MEMORANDUM FOR: SIRSIDYNIX

SUBJECT: Request for Proposal (RFP) FA4452-15-R-0023, Integrated Library Management System (ILMS) maintenance and support services.
1. This is an RFP for hosting, licensing, personnel, tools, equipment, materials, software, and supervision to maintain and support current ILMS software, SYMPHONY, throughout 9 AMC libraries. This requirement is a follow-on contract to FA4452-11-C-0006. This requirement is being solicited as a sole source to SIRSI Corporation, doing business as, SirsiDynix located at 3300 N Ashton Blvd Suite 500 Lehi, UT, 84043-0000. Based on proprietary rights and the fact that SirsiDynix has not granted authorization to outside companies to perform maintenance on their software, SirsiDynix is the only source that can meet this requirement. The North American Industry Classification System (NAICS) for this acquisition is 541511, small business size standard of $27,500,000.00.
2. A response to this request shall consist of the submission of the following:
1.) Pricing- Completion of FA4452-15-R-0023 Information Sheet, Attachment 1. Any supporting documentation that can be used in evaluating the contractor’s price for completeness and reasonableness.
(a) - The six-month extension of services is IAW FAR 52.217-8, "Option to Extend Services" and will only be utilized if necessary. The six-month extension of services is not to be considered part of option period three and will be a separate option exercised if it is utilized. Offerors will not be required to submit pricing for a six-month Option to Extend Services. The Government will develop a total evaluated price (TEP) which will include this pricing, based on the offeror’s proposed option period three pricing.
2.) Technical- Maintenance and support agreement subscription descriptions. Any other documentation that demonstrates meeting all requirements set forth in the PWS.
The PWS, Attachment 2, and applicable provisions and clauses, Attachment 3, are provided for preparation of a proposal. Offerors are required to provide notification that their Representations and Certifications are available in the System for Award Management (SAM) database before an award can be made to them. If the offeror is not registered in the SAM, it may do so through the SAM website at https://www.sam.gov/portal/public/SAM/. 3. By submission of a proposal, the offeror accedes to all RFP requirements including: terms and conditions and representations and certifications. Offeror shall clearly identify any exception to the RFP terms and conditions and provide complete accompanying rationale.
4. The anticipated period of performance is as follows:
Base Period (12 Months) 01 Sep 15 - 31 Aug 16 Option Period 1 (12 months) 01 Sep 16 - 31 Aug 17 Option Period 2 (12 months) 01 Sep 17 - 31 Aug 18 Option Period 3 (12 months) 01 Sep 18 - 31 Aug 19
5. This RFP is not authorization to begin performance, and in no way obligates the Government for any costs incurred by the contractor for this requirement. The Government reserves the right not to award a contract in response to this RFP. Prior to commencement of any activities associated with performance of this requirement, the Government will issue a written directive or contractual document signed by the Contracting Officer with appropriate consideration established.
