PERFORMANCE WORK STATEMENT
WASHER/DRYER MAINTENANCE, REPAIR AND LEASING SERVICE
1. DESCRIPTION OF SERVICES. The contractor shall provide all management, tools, supplies, equipment and labor necessary to purchase, deliver, maintain and repair washers and dryers at various locations on Goodfellow Air
Force Base, San Angelo, Texas. Services shall be performed in accordance with manufacturer’s recommended commercial practices and this Performance Work Statement (PWS). The contractor shall be responsible for cleaning the exhaust duct systems.
1.2. Phase-Out and Phase-In. The Contracting Officer’s Representative (COR) and the Contracting Officer (CO) will coordinate the phase-out and phase-in. The phase-out of old appliances and phase-in of new appliances shall be performed during the 30-day period after the expiration of the current contract award (28 February 2015). The new contractor shall submit a phase-in schedule to the CO for review and acceptance. The schedule shall be submitted no later than 10 workdays prior to the start of the phase-in. The successful offeror shall work closely with the current contractor to ensure that there are no lapses in service. The phase-out/phase in schedule shall provide the
Government no doubt of the of the offeror’s abilities to implement a comprehensive phase-in/phase-out plan. The phase-in schedule shall be typewritten and shall include the building number and date the appliance will be installed.
Installation of all appliances shall be performed in accordance with the phase-in schedule and shall be completed no later than 30 calendar days after start of phase-in (31 March 2015). The contractor shall ensure appliances are fully operational before proceeding to another building. No building will be without washer and dryer services for any period longer than 12 hours. After completion of work at each location, the contractor shall remove all packing boxes, paper, and excess materials. The government is not obligated to provide storage areas for machines during the transition period. The government will prorate the monthly payment for each appliance according to the number of days the appliance is available for use during the transition. The contractor shall provide a daily report of the number of appliances installed by building number to the COR.
1.2.1. Appliance Inventory. After completion of the phase-in period the contractor shall provide the CO and COR with a complete appliance inventory. The inventory shall contain, at a minimum, appliance type (washer or dryer), size, serial number, assigned machine identification number and building number where the machine is installed.
An updated appliance inventory shall be submitted whenever there is a change in the inventory and semiannually
(October and April). The contractor shall submit a corrected inventory within 3 days after COR or CO identifies any inaccuracies in the inventory.
1.3. Equipment Quality Standards. The contractor shall furnish, deliver, install, repair, and maintain the number of washers and dryers for each building as listed in Appendix A. All appliances shall be new (out of the box) at time of installation. The contractor shall ensure the quantity, size, width, and height of the appliances for that use that will fit at the specified location as listed in Appendix A. The contractor must provide catalogs, brochures, or other manufacturer written literature and specifications on washer and dryer equipment that the contractor proposes to furnish under this contract for approval. Literature must be provided as part of the contractor’s proposal and with all new requirements during this contract prior to contractor’s purchase. Contractor shall furnish all parts and accessories necessary to connect appliances at no additional cost to the government. All appliances shall meet current Energy Star® (green procurement) guidelines. Additional requirements and replacement of appliances during the life of the contract shall be new and the same size and color of existing appliances in that building.
Washers and dryers shall operate without the use of coins or tokens and shall not have any coin or token slots.
1.3.1. Washers. Washers shall be commercial type, heavy duty with a minimum of 16 lb load capacity standard size. Washers shall be energy star rated. As a minimum they shall be equipped with a 1-position water level with an automatic timer or with a manually adjustable timer and a 3-position water temperature control (hot, warm, and cold). Washers shall be equipped with a safety feature that will stop the flow of water, the agitator, and spin action when the lid is open. Washers shall have standard hot and cold water connections. They will have a minimum of three (3) cycles, normal, permanent press, and delicate.
1.3.1.1. Water Condition. San Angelo is known to have very hard water. The contractor should take any precaution they deem necessary to protect and minimize any potential damage to the machine. The government will not be held responsible for the condition of the water nor any damage that may be incurred.
1.3.2. Dryers. Dryers will be commercial type, heavy duty, energy star rated. The dryers will have a minimum of three (3) cycles, normal, permanent press, and delicate. The dryers will be equipped with a timer. A safety switch to stop the dryer’s operation when the door is opened is mandatory. All dryers will have a fireproof, flexible, metal dryer vent attached to the wall unit.
1.3.3. Stackable Dryer. Contractor shall be responsible for ensuring the connections for the utilities and vents are compatible with the existing connections. In accordance with the Appendix A of this solicitation, 61 stackable dryer units with a minimum of three (3) cycles; no heat, permanent press/regular, and delicate are required. Stackable dryer units shall be no taller than 79” and shall be placed just outside of the enclosed area where the current stackable dryers are located in buildings B3135 and B3139 on the first, second and third floors. Building B3126 shall be modified by the government on the first, second and third floors to accommodate stackable dryer units no taller than 79” before contract award. The dryers will be equipped with a timer and have an 18 lb capacity cylinder.
A safety switch to stop the dryer’s operation when the door is opened is mandatory.
1.4. Appliance Identification and Signage. The contractor shall place an approved sign or decal with operating instructions on each appliance. If the appliance has operating instructions preprinted on the lid this will meet the requirement. Each appliance shall be labeled with a numeric or alpha/numeric combination, which uniquely identifies each appliance. The contractor shall ensure that all machines are numbered by their location, for example
Bldg 255, first floor machines shall be numbered 255-1-0001 in consecutive sequence. These numbers shall be maintained during the contract period. The operator when calling in equipment malfunctions will use this number.
The label will be legible at all times and shall be replaced if necessary at no additional cost to the government.
Numbers must be waterproof and shall be replaced when they become faded, soiled or defaced or within 2 duty days (Monday through Friday), of notification by the CO or the COR. The label shall be affixed on the front of each appliance and shall have at a minimum the following information:

1.5. Additions, Deletions, and Relocation of Appliances. The CO will notify the contractor when appliances need to be added, deleted, or relocated during the life of this contract. Only the CO has the authority to negotiate with the contractor regarding quantities, dates, and equipment.
1.5.1. Additions. The CO will notify the contractor when additional appliances are required. Within fourteen (14) calendar days of the notification or on the date agreed upon by the CO and the contractor, after which the contractor shall install the appliance(s). Appliance(s) shall be new, out of the box and shall comply with equipment standards as outlined in the PWS. The contractor shall invoice at a prorated daily rate beginning with the day the appliance becomes operational.
1.5.2. Deletions. The CO will notify the contractor when appliances are to be deleted from the contract. Within fourteen (14) calendar days of the notification or on the date agreed upon by the CO and the contractor, the contractor shall remove the appliance. The contractor shall invoice at a prorated daily rate ending with the day the appliance was removed.
1.5.3. Relocations. The CO will notify the contractor when appliances need to be relocated. Within three (3) working days of the notification or on the date agreed upon by the CO and the contractor, after which the contractor shall move the appliance. In the event of an emergency requiring immediate relocation of the appliances, the contractor acknowledges that contractor owned appliances may be moved by Civil Engineer personnel. An example of an emergency may include but is not limited to: pipes bursting, water leaks, gas leaks, or structure damages in which it is vital for the appliance to be relocated to gain access to the area in need of attention. The COR will immediately notify the contractor in this situation. After the emergency repairs have been complete, the contractor will be notified and will have twelve (12) hours to reinstall the appliances. Relocation services performed by the contractor will be invoiced under the appropriate line item. Relocation fees will be assessed each time the contractor is required to move the appliance (i.e. it will be charged to move it and another charge to replace it back to the original location).
1.6. Scheduled Preventive Maintenance. The contractor shall submit a monthly maintenance schedule to the COR for all appliances within 10 days after contract award and no later than the 25 th of each subsequent month for the next month’s schedule. The schedule shall include at a minimum the building number, appliance number, and the date in which services will be performed. If there are any revisions to the schedule, the contractor shall submit a revised schedule at least 3 days prior to the effective change. The contractor shall inspect appliances for cleanliness and ensure equipment is operational and performing according to design. The contractor will ensure all machines operate safely, are clean and are free of corrosion. The contractor shall minimize disruptions and inconveniences to users. All replacement parts will be new. If contractor discovers any abuse or damage to the appliances or if maintenance is required involving government-supplied utilities or property such as electrical outlets, drains, or faucets, the contractor shall immediately notify the COR in writing with all applicable information, i.e., bldg, machine number, etc. If an appliance requires maintenance, the contractor shall post a professional sign stating
“OUT OF SERVICE – DO NOT USE” with the date the problem was noted, and the estimated date of repair/returned to service. Any appliance out of service for more than 72 hours (excluding holidays and weekends) from the time of notification may result in a deduction calculated from a daily amount of $5.00 per machine contingent upon the approval of the CO. If an appliance cannot be repaired within 72 hours, the contractor shall replace the appliance with a new appliance and notify the COR of the replacement and the new appliance’s identification number.
1.6.1 Dryer Lint Filters. Contractor shall inspect dryer lint filters and replace torn or bent filters. Contractor shall ensure dryer exhaust vents are properly secured to dryer and exhaust ducting at all times to prevent fire hazards. In addition, the contractor is responsible for cleaning the area behind, between, and around the appliances to remove all lint and debris. Items of value found in the process of cleaning around the machine are considered government property and should be turned into the building manager.
1.6.2. Permanent Exhaust-Duct Cleaning. The contractor shall quarterly (Oct, Jan, Apr, and Jul) inspect and clean all accessible permanent horizontal and vertical exhaust ducts. The contractor shall submit a quarterly schedule no later than 3 days prior to the required month and shall include at a minimum the building number and the date in which services will be performed. If there are any revisions to the schedule, the contractor shall submit a revised schedule at least 3 days prior to the effective change. Notification of inaccessible areas must be submitted to the CO in writing. The contractor shall affix a label to each permanent duct which identifies the date when the ducting was cleaned and inspected and date when next inspection and cleaning is due. The date shall be written out by month and year; to avoid confusion the month shall not be written out numerically. The label must include a place for contractor’s signature indicating the technician’s name that performed services. The location of the label shall be either the door or duct at the point the dryer vent enters the permanent duct. The cleaning and inspection of the duct work is critical to the government to eliminate potential fire hazards.
1.7. Service Calls. The contractor shall respond to a repair service requests within 24 hours. Service repair calls will occur when machines malfunction or fail to operate properly. The contractor is required to place a professional sign on the appliance that is non-operational. The sign shall include the following information: “OUT OF
SERVICE - DO NOT USE,” the date when the contractor made initial response to service call, and the estimated repair date. If an appliance cannot be repaired within 72 hours, the contractor shall replace the appliance with a new appliance, and notify the COR of the replacement and the new appliance’s identification number.
1.8. Maintenance, Repair, and Replacement. Any inoperative machines shall be repaired or replaced by the contractor within 72 hours (excluding holidays and weekends) after notification is given the contractor by the CO or
COR. The contractor shall not accept notification directly from the machine users. Upon completion of repairs, the contractor shall notify the COR in writing. This report shall contain the date the repair was called in and completed, the building number, the machine number, the serial number, the model number, the complaint and action taken.
Upon completion of machine installation, removal and/or repair, the entire site shall be left clean. The contractor shall clean and wipe down each machine after maintenance/repairs are completed. The contractor shall notify the
CO when any replacement part must be ordered and repair delays are expected. The contractor must notify the COR of the expected delay and provide a replacement machine if repair time exceeds 2 working days.
1.8.1. Maintenance Information. The contractor shall maintain, repair, and/or replace all machines furnished under this contract. This shall include, but is not limited to: all parts and labor, hot and cold water connection hoses, include cleaning water hose screens, dryer vents, dryer clamps and flexible exhaust hoses.
1.9. Clean-up. The contractor shall be responsible for leaving appliances and the area clean where work or maintenance is performed. Debris generated by the contractor shall be disposed at an off-base disposal site.
Appliances and parts will not be disposed on base.
1.10. CONTRACTOR PERSONNEL:
1.10.1. Contract Manager:
The contractor shall provide a contract manager who will be responsible for the operations, management, and ensure compliance of contract requirements. The contract manager shall be designated in writing to the CO prior to the contract start date. A point of contact (POC) shall be provided for emergencies during off-duty hours.
1.10.2. Repair Technician
The contractor shall have a local, qualified, and experienced repair technician who will be responsible for the day-to-day appliance repair and maintenance. The repair technician shall be designated in writing to the CO prior to the contract start date.
1.10.3 Communications:
The contractor will provide a phone number where the contract manager and repair technician can be contacted during normal duty hours. The contractor shall establish and maintain a cellular/mobile telephone system for two-way communications with the COR prior to the contract start date. The communications system shall be no cost to the government and consists of sufficient phones, power, and range to establish and maintain immediate, direct, and sustained communications between any key contractor personnel and the COR.
1.11. Employee Identification:
Contractor personnel shall present a professional, neat and clean appearance and be easily recognized as contractor employees. This will be accomplished by wearing uniform clothing bearing the name of the company and the employee or by wearing contractor issued badges, which contain the company's name, employee's full name, employee photo and the words "Washer and Dryer Service Contract".
1.12. Base Rules and Regulations:
Contractor personnel shall report to an appropriate authority, any information or circumstances of which they are aware may pose a threat to the security of Department of Defense (DoD) personnel, contractor personnel, resources, and classified or unclassified defense information. Contractor employees are required to obey base rules and regulations. All contract personnel will obey posted signs (controlled entry, speed limits). The contractor personnel shall comply with base traffic regulations; failure to do so could result in terminating base access. Smoking is prohibited in government facilities.
1.13. General Information:
1.13.1. Hours of Operation. Normal business hours are defined as Monday through Friday, 0730-1630. The contractor shall perform the services required during normal business hours unless otherwise approved by the contracting officer.
1.13.2. Base Closure due to Weather:
The local television news broadcasts and local radio station will broadcast the official notification of base closure due to inclement weather. If the base is officially closed and the contractor cannot gain access to the base to perform preventive maintenance, unscheduled maintenance, additions, deletions, and relocation timeframes will be adjusted accordingly and will not be counted against contractor’s performance dates.
1.13.3. Legal Holidays. The Contractor is not required to provide service on the following days:

While AETC Family Days have not been identified past calendar year 2016 it is anticipated that the same number of
AETC Family days will be declared each year.
If a holiday falls on Saturday, the preceding Friday will be observed. If a holiday falls on a Sunday, the following
Monday will be observed. The number of “AETC Family Days” are approximately the same number of days each calendar year.
1.14. Workload Information. The contractor is advised washer and dryers will receive a very high use above normal wear and tear due to the training mission on Goodfellow AFB and the number of training students that process in and out. There is a continuous flow of students. The contractor can also expect to continually replace expendable parts due to high use and/or overloading such as but not limited to knobs and handles at no additional cost to the government.
1.15. Equipment Use/Abuse:
The contractor shall notify the CO and COR in writing of any suspected abuse of appliances. The COR will validate the alleged abuse and forward the findings to the CO for a final determination. An equitable price adjustment for abuse of appliances shall be verified and approved by the CO. Payment for abuse will be negotiated with the CO and the final ruling will be documented. The documentation will include the method and steps in which the contractor will take to recoup payment; the COR has no authority to make a determination.
1.16. Fire Codes. The contractor shall comply with all fire and safety codes. This will include using metal/non-flammable exhaust vent hoses for all dryers. The contractor will also insure that gas connection for the dryers comply with local fire codes and that all dryer safety devices are fully operational.
2. SERVICE SUMMARY (SS):
The contractor service requirements are summarized into performance objectives that relate directly to mission essential items. The performance threshold briefly describes the minimum acceptable levels of service required for each requirement. These thresholds are critical to contract success.

3. QUALITY CONTROL:
The contractor shall develop and maintain a Quality Control program to ensure services are performed in accordance with commonly accepted commercial practices for appliance maintenance and repair. A written Quality Control
Plan (QCP) shall be submitted to the CO for review, feedback, and acceptance. The QCP shall be submitted with the contractor’s proposal. The QCP shall specifically outline procedures to identify and prevent defective services from recurring. As a minimum, the Contractor shall develop quality control procedures that address the areas identified in the Services Summary. The QCP shall provide the Government no doubt of the offeror’s abilities to manage all resources required to perform the requirements outlined in the solicitation.
3.1. Quality Assurance. The government will periodically evaluate the contractor's performance in accordance with the Performance Plan for those tasks listed in the Service Summary (SS), Section 2 in this Performance Work
Statement (PWS). The COR will use customer complaints, periodic surveillance, and scheduled inspection as quality assurance methods.
3.2. Documentation. The contractor shall maintain a log for all inspections, repairs, and preventive maintenance performed. The log shall be made available to the COR upon request.
4. GOVERNMENT FURNISHED SERVICES/EQUIPMENT:
4.1. Utilities:
The government will provide water, electricity, and gas as needed in performance of work and when appliances are in operations. The contractor shall provide their own tools, equipment, and other items as necessary to make utility connections. Any utility connections shall be coordinated with the COR. If the contractor notes a deficiency regarding the utilities, immediate notification is required to the COR (email would be an acceptable form of notification). Should this be the case then the contractor will not be held to the 48 hour performance threshold until the government corrects the utility deficiency. For example: if an electrical outlet is non-operational therefore the appliance will not operate this will not be counted against the contractor’s performance threshold. However, once the government corrects the deficiency, contractor is then responsible to ensure machine is operational.
5. SECURITY REQUIREMENTS:
5.1. PHYSICAL SECURITY. The service provider shall ensure service provider employees comply with base operations plans/instructions for Force Protection Condition (FPCON) procedures, Random Antiterrorism Measures
(RAMS) and local search/identification requirements.
The government will provide/make available AT Level I training/information at the contractor's request. This information can be provided through the 17 TRW contractors AT Level I awareness pamphlets/binders, through computer based training programs, or through the unit/installation Antiterrorism Officer.
6. PERFORMANCE OF SERVICES DURING CRISIS DECLARED BY THE NATIONAL COMMAND
AUTHORITY:
According to DFARS 252.237-7023, Continuation of Essential Contractor Services During Crises, and the Air
Force implementation thereof, unless otherwise directed by an authorized government representative, it is determined that Washer/Dryer requirements under this Performance Work Statement (PWS) are essential to be performed during a crisis.
PART 7 - ENVIRONMENTAL REQUIREMENTS:
7.1. Compliance With Laws. Contractors are NOT exempt from air emission, storm water, hazardous waste, and other environmental compliance rules and regulations. The Contractor shall comply and ensure that all Sub-
Contractors comply with all applicable federal, state, and local laws, regulations, ordinances and standards related to environmental matters. The Contractor shall also comply and ensure that all Sub-Contractors comply with all specific instructions or directions given to the Contractor by Goodfellow AFB regarding environmental matters.
7.2. Hazardous and Special Wastes Generated by the Contractor. The Contractor shall identify, characterize, containerize, store and dispose of hazardous wastes in strict accordance with federal guidelines found in the Code of
Federal Regulations, Title 40 (40 CFR) parts 260-270, state regulation 30 TAC 335, all local guidelines, and as specified. A Uniform Hazardous Waste Manifest shall be used by the Contractor to document all parties and locations involved in the transportation, storage and disposal of all hazardous and special wastes. This form shall be provided to the government by the Contractor and signed by the Base Environmental Coordinator (CEIE) before the waste is transported from the limits of government property. A copy of the manifest shall be signed by the receiver of the waste and submitted to the Contracting Officer not later than forty-five days after disposal has taken place.
Hazardous waste treatment, storage and disposal facility shall be located within in the state of Texas, permitted by the U.S. EPA and approved by CEIE.
7.3. Contractor Encountered Hazardous Waste. The Contractor shall notify the COR and CEIE upon encountering any material not identified in this Statement of Work thought to be hazardous that could jeopardize the safety of workers or personnel in the area. The Government will be responsible for characterization, transportation, storage and disposal of the waste if necessary.
7.4. Hazardous Materials. If any of the material used by the contractor are classified as hazardous in accordance with AFI 32-7086, the Contractor will submit an AF Form 3952, (Chemical/Hazardous Material Request
Authorization) for each material item with all supporting information as required for approval. The Contractor must obtain authorization from the Contracting Officer prior to bringing or using hazardous materials on the installation.
The Contractor must supply an up-to-date MSDS for each requested AF Form 3952 item listed as a hazardous material, as defined to be delivered under this contract. The hazardous material shall be properly identified and include any applicable identification number, such as National Stock Number or Special Item Number. This information shall also be included on the Material Safety Data Sheet submitted under this contract. The Contractor must maintain a file of all MSDS. No hazardous materials, lubricants, oils, liquids or related materials shall be deposited in the refuse containers on base. The Contractor shall contact CEIE for disposal guidelines of any spent hazardous materials.
7.5. Nuisance and Polluting Activity Prohibited. Polluting, dumping, or discharging of any harmful, nuisance, or regulated materials (such as but not limited to concrete truck washout, vehicle maintenance fluids, residue from saw cutting operations, solid waste and hazardous substances) into building drains, site drains, streams, waterways, holding ponds or to the ground surface shall not be permitted and the Contractor shall be held responsible for any and all damages which may result. Further, the Contractor shall conduct work activities in such a fashion as to avoid creating any legal nuisance, including but not limited to, suppression of noise and dust, control of erosion, and implementation of other measures as necessary to minimize offsite impacts of work activities.
