MEMORANDUM FOR CONS

FROM: 628 CES/CEOH

SUBJECT: Statement of work for fixed sky light Bldg. 16000

1. The sky light in the Wing Headquarters Building 16000 on the Air Base has been leaking for several months during heavy rain storms. The 628 Civil Engineer Squadron has made numerous attempts to seal the skylight without success. CES has deemed the scope of work is beyond Squadron capabilities and suggest a new skylight be purchased and installed via contractor to reduce facility degradation and safety concerns of building occupants.

2. Contractor requirements and responsibilities are as follows:

Contractor will be responsible for transportation logistics, delivery to site, site off load, and material transfer to roof deck. Old skylights will be removed from roof deck and site. New skylight configuration to be installed and fastened to top of existing curb. Contractor will be responsible for fastening, securing and properly sealing the new skylight components to the top of the existing curb without disturbing the existing roof deck and waterproofed area of the curb.

Contractor will conduct a quality assurance moisture leak test upon completion of installation.
