization or proof of immunity to MMR, and a history of varicella or proof of immunity.

5.14 EMERGENCY CARE. The MTF may provide emergency health care to contract personnel for injuries occurring while on duty in the MTF. These services will be billed to the Contractor at the current full reimbursement rate. For emergency police or fire department needs, JB MDL is on the 911 system.

5.14.1 SAFETY AND ACCIDENT REPORTING. The Contractor will submit a copy of its written Safety plan for 87th ABW Safety Office review. The Contractor will provide the name and contact information for its onsite safety representative. The Contractor is responsible for providing services under this contract IAW all applicable local, State, and Federal Occupational Safety and Health requirements. The contractor shall ensure that all services are conducted safely at all times. The Contractor shall use mechanically sound and safe vehicles and equipment in the performance of services under this contract. The Contractor shall take necessary corrective actions if the Contractor’s employees are conducting operations in an unsafe manner. In the event of an on-the-job accident involving Contractor employee(s), the Contractor shall contact the base emergency 911 service and will notify MTF Facility Manager/FRED immediately. The Contractor is responsible for correcting hazards its employees, and/or operations cause, and must report those hazards to the Facility manager/FRED. Contractor employees may report hazards directly to the Facility manager/FRED, or the 87th ABW Safety Office. See the 87 ABW Safety Guide for Civilian Contractors attached for additional information.

5.15 HOURS AND LOCATION OF OPERATION. The contractor will provide required services under this contract during the following hours at the specified location:

Day Shift: 0700-1600, Monday through Friday (Bldg. 3458, 3457, 2417, 3418) Night Shift: 1630-2130, Monday through Friday (Bldg. 3458, 2417 and 3259)

5.16. FEDERAL HOLIDAYS. The Contractor will not be required to perform services on the following government observed holidays. If the holiday falls on a Saturday, it will be observed on Friday. If the holiday falls on Sunday, it will be observed on Monday. The following government days are observed:

5.17 MEDICAL QUALITY ASSURANCE/RISK MANAGEMENT (QA/RM).

Contractor employees shall participate in the QA/RM monthly activities as much as 1 hour as required by the MTF QA/RM plans and regulations. Nothing in this paragraph precludes the Government from also conducting inspections under the Inspection of Services clause.

5.18 ORIENTATION. The Contractor shall ensure that all contract employees participate in the MTF orientation procedures to include MTF regulations and Air Force policies and procedures.
