MEMORANDUM FOR 17 SFS/S5B

SUBJECT: Request for Base Access Pass

1. The following contractor, (Contractor, Phone Number) has non-ID card holding personnel requiring access to the base for work at (Fire School).

2. Request access to Goodfellow AFB for the following individuals

3. I will ensure above personnel are briefed that only properly licensed visitors appropriately covered by vehicle insurance, will operate a vehicle on Goodfellow AFB. Authorized vehicle operators will have in their possession a valid driver’s license, current vehicle registration and a valid insurance certificate at all times when operating a vehicle on the installation.

4. Upon completion or termination of this visit, or expiration of the issued identification pass, I will ensure all passes are returned to the Visitor Control Center.
