SF 330 PART I Section H Item # 5
A-E Firm Questionnaire
1. When verified A-E design deficiencies are discovered during the proposal process or the construction phase, the government will notify you by phone or email. Please describe the process and timeliness that your firm will use to provide resolution to the government.
2. The government usually requires a fee proposal from you in as little as five days which may include weekends and may require a pre-proposal site visit. Can you respond within these time limitations?
3. The government often requires design cycles of as little as seven months. This will require designs completed with submittals at 30%, 65%, 95%, and 100% with 5-20 days for government reviews at each submittal stage. Can your staff support the schedule for a typical remodel project with an estimated one million dollar construction cost limit? Has your firm ever failed to deliver an acceptable finished product within the contract performance period? If so, please explain the circumstances and the procedures/processes you have in place to ensure timely submittal on future projects.
4. Please describe the procedures your firm employs to review both internal and subcontracted work for completeness, quality and compatibility between disciplines.
What is the typical timeline for this process?
5. What CAD software package does your firm most often utilize for production of design drawings? Please describe your standard drafting methods, including use of model space/paper space, scaling, layer naming, plotting configuration, etc. Please describe how your methods all for flexibility to comply with LRAFB standards.
6. Please describe the methods your firm employs to produce architectural renderings, fly arounds, walk-throughs and similar products.
7. Please discuss the qualifications of the personnel and the procedures your firm uses to prepare construction cost estimates during the design process. How does this process ensure that design cost estimates reflect actual, current construction costs (both materials and labor) for the Little Rock area?

8. Many of our projects require little or no architectural support. Such projects include airfield and road pavements, aircraft fuel storage and piping systems, and other utilities and systems such as gas, electric, sanitary sewer and storm drainage. What is your firms experience with these types of projects? What is the experience level and availability of the designs professionals from the appropriate disciplines that would be in responsible charge of the design of these projects?
9. Some projects such as building additions require intensive architectural, mechanical and electrical support. In these situations how does your firm typically handle civil design?
10. If your team does not include specialist such as a Landscape Architect, Airfield Paving
Engineer, Fire Protection Engineer or Registered Communications Systems Designer, how will you address projects that need these capabilities?
11. During the delivery order proposal stage for an individual project, how will you insure that all design work necessary to prepare a complete and usable project will be included in the proposal?
12. What is your status as a small A-E firm or joint venture?
