STATEMENT OF WORK
For
C-5M RESTORATION MAINTENANCE PACKAGE
1.0 GENERAL SCOPE OF WORK
This requirement will provide restorations to the C-5M flight deck, cargo compartment, and troop compartment.
2.0 REQUIREMENT
2.1 Restoration Maintenance.
The C-5M fleet shall receive restorations in accordance with (IAW) this Statement of Work (SOW). The Contractor shall work in conjunction with the Air Force MSG-3 Programmed Depot Maintenance (PDM)/Isochronal Inspection Team. The Contractor shall execute with minimal impact to the Isochronal Inspection Team’s schedule and work flow. The Contractor shall work off-shift schedules, if necessary, to accomplish the tasks outlined in section 2.3, without negatively impacting the PDM/Isochronal Inspection Team’s schedule. For the purposes of this SOW, off-shift schedules are Contractor shift schedules that do not run concurrently with the Air Force PDM/Isochronal Inspection Team’s schedules. With exception of Government Furnished Property (GFP), the Contractor shall provide all materials, associated material information/patterns, and installation services required for C-5M restoration.
2.2 Technical Data.
2.2.1 Technical Data Package. The Government will provide the Technical Data Package (TDP) as Government Furnished Information (GFI). The Contractor shall utilize the standard TDP when performing tasks outlined in Section 2.3. The TDP will provide the required information to permit the procurement or manufacture of an item that duplicates the physical and performance characteristics of the original product (i.e., insulation blankets) without additional design engineering effort or recourse to the original design activity. Minor adjustments and/or variations to blankets may be required during installation to accommodate unique aircraft configuration.
The Contractor and their subcontractors shall provide updates and maintain engineering data for the item(s) designed/ developed/modified/produced by this contract during the period of performance of this contract. The Contractor shall provide AMC/A4MYA recommendations of changes to technical data not produced within this SOW.
2.2.2 Data Rights. The Government has unlimited rights to all documents/material produced under this contract. All documents, materials, and source codes of any software produced (if any) under this contract shall be Government owned and are the property of the Government with all rights and privileges of ownership/copyright belonging exclusively to the Government. Without written permission from the Contracting Officer (CO), the Contractor may not use or sell these documents and materials. All materials supplied to the Government shall be the sole property of the Government, and the Government prohibits any other usage. This right does not abrogate any other Government rights.
2.3 Restoration Tasks.
Upon receipt of a delivery order, Contractor will purchase materials to begin C-5M aircraft restorations. In accordance with TDP in Section 2.2.1, the Contractor shall provide restorations for each C-5M as follows:
2.3.1 Flight Deck
2.3.1.1 Bunk room areas – The Contractor shall remove and replace the mattresses and mattress covers from six bunk beds located in the flight deck bunkrooms. Contractor shall replace vinyl access covers for storage shelves.
2.3.1.2 Overhead panels – The Contractor shall remove and re-cover the flight deck overhead panels with white bisque vinyl material.
2.3.1.3 Avionic bay/boiler room access doors and boiler room bulkhead wall – The Contractor shall remove, fabricate, and install replacement diamond quilted soundproofing in bay 4 and on both doors for the avionics bays, boiler room access door, and forward boiler room bulkhead walls.
2.3.1.4 Flight station access overhead area – The contractor shall fabricate and install grey diamond-quilted material above flight station ladder. Government field personnel will submit engineer disposition 107 to engineering for approval for diamond-quilted soft panel blanket installed in this area, in lieu of the pre-existing hard panel.
2.3.2 Cargo Compartment
2.3.2.1 Diamond quilted blanket – The Contractor shall fabricate and install (no removal necessary) diamond quilted blankets for left and right side from fuselage station 400 to 2080. As required, the Contractor shall install removable covers and access doors. The Contractor shall ensure that cargo compartment cabin blankets extend from catwalk (water line 153.5) to the ceiling (water line 320.5) and provide a fully integrated appearance with deviations as necessary (ramp blankets begin at water line 140.5). The Contractor shall ensure that the blankets accommodate the Reliability Enhancement and Re-engineering Program (RERP) modification of the new horizontal fluorescent light assemblies installed in the cargo compartment. A two blanket high configuration whereby the top blanket overlaps the bottom blanket just behind environmental duct is required. Forward blankets shall overlap aft blankets for length of fuselage with deviations as necessary (i.e., adjacent hard panels).
The Contractor shall provide all tools/hardware to support this requirement. The Contractor will provide all standoff brackets per the TDP, screws, nut plates, and washers associated with C-5M restorations. Upper blankets will have grommets to ensure they fit on previously mentioned standoff brackets via cotter pins, this will also provide a cleaner form-fit-function. Lower blankets will install into pre-existing standoffs via a screw-washer assembly. Blankets installed on the ramps (FS 400-480 & FS 1960 – 2080) or blankets above crew entry door (CED) (FS 480-520) may require snaps to install.
2.3.2.2 Environmental Duct Insulation – The Contractor shall remove, fabricate, and install new insulation. The Government will remove and install environmental ducts from the aircraft. Ducts will be fitted for new insulation on site.
2.3.2.3 Anti-Skid Surfaces – The Contractor shall remove all anti-skid tape and old adhesive throughout the aircraft using the Carolina Equipment’s Aqua Miser or equivalent, and replace with new anti-skid utilizing engineering templates. The Government will provide the new anti-skid material as Government Furnished Material. Areas of concern are as follows: cargo floor, catwalks, flight station ladder, troop ladder, crew entry ladder, forward toes, forward extension, forward ramp, aft ramp, pressure door, and aft ramp toes.
2.3.2.4 The contractor shall fabricate and install diamond-quilted material for 7L and 7R personnel doors above and below the catwalk.
2.3.3 Troop Compartment
2.3.3.1 The Contractor shall fabricate, install, and replace diamond-quilted material in numerous areas of troop compartment as follows:
2.3.3.1.1 Contractor will replace forward troop compartment frangible wall (flap pack area) and slatted bulkhead access web gate.
2.3.3.1.2 The Contractor shall install (no removal necessary) between Troop Compartment Aft Emergency Escape Hatch #4 (F/S 1864, W/L 352) and Troop Compartment Aft Emergency Escape/Service Door #6 (F/S 1864, W/L 352). Government field personnel will submit an engineer disposition 107 to engineering for approval for diamond-quilted soft panel blanket installed in this area, in lieu of the pre-existing hard panel.
2.3.3.1.3 The contractor shall install material for #3L, #3R and #4 Emergency Escape Hatch and #6 Service Door. Government field personnel will submit an engineer disposition 107 to engineering for approval for diamond-quilted soft panel blankets installed in these areas, in lieu of the pre-existing hard panels.
2.3.3.1.4 The contractor shall replace entry material (triangular shape) for access behind lavatories.
2.3.4 Miscellaneous
2.3.4.1 The Contractor shall remove, fabricate and install new curtains; vinyl window shades; and re-web restraint gates. Contractor shall replace restraint straps for curtains, crash axes, emergency escape breathing device, escape ropes, and crew chief ladder. Contractor shall ensure that the web material meets or exceeds Mil-W-4088 Type VII (nylon webbing).
2.3.4.2 Contractor will receive specific guidance from Contracting Officer Representative (COR) regarding the designated storage area or disposal of each particular item removed from the aircraft.
3.0 DELIVERABLES
3.1 Reports.
3.1.1 Technical Report. The Contractor shall prepare and deliver a Technical Report, which includes a record of all significant events, findings, and recommendations. The Contractor shall deliver a draft Technical Report for format and technical content review to the COR no later than (NLT) 90 calendar days before contract completion. Government will provide review comments, if necessary, 30 calendar days after draft technical report submittal. The Contractor shall submit the final Technical Report upon contract completion date.
3.1.2 Status Report. The Contractor shall submit Status Reports to the COR by the 10th business day after receipt of each aircraft. The Status Report shall provide a summary and status of work performed during the reporting period to include funds expended, progress in relation to schedule and cost, and all analyses.
3.1.3 Technical Data Package. The Contractor shall provide any new/improved data discovered/identified during the contract that accurately supports the final restoration maintenance product. The updated data for the TDP shall provide the design, engineering, manufacturing, testing, and quality assurance requirements information necessary to enable the procurement or manufacture of an interchangeable item that duplicates the physical and performance characteristics of the original product, without additional design engineering effort or recourse to the original design activity. Product data shall reflect the approved, tested, and accepted configuration of the defined delivered item. All engineering product definition data created as a result of this contract shall be considered a part of the TDP. The Contractor shall deliver to the COR the revised TDP for the technical restoration requirement NLT 30 calendar days after COR approval .

4.0 GENERAL INFORMATION
4.1 Period of Performance.
4.1.1 Period of Performance. Period of performance for this contract shall include a one-year base period and four one-year options.
4.1.2 Scheduled Holidays. Scheduled holidays are as follows:
· New Year’s Day
· Martin Luther King’s Birthday
· President’s Day
· Memorial Day
· Independence Day
· Labor Day
· Columbus Day
· Veteran’s Day
· Thanksgiving Day
· Christmas Day
· Any other day designated by Federal Statute, Executive Order, and/or Presidential Proclamation.
4.2 Place of Performance.
4.2.1 Contractor shall be capable of performing two restoration efforts simultaneously at locations designated by Contracting Officer’s Representative (COR). Restoration tasks may take place at any of the following Government designated locations:
· Stewart ANG
· Dover AFB
· Warner Robins AFB
· Travis AFB
4.2.2 Aircraft may not be available during scheduled holidays, inclement weather, weekends, after duty hours, or Air Force down days.
4.3 Post Award Conference.
4.3.1 Within 10 workdays following the contract award date, the Contractor shall attend a post-award conference. This conference will be held at the C-5M refurbishment location (i.e., Stewart ANGB) or by teleconference. At the conference, the Functional Director, Contracting Officer (CO), and COR will review requirements of this contract and discuss technical requirements. Teleconference access will be available for individuals unable to attend the conference. The post-award conference, led by the CO and AMC/A4MYA representatives, will be conducted to ensure a common understanding of the requirements, expectations, and ultimate end products. The Contractor shall discuss the overall understanding of the project and review the background information and materials provided by the Government. Discussions will include the scope of work, how the efforts will be organized, and projects conducted. Nothing discussed during the post-award conference or subsequent meetings or discussions between the Government and Contractor shall be construed as adding, deleting, or modifying any SOW requirements to include specifications and due dates.
4.4 Travel.
4.4.1 Travel. The contractor may be required to travel during the performance of this SOW. All contractor travel shall be coordinated with the COR at least five working days prior to the travel start date. The Government shall reimburse the contractor for travel expenses IAW Federal Acquisition Regulation (FAR) 31.205-46 and the applicable travel regulation therein.
4.4.2 Travel Reimbursement. Per Diem and travel rates shall be reimbursed on an actual cost basis, not to exceed the amount authorized IAW FAR 31.205-46 and the applicable travel regulation. Airfare shall be the prevailing rates for commercial airlines at economy class. The most reasonable means of ground transportation (i.e. taxi, bus, rental car) shall also be used. The Government will not reimburse local travel and related expenses to the contractor. Local travel is defined as any travel 50 miles or less one-way from place of performance. At least five working days prior to the travel start date, the contractor shall submit a travel itinerary and estimated cost for coordination with the COR.
4.5 Government-Furnished Property/Equipment/Services.
4.5.1 Any data collected by the Contractor under this contract is the sole property of the Government. The Contractor can obtain point of contact (POC) information for any system via the COR.
4.5.2 The Government will provide access to aircraft as required in order for Contractor to perform duties. The Government will make aircraft available based on production schedule.
4.5.3 Government will provide applicable engineering reports, technical orders, and engineering drawings as necessary. When necessary, the Contractor will receive C-5 Original Equipment Manufacturer (OEM) “4E09012” and other program documents.
4.5.4 At the end of the contract period of performance, Contractor shall return any Government provided property to the Government via the COR.
4.6 Management.
4.6.1 Task Management. CO will appoint a COR and issue a COR appointment letter stating the authority for the COR. The Contractor will receive a copy of the written designation. The designated COR will provide technical assistance and clarification required for the performance of the tasks, participate in project meetings, and receive delivery orders.
4.6.2 Contracting Authority. Via modification to the contract, only the CO may take action to change the performance requirements, place of performance, scope of the contract, or any other terms or conditions.
4.6.3 Program Manager. The Contractor shall assign a qualified Program Manager and Site Manager to plan and execute the tasks required. The Program Manager selected shall be the single point of contact for the Government.
4.6.3.1 Program Manager Responsibilities:
· Post meeting minutes.
· Understand in detail the requirements, terms and conditions of the contract.
· Assign, track progress, and review performance on a daily basis.
4.6.3.2 Site Manager Responsibilities:
· Primary oversight of work accomplished at designated site.
· Ensures all employees are compliant with federal/state/base safety and security requirements.
· Primary focal point for communication with COR or designated Government representative. Coordinate daily Contractor refurbishment activities with Air Force PDM/MSG-3 Isochronal Inspection Team Chief.
4.7 Security.
4.7.1 Installation Security.
4.7.1.1 Regarding access to installation, the Contractor will be assigned a mission essential designation and gain both routine and/or increased Force Protection Conditions (FPCONS) access to the installation IAW the installation’s Antiterrorism/Integrated Defense Plan(s). Only the installation commander or the unit commander requesting contract support will assign the mission essential designation.
4.7.1.2 Property protection for the assigned facility where Contractor’s primary work center is located will be the responsibility of the local facility manager and local Government Security Manager, or their duly authorized representative IAW AFI 31-101, Integrated Defense, and command/local directives. Contractor must safeguard all Government-owned equipment and materials in his/her possession or use.
4.7.2 Operational/Communication Security.
4.7.2.1 Actual knowledge of, generation, or production of classified information is not required for performance of the contract. Contractor personnel shall not be authorized access to classified information and materials or have permission to work on classified projects without proper security clearances and a need-to-know. Contractor personnel will participate in the Operational Security program locally administered by the AF activity.
4.7.2.2 Regarding the Safeguarding of Unclassified and/or For Official Use Only (FOUO) Information, Contractor must meet Air Force standards for storing, processing, and handling unclassified and/or Controlled Unclassified Information (CUI) information and systems.
4.7.2.3 Additionally, upon termination of employment or the end of the contract performance period, the Contractor must surrender all resources (e.g. maps, publication/instructions, photos), that were provided by the Government to the Contractor in support of the performance of the contract.
4.7.3 Employee Clearances.
4.7.3.1 Contractor personnel are not required to have security clearance. Contractor employees that need access to Government owned Automated Information Systems (AIS) and or unescorted access to Government facilities will require the minimum of a Favorable National Agency Check Investigation (NACI). Contact the Local Security Forces Information Protection Office for guidance on submitting NACI request.
4.7.3.2 When the Contractor requires access to the controlled or restricted area and does not have the proper security investigation, Government will escort Contractor at all times within controlled or restricted area. Contractor must have an issued AF Form 1199 or be under escort. Contractor can only escort visitors from the same company for which he/she works and all visits must be for official business. Additionally, Contractors must fulfill, maintain, and comply with all security requirements IAW AFI 31-101, Integrated Defense and command/local directives.
4.7.4 Security Incidents and Violations.
The Contractor will immediately notify the wing security team of any actual security violation, security incident, or of any indication of a potential unauthorized disclosure or compromise of classified or sensitive but unclassified information.
4.7.5 Security Education/Training.
4.7.5.1 All full-time Contractor employees occupying physical space on Government installation will comply with the installation’s Security Education and Awareness program. The program consists of the following components: initial training, annual refresher training, and periodic tailored training.
4.7.5.2 Anti-Terrorism Level I training is required. Within 30 calendar days after contract start, all Contractor personnel shall complete Antiterrorism Level I training, as required by DODI 2000.16. Newly hired personnel shall complete the Antiterrorism Level I Training within the first 30 calendar days of their employment. Refresher Antiterrorism Level I training shall be completed and documented annually thereafter. The training is provided at https://atlevel1.dtic.mil/at.
4.7.6 External Certification Authority for Network Access.
For Contractor personnel who require access to Government web sites, but who do not work on a Government facility, the Contractor shall obtain a Public Key Infrastructure (PKI) Certificate(s) from an External Certification Authority (ECA). As of the SOW date, Operational Research Consultants, Inc.; VeriSign, Inc.; and IdenTrust, Inc. are the three ECA vendors approved by the DOD. The COR will provide a list of current DOD-approved ECA vendors upon the Contractor’s request.
4.7.7 Common Access Card for Base Access.
The Contractor’s Contract Administrator shall forward a common access card (CAC) request to the COR as a letter containing the following information:
· contractor’s company name and address
· contractor’s name
· contractor’s social security number
· contractor’s date of birth
· contractor’s home address
· contractor’s work phone number
· contractor’s contract number
· contractor’s contract expiration date Thereafter, the Contractor’s Contract Administrator will receive the approved letter from the COR. The Contractor has 30 calendar days to comply with the instructions in the COR’s response to obtain the CAC.
4.7.8 Identification Credential Requirements.
In accordance with the Procuring Contracting Officer’s (PCO) instructions, the Contractor’s Contract Administrator shall forward a CAC request to the Government COR. Thereafter, the COR will forward the approved letter back to Contractor’s Contract Administrator. The Contractor’s Contract Administrator shall forward the CAC request to the Government Trusted Agent (TA) for processing. The TA will provide email notification to Contractor personnel once information is loaded in the Contractor Validation System (CVS). Contractor will have a User ID and temporary password for 30 calendar days until the application process is complete. Contractor personnel have seven days to initially log into the CVS system. Upon closure of the contract or option or transfer of Contractor personnel, Contractor personnel shall return the CAC to the TA.
4.8 Safety.
4.8.1 Contractor Compliance.
Contractors shall comply with Occupational Safety and Health Act (OSHA) (Public Law 91-596) and OSHA Standard 29 CFR 1910, 1926, for the protection of their employees. The Air Force is part of OSHA’s Voluntary Protection Programs (VPP). All Contractors are required to familiarize themselves with the requirements of VPP. 4.8.2 Mishap Notification.
Within four hours, the Contractor shall telephonically notify the Installation Safety Office of all mishaps or incidents involving Government property, Government property damage, or employee injuries caused because of contract performance.
4.8.3 Mishap Investigation.
If requested by the Government PCO or ACO, the Contractor shall immediately secure the mishap scene/damaged property and impound pertinent maintenances and training records until released by the investigating Safety Office. If the Government investigates the mishap, the Contractor and the subcontractors shall cooperate fully and assist the Government personnel until the investigation is completed.
4.8.4 Environmental, Safety, and Occupational Health (ESOH).
The Contractor is solely responsible for compliance with the OSHA (Public Law 91-596) and the resulting standards, OSHA Standard 29 CFR 1910 and 29 CFR 1926, as applicable and the protection of their employees. Additionally, the Contractor is responsible for the safety and health of all subcontractor employees.
4.8.5 Contractor Safety Program.
The Contractor shall establish and maintain an acceptable safety program in accordance with their accepted Safety and Health Plan. The Functional Director must approve any changes to plans. The Contractor shall submit changes to plans in writing.

APPENDIX B
NON-DISCLOSURE AGREEMENT FOR CONTRACTOR EMPLOYEES
SUPPORTING AMC CONTRACTS
NOTE: This Non-Disclosure Agreement is an agreement designed for use by contractor (including subcontractor) employees assigned to work on AMC contracts. Its use is designed to protect non-public Government information from disclosure and prevent violations of federal statutes/regulations. The restrictions contained in this agreement also serve contractors by promoting compliant behavior that keeps contractors eligible to compete for Government contracts. In addition to the potential impact on future business opportunities, failure to abide by this agreement could result in administrative, civil or criminal penalties specified by statute or regulation.
"Non-public information," as used herein, includes trade secrets, confidential or proprietary business information (as defined for Government employees in 18 USC 1905); advance procurement information (future requirements, acquisition strategies, statements of work, budget/program/planning data, etc.); source selection information (quote rankings, source selection plans, contractor bid or quote information); information protected by the Privacy Act (social security numbers, home addresses, etc.); sensitive information protected from release under the Freedom of Information Act (pre-decisional deliberations, litigation materials, privileged material, etc.); and information that has not been released to the general public and has not been authorized for such release (as defined for Government employees in 5 CFR 2635.703).

I understand that I will have access to confidential business information (as defined by 18 USC 1905), contractor bid or quote information (as defined by FAR 3.104-3), and/or source selection sensitive information (as defined by FAR 3.104-3) either for contract performance or as a result of working in a AMC facility or of working near AMC personnel, contractors, visitors, etc. I fully understand that such information is sensitive and must be protected in accordance with 41 U.S. Code Section 423 and 18 U.S. Code Section 1905 and FAR Part 3. I also certify that I do not have any real or apparent conflicts of interest with respect to the information disclosed. If any potential conflicts of interest, real or otherwise, do present themselves, then I shall immediately disclose the pertinent information that may be a potential conflict to an agency ethics official who shall review the circumstances.
3. In the course of performing under contract/order # _________________________ or some other contract or subcontract for the AMC I agree to:
a) Use only for Government purpose any and all confidential business information, contractor bid or quote information, and/or source selection sensitive information to which I am given access. I agree not to disclose "nonpublic information" by any means (in whole or in part, alone or in combination with other information, directly or indirectly or derivatively) to any person except to a U.S. Government official with a need to know or to a non-government person (including, but not limited to, a person in my company, affiliated companies, subcontractors, etc.) who has a need to know related to the immediate contract/order, has executed a valid form of this non-disclosure agreement, and receives prior clearance by the contracting officer. All distribution of the documents will be controlled with the concurrence of the contracting officer.
b) Not use such information for any non-governmental purposes, including, but not limited to, the preparation of bids or quotes, or the development or execution of other business or commercial ventures.
c) Store the information in such a manner as to prevent inadvertent disclosure or releases to individuals who has not been authorized access to it.
4. I understand that I must never make an unauthorized disclosure or use of confidential business information, contractor bid or quote information, and/or source selection sensitive information unless:
a) The information has otherwise been made available without restriction to the Government, to a competing contractor, or to the public;
b) The contracting officer determines that such information is not subject to protection from release.
5. I agree that I shall not seek access to "non-public information" beyond what is required for the performance of the services I am contracted to perform. I agree that when I seek access to such information or attend meetings or communicate with other parties about such information, I will identify myself as a contractor. Should I become aware of any improper or unintentional release or disclosure of "non-public information," I will immediately report it to the contracting officer in writing. I agree that I will return all forms (including copies or reproduction of original documents) of any "non-public information" provided to me by the Government for use in performing my duties to the control of the Government when my duties no longer require this information. By signing below, I certify that I have read and understand the terms of this Non-Disclosure Agreement and voluntarily agree to be bound by its terms.
