PERFORMANCE WORK STATEMENT
REPLACE OVER HEAD DOOR AT BUILDING 278
1. SCOPE: The contractor shall furnish all resources necessary for the removal and installation of one overhead door and electrical operator. The furnished roll-up door shall bear the following: 14’x20’ in size, dark brown finish, rated 30 pounds per square foot minimum wind load, R-8 minimum insulation, and 24 gauge steel on front and back side. The furnished door operator shall include a 460 volt/3 phase motor, an emergency chain hoist, and a 3 button control system mounted near the door opening. Door shall be fully functional with all safeties required to comply with OSHA standards and UL 325 requirements.
2. PERIOD OF PERFORMANCE: Work must be performed within 121 calendar days of receipt in building 278, Altus AFB, OK 73523.
3. PRINCIPAL FEATURES: The principal work includes removing one overhead door assembly and electrical operator and installing one insulated roll up door assembly and electrical operator.
a. De-energize and disconnect electrical wiring from existing electrical operator.
b. Remove and discard existing door assembly and electrical operator.
c. Install new roll up door assembly.
d. Install new 460 volts/3 phase electrical operator with emergency chain hoist and reflective photocell safeties (all control wiring to door controls and photo cells shall be incased in metal conduit IAW NFPA 70, UFC 3-600-01, and any local codes or standards).
e. Install new control switches and make all necessary adjustments to operator to ensure the door operate safely and correctly.
f. All electrical requirements and door sizes shall be field verified by contractor.
4. GENERAL: All work shall be performed in accordance with the Manufacturer’s Specifications and procedures for each type of work required. When work is not covered by specifications, installation instructions, or performance procedures, said work shall be performed only after consultation, guidance and approval from manufacturer and written verification provided.
5. SUBMITTALS: Contractor is responsible to provide detailed records of all replacement parts and any and all warranty information to the customer.
6. SPECIAL REQUIREMENTS: All work shall be coordinated with the 97 CES Operations Flight. Smoking is permitted only in the designated tobacco use area for the facility.
7. The contractor will assign employees who possess certification to handle all required material handling, transportation, loading, unloading and product installation tasks to meet OSHA and base safety requirements. The contractor will ensure product meets required safety standards at the time the project is finished and accepted by the customer.
8. WARRANTY: Contractor must provide the standard commercial product warranty certificate specifying warranty period.
9. SECURITY REQUIREMENTS: The contractors will comply at all times with base security requirements to include base pass requirements and personnel security clearance.
10. ANTITERRORISM CLAUSE: Altus Air Force Base has an antiterrorism program that is responsible for helping secure and protect the base and its personnel from terrorist attacks. As a contractor working on Altus Air Force Base, you and your sub-contractors are required to obey all orders from Security Forces, and adhere to all security measures implemented as a result of Force Protection Condition changes (security posture changes).
Additionally, you and your sub-contractors are expected to report to base security forces any suspicious activities, packages, or items you see while conducting work on Altus Air Force Base. Suspicious activities could include personnel conducting surveillance of the installation, unauthorized personnel requesting access to the installation, or somebody asking a lot of questions about the base. In an effort to help familiarize you, your employees, and sub-contractors about antiterrorism, Altus Air Force Base has an antiterrorism binder with all of the pertinent information for awareness training available for review at the Pass and Registration Office located at the south gate, Altus Air Force Base, Oklahoma.
11. IP-Security Clause
11.1. Pass and Identification Items. The contractor shall ensure the following pass and identification items required for contract performance are obtained for employees and non-government owned vehicles requiring access to Altus AFB to perform contract requirements.
11.2. The contractor is responsible for providing information to assist in completing background checks for those employees requiring access to Altus AFB. This information shall consist of, but not limited to, the following: full name to include first, middle, and last, social security number, date of birth, address, driver’s license number and state of issue, number of years resident of the State of Oklahoma and Jackson County. This information will be protected under the procedures listed under the Privacy Act of 1974. A background check must be completed prior to unescorted access. Those contractor employees whose background checks reveal adverse criminal history may be denied access to Altus AFB.
11.3. The contractor is responsible for tracking and retrieving badges from employees upon separation, termination and/or completion of business on Altus AFB, Oklahoma. Badges from employees no longer working on the contract must be turned in to the Contracting office within 30 days of leaving.
11.4. Businesses may lose base pass privileges if they fail to control use of the badges or do not return them upon completion of use. Excessive abuse may result in termination or a decision not to exercise option periods for this contract. Employees who abuse their badge privileges by attempting base access beyond the dates, times, locations or specified work and other limitations may be subject to revocation of base access
12. Environmental Statement: Contractors must comply with all environmental requirements to include completion of initial Environmental Management Systems (EMS) awareness level training within 10 days of contract award or with a new contract employee beginning work on the Air Force Installation and the appropriate documentation of training completion is to be maintained. 13. Contractor personnel shall present a neat and clean appearance and be easily recognized as contractor employees. The contractor shall provide contractor personnel working at Altus AFB with a method of identifying the company name, employee’s first and last name, and job title. This may be accomplished by a name badge or a company shirt.
Employees must wear identification at all times.
