SECTION SF 30 BLOCK 14 CONTINUATION PAGE
SUMMARY OF CHANGES
SECTION SF 1449 - CONTINUATION SHEET
The following have been modified:
ADDENDUM TO FAR 52.212-1
Addendum to 52.212-1, Instructions to Offerors – Commercial Items (Feb 2012)
(b)(4)
A. To assure timely and equitable evaluation of proposals, offerors must follow the instructions contained herein. Offerors are required to meet all solicitation requirements, including terms and conditions, representations and certifications, and technical requirements, in addition to those identified as evaluation factors or subfactors. Failure to meet a requirement may result in an offer being ineligible for award. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale. This solicitation, representing four separable requirements, is broken down into four sets of CLINs, one set for each of the three aircraft types and one set for Rodeo support. A “set” will be comprised of the base year plus option years for each separate CLIN – as identified in the CLIN SET chart in paragraph (b)(12) below.
B. Offerors shall submit one Technical proposal for each CLIN set that the offeror wishes to propose on; an offeror may submit a proposal for one, two, three, or all four sets of these CLINs. Offerors shall submit one original, and three identical hardcopies of each Technical proposal on or before the solicitation closing date and time to: Julie Brueggemann, Contract Specialist, HQ AMC/A7KQC, 507 Symington Drive, Room W202, Scott AFB, IL 62225-5022. In addition to the hard copy volumes, an identical electronic copy must be provided by CD or by e-mail on or before the closing date and time to Julia Brueggemann, Contract Specialist, at Julia.Brueggemann@us.af.mil, and John Baggett, Contracting Officer, at John.Baggett@us.af.mil. Ensure that the proposals are sent to the email addresses of both Julia Brueggemann and John Baggett. The electronic copies of the proposal shall be submitted in a format readable by Microsoft (MS) Office suite of applications or in Portable Document Format (PDF). Your email submission must be checked and determined to be “virus free” prior to submission. The Contracting Officer and Contract Specialist are the sole points of contact for this acquisition.
C. Each Technical proposal shall be submitted in sufficient detail to evaluate compliance with the requirements of the solicitation and must address the requirements defined in the Performance Work Statement. Each technical proposal is limited to 30 pages. Course syllabi shall be included in the technical proposal but shall not be included in the 30-page limitation. Subcontracting plans are limited to 10 pages, and shall be included in the technical proposal, but shall not be in included in the technical proposal’s 30-page limitation.
D. Specific Instructions:
1. Technical Proposal – No more than 30 pages
1.1 Staffing Approach
1.2 Technical Approach
2. Price Proposal
2.1 Complete blocks, 12, 17, and 30 of the RFP, SF1449. By completing referenced blocks, the offeror accepts the contract terms and conditions as written in the RFP.
2.2 Complete the representations and certifications as required in provision 52.212-3(b)(1) and (2).
2.3 Provide pricing (to include unit and total prices) for all CLINs included in your technical proposal.
(b)(12) Offerors shall submit all price proposals on one complete, most current, conformed copy of the solicitation. The conformed copy of the solicitation with all prices and other necessary information completed shall constitute the price proposal. Offerors shall submit one original, and three identical hardcopies of the price proposal on or before the solicitation closing date and time to: Julie Brueggemann, Contract Specialist, HQ AMC/A7KQC, 507 Symington Drive, Room W202, Scott AFB, IL 62225-5022. The electronic copies of the proposal shall be submitted in a format readable by Microsoft (MS) Office suite of applications. Your email submission must be checked and determined to be “virus free” prior to submission. The Contracting Officer and Contract Specialist are the sole points of contact for this acquisition. Although the price proposal shall be submitted on only one conformed copy of the solicitation, the CLIN sets will be evaluated individually. Offerors shall fill in the net totals next to each CLIN and then provide a total proposed price for the entire CLIN set in the appropriate space allocated on page 22 of the solicitation. Offerors not desiring to provide proposals for one or more aircraft types shall leave the spaces allocated for the price blank for each CLIN associated with those aircraft types. Offerors submitting price proposals on a CLIN set must provide a price for each CLIN within the CLIN set. Below is a chart showing the breakdown of the CLIN sets:
