TECHNICAL PROVISIONS

DIVISION 1 - GENERAL REQUIREMENTS

SECTION 01010 - GENERAL

1. SCOPE: The work covered by this project consists of the Contractor furnishing all layout, survey, plant, labor, supervision, quality control, materials, equipment, machines, tools, appliances, services, supplies, and incidentals and performing all operations in connection with the renovation of the Western Winds Dining Facility, Goodfellow AFB, Texas, complete and in strict accordance with the plans and specifications.

2. WORKING CONDITIONS:

2.1 Background: Goodfellow AFB currently has 2 (two) dining facilities;

(1) Western Winds B334 (Opened Weekdays for Midnight, Breakfast, Lunch and Dinner. Closed on Weekends.); (2) Cressman Dining Facility B3220 (Opened weekdays: Breakfast and Lunch. Weekends: Brunch and Supper. Both dining facilities serve on average over 3488 meals daily. While Western Winds dining facility is under renovation, Cressmen Dining facility will be the only full time base dining facility. Currently Western Winds serves on average 2131 meals daily. No renovation work at Western Winds Dining facility can occur until all Phase 1 temporary construction/alterations at Cressman Dining facility have been completed in order to accommodate the increased demand of daily meals that will occur while the base has only one remaining open dining facility.

In order for this to occur the work shall be executed in four (4) Phases summarized below. No additional time will be allowed for concurrent construction of any awarded CLIN items. Also the Contractor shall schedule his work to be executed in consecutive Phases as described below. The Contractor cannot start work on the next Phase of work until the prior Phase has been accepted by the Government.

2.2 PHASE 1, Cressman Temporary Construction: Perform all temporary construction work for Cressman as described below and described in the temporary construction plans. The Contractor shall perform all temporary facility erection/installation/operation, provide temporary heating/cooling, and temporary electrical/power as well as maintain/repair all temporary facilities during the entire duration of this contract. The performance period for Phase 1 shall consist of 80 calendar days from Phase 1 NTP for facility erection and installation to an operational capability.

The maintenance/repair of the temporary facilities will consist of 365 calendar days from Phase 1 NTP, or the entirety of the contract. Note, the Government will have a 15 calendar day transition time from the completion of Phase 1 prior to the start of Phase 2.

2.3 PHASE 2, Moving/Storage for Relocation to Cressman: Relocation includes all moving from Western Winds to Cressman, and required long term and short term storage. During the entire duration of this contract, with the exception of food service operations and sanitation, the Contractor shall be responsible for fully maintaining/repairing all temporary construction facilities along with providing adequate utilities for power, lighting and thermal comfort for the troops as well as ensuring all temporary food storage facilities are fully operational. Refer to Attachment “A” for a complete list of furnishings and equipment to be moved

SECTION 01010 Page 2 to Cressman, long term storage trailer(s) and short term storage trailer(s). The contractor will be responsible for obtaining/maintaining storage trailer(s). The performance period for Phase 2 shall consist of 30 calendar days from Phase 2 NTP for moving for relocation to Cressman. The storage for relocation to Cressman is 270 calendar days. Note, the Government in addition will have a 15 calendar day transition time from the completion of Phase 2 prior to the start of Phase 3. The Contractor shall also mobilize his staging area at Western Winds.

2.4 PHASE 3, Repair Western Winds: Perform all awarded CLIN demolition, repair and minor construction work at Western Winds as described in this contract. The performance period for Phase 3 shall consist of 180 calendar days from Phase 3 NTP. Note, the Government in addition will have a 15 calendar day transition time from the completion of Phase 3 prior to the Phase 3 NTP.

2.5 PHASE 4, Returning to Western Winds: Moving/returning/reinstallation of all equipment and furnishings back to Western Winds from Cressman and temporary facilities. The performance period for Phase 4 shall consist of 30 calendar days from Phase 4 NTP. Upon completion of Phase 3, the Contractor shall load, transport, return and install all equipment and furnishings to the Western Winds facility at locations as determined by the Government. Upon completion of all work, the Contractor shall restore the Cressman temporary construction site to its original condition. This work includes disassembly/storage of all temporary facilities, reconnection/rerouting of all utilities and site cleanup at both the Cressman site and the Contractor parking lot staging area, as required.

2.6 General: During the entire duration of all four (4) Phases of this contract, total performance period of 365 days, Cressman will remain open and fully operational at all times. The Contractor shall also exercise care to reduce noise and ensure safe construction activities while minimizing disturbances to Cressman as well as the adjacent dormitories and occupied mission support facilities at both the Cressman and Western Winds sites at all times. The Contractor shall coordinate the work of all activities whereby both the Government and the Contractor can continue operations with the least possible interference and inconvenience.

a). Western Winds: Nearby dorms and mission support facilities include Dorms 241, 242 and 243. Also Medical Admin facility B246, Consolidated Learning Center B316, Drug Demand B330, and Post Office B337.

b). Cressman: Nearby dorms and mission support facilities include Dorms B3206, B3208 and B3210 to the south and the Angelo Inn complex B3305, B3307 and B3311, along with the Personnel Office B3303 and the 1st Community Federal Credit Union B3218.

2.6.1 The first fifteen (15) calendar days after the date of the Phase 1 NTP, shall be for the Contractor lead time. This time is allocated solely for the Contractor to perform such tasks as compiling submittals for approval and/or information, ordering materials to be available for the actual construction work for all phases, performing site surveys, materials surveys, ordering long lead-time items, and verifying dimensions. The Contractor shall have full access to all facilities but no actual work shall be permitted during this period, only site survey work. Western Winds and Cressman facilities will remain occupied and shall be fully operational during this period of the contract.

2.6.2 The Contractor shall have a traffic and pedestrian management plan for after the Phase 1 NTP showing pedestrian and traffic flow altered by demolition, new temporary facilities, new construction, and proposed alternate routing. Note, at all times east/west thruway traffic shall be permitted on both the Angelo Inn parking lot north of Cressman and the parking lot south of Cressman.

2.6.3 In accordance with the Goodfellow Utilities Specification, the Contractor shall submit written notification regarding any and all utility outages to the Contracting Officer 14 calendar days in advance of the proposed outage. All water, power, or communication outages must be scheduled for weekends only.

2.6.4 The Contractor shall have the responsibility to prevent freezing of utility lines and stoppages in sanitary sewer lines during construction/demolition operations. Upon completion and prior to final inspection, the Contractor shall flush all lines and prove flow through the lines.

2.6.5 For all work that shall be accomplished on a weekend, a (7) seven calendar days advance written request seeking permission to work on the weekend shall be submitted to the Contracting Officer. This request shall specify the amount of time and number of workers necessary to accomplish the work. After the above work is complete, the Contractor shall complete all demolition and new construction, as required.

2.6.6 During the performance of the entire project, weekly/periodic progress/coordination meetings between the Government and Contractor will occur. Meeting conference scheduling will be determined by the Government during the pre-construction meeting. At any time, no more than one phase of construction can occur.

2.6.7 Asbestos and Lead Based Paint: The mastic for the VCT that will be replaced in Rooms 109 and 110 contains asbestos containing material. The contractor shall follow the requirements in SPECIFICATION SECTION 02050, DEMOLITION/REMOVAL/AND ASBESTOS REMOVAL USING RESILIENT FLOORING COVERING

INTSTITUTE (RFCI) METHODS for removal and disposal of asbestos containing materials. To the best of the Government’s knowledge, no other asbestos or Lead Based Paint will be disturbed by this project. Should the Contractor encounter other previously unidentified or suspected Asbestos or Lead Based Paint which must be disturbed to comply with the contract documents, the Contractor shall cease all work which would disturb the suspect materials and shall immediately notify the Contracting Officer. The Government will take steps, as appropriate, to ascertain the material’s composition and determine any remedial actions necessary.

2.7 PHASE 1:

2.7.1 The Contractor shall ensure continuous functional operation of all Cressman building utility systems (power, gas, HVAC systems and HVAC lines, fire, alarm systems, domestic water to include cold water and hot water distribution, sewer, and communications to include cable TV, telephone, internet, are fully operational). Note, there are no facility security systems in Cressman nor is there any work to be done inside of Cressman.

2.7.2 The Contractor shall conduct all work such that means of Cressman Dining facility main north and south doors/ingress and egress for fire response routes, etc., are maintained at all times. The Contractor shall be responsible for providing suitable, approved temporary facility interior signs, roadway barricades, roped barriers, warning tape etc., to warn occupants of hazardous areas at both job sites for the entire duration of the contract at no additional cost to the Government. Also, the Contractor shall ensure safe pedestrian/troop marching access of all troopwalks at all times.

2.7.3 Temporary Dining Covered Shelter(s) Facilities: Refer to Temporary Construction Site Plan, the Contractor shall provide all covered weather-tight sealed shelter(s) to accommodate the feeding at any one time for 300 troops as manufactured by Western Shelter Systems (1-800-971-7201) using Product 2360 with 1,274 SF of usable floor (22ft 8in by 60ft 1in) or equal, refer to Attachment “B”. The Contractor shall provide and install/erect all covered shelter(s), all furnishings including seating and tables, lighting, heating/cooling and the serving line equipment, refer to Attachment “C”. All cover shelter(s) shall include a Western Shelter Systems modular floor product S)-FL1935BT or equal raised 2 inches off of the ground/pavement surface. Also all covered shelter(s) shall be capable of being locked down when not in use. Each shelter shall have a posted ingress and egress sign and shall be interconnected. In advance of the covered shelter installation, the Contractor shall receive training from the covered shelter manufacturer.

2.7.3.1 The Contractor shall ensure all covered shelter(s) are adequately maintained in accordance with the manufacturer’s directions. The Contractor shall ensure all cables/ropes/anchors/turnbuckles are firmly secured to prevent disconnection during windy/inclement weather. On a daily basis, the Contractor shall visually inspect all covered shelter(s) to ensure they are properly anchored/secured without excessive slack.

2.7.3.2 The Contractor shall be responsible for all covered shelter repair/maintenance for intended use. The Contractor shall immediately respond no later than within 1 hour to all Government requested troubleshooting calls.

2.7.3.3 Temporary Electrical/Lighting: The Contractor shall provide new power to all food serving line equipment using Western Shelter Systems electrical distribution kit WS-EDK50 or equal. Bulbs attached to temporary lighting strings and extension cords shall be protected by guards. All exposed empty light sockets and broken bulbs shall be immediately replaced.

Also all shelter(s) shall exterior lighting around 4 sides of each shelter and have emergency exit lights at each door entry. All exterior lighting site and shelter lighting shall have a minimum of .01 horizontal and .01 vertical footcandles uniform lighting at all shelter site boundaries for safe ingress/egress access.

2.7.3.4 Temporary Heating/Cooling: The Contractor shall provide Western Shelter Systems product XE-1200 (2 per covered shelter) or equal. Each heating/cooling devices shall have a permanently affixed safety plate describing clearance from combustible materials, ventilation, electrical power supply characteristics, moving/handing instructions and name/address of manufacturer. Also the Contractor shall provide a minimum of two new ceiling fans (equally spaced) for each covered shelter.

2.7.3.5 Temporary Power for Food Serving Line: The Contractor shall provide new power to all food serving line equipment using Western Shelter

Systems electrical distribution kit WS-EDK50 or equal. Refer to Attachment “C” for a complete list of Government provided food serving equipment and the serving line configuration at the Cressman temporary construction site to be installed by the Contractor. Note, The Government will prepare all food in the Cressman Kitchen and serve from the food serving line inside the designated shelter with the serving line. All food deliveries will occur from the Cressman north side entry door. The Contractor shall provide a portable aluminum adjustable loading ramp minimum width of existing steps to enable wheeled food cart access to/from Cressman and the shelter(s).

2.7.3.6 Hand Washing Sinks: The Contractor shall provide Western Shelter Field Sinks (2) product WS-S3A or equal. The Contractor shall provide all plumbing lines, connect an above ground freeze protected water supply line, 100 gal potable water tank, min 20 gal hot water tank, and provide a 50 gal min drainage tank for each hand washing station. The location of the hand washing sink shall be at the serving line shelter entrance.

2.7.3.7 Pest Control: The Government will be responsible for pest control at the temporary shelter(s) and storage facilities besides the air curtain.

The contractor will ensure a proper air curtain is installed/maintained in order to control climate and pests.

2.7.3.8 The Government will provide all dumpster(s) to be located in the rear of Cressman. The Contractor shall provide two (2) 80 gal or greater trash receptacles at each shelter entry/exit. Note the Government will be responsible for all food service hygienic sanitation responsibilities including trash removal.

2.7.3.9 The Government will be fully responsible for all food service operations including all housekeeping/cleaning.

2.7.3.10 The Contractor shall provide one 10 pound,4A: 60B: C rechargeable fire extinguisher for each covered shelter.

2.7.4 Temporary Refrigerated, Freezer & Dry Food Storage Units: The Contractor shall provide temporary lockable walk-in food storage facilities complete with temporary utilities for frozen, refrigerated, and dry food storage as described on the Temporary Construction Site Plan, Attachment B.

Approximate square footage requirements are 320 Sq. Ft. for freezer space with 40 in doorway and loading ramp, 320 Sq. Ft. for refrigerated space with 40 in doorway and loading ramp, and 500 Sq. Ft. for dry storage with 40 in doorway and loading ramp. The Contractor shall provide all site improvements or modifications as required to accommodate installation of temporary food storage facilities including exterior lights above all front doorways. Temporary food storage facilities shall be designed and constructed for exterior use without need for additional work to protect them from the weather. Materials used for interior construction of shall meet all applicable health requirements and shall be easy to maintain and clean. Interior lighting and exterior lighting at the entrance doorways shall be provided. Temporary food storage facilities shall be installed and in operation prior to closure of Western Winds. The Contractor shall provide shelving to meet requirements for full utilization of storage space and shall be made of materials compatible with food storage and the storage environment. Temporary food storage facilities shall be skid mounted rather than trailer mounted units to avoid the need for elevated docks, ramps, and stairways for access. The Contractor shall provide temporary ramps, landings, and walkways as required for efficient access to the units. The

SECTION 01010 Page 6 dry storage facility shall have a fly fan above the main entry door and a fire extinguisher mounted on the interior wall. The Contractor shall not be responsible for moving any food or food preparation products. At completion of project after Western Winds is back in operation, the Contractor shall remove temporary refrigerated and dry storage units and restore installation site to same or better condition than prior to installation.

2.7.5 Temporary Construction Management Plan (GA): The Contractor shall submit a management plan outlining the Phase 1 work, the Contractor’s methods of achieving work shall be provided within 7 calendar days after construction Notice to Proceed (NTP) for Contracting Officer approval.

Contracting Officer must approve this submit plan before Contractor can commence any temporary construction work. This submittal shall also include all product information including the proposed shelter(s), storage, refrigerators, freezers and loading ramp.

2.7.6 Upon completion and acceptance of all temporary construction work.

The Government will have 15 calendar days lead time to transition from the old facility (Western Winds) into the modified Cressman Dining facility. No demolition at Western Winds can occur/start during these 15 calendar days.

The Contractor shall move all identified (see Attachment “A” for inventory list of all furnishings out of building 334 (Western Winds) during the 15 day lead-time.

2.8 PHASE 2: Moving/Storage for Relocation to Cressman: The Contractor shall move all identified items, refer to Attachment “A”, furnishings and equipment out of building 334 (Western Winds) during the 30 day lead-time.

The Contractor shall be responsible for moving/storage and protection of all furnishings/equipment and other items, exclusive of electronics, and personal items as required. The Contractor shall, with the Contracting Officer’s Designated Representative, inventory all furnishings, equipment and other items, and make note of any large scratches, dents, missing hardware, and other existing defects/damage. The Contractor shall protect and clean all items. Damage caused to equipment and furniture or other Government property as a result of Contractor operations shall be repaired or replaced at no additional cost to the Government. Contractor shall replace or repair all furnishings to its original location unless directed otherwise by the Contracting Officer’s Designated Representative. The Contractor shall block open the refrigerator to allow the passage of air in order to prevent mildew. Refer to Attachment “A” inventory list for special handing instructions. There is no available on-base storage.

2.8.1 The Contractor shall move/transport/off-load all long term furnishings/equipment to an approved designated local Contractor provided storage location. The Contractor shall move/transport/off-load all short term furnishings/equipment to the location shown on the temporary construction site plan. Note, short term storage is provided to enable the Government to easily access furnishings/equipment during the contract renovation while long term storage is intended to be in storage for the duration of the contract. There are no short terms or long term storage facilities available at Goodfellow AFB.

2.8.2 The Contractor shall be fully responsible for storing and preparing materials in the manner specified herein, or by the manufacturer’s instructions, so that materials are properly suited for installation. Off-site acclimatization of materials shall be at no additional cost to the

Government. All refrigerator(s) shall be properly stored and sanitized to avoid health risks when used once construction is finished.

2.8.3 Moving, replacement, disassembly, and re-assembly of all furnishings and kitchen equipment shall be accomplished by firms or persons possessing prior experience in this field of expertise. The Contractor shall have either the required experience or a firm agreement with a Sub-Contractor who possesses the required experience. Under no circumstances shall any demolition occur until all moving has been completed and all remaining items have been suitably protected.

2.8.4 All electronic/communication equipment at Western Winds will be solely the responsibility of the Government.

2.8.5 The Contractor shall provide dust covers and protect all bulk items that cannot be moved and will remain in place at Western Winds. Refer to Attachment “A”, for list of items to remain requiring protection. Also, the Contractor shall provide all necessary covers to protect all remaining facility items which are to remain in-place.

2.8.6 The Government will have 15 calendar days lead time to transition from the Western Winds into the new temporary facilities. During this 15-day transition time the Government will relocate consumables/food from Western Winds. No demolition can occur/start during these 15 calendar days.

Under no circumstances shall the Contractor be responsible for the transfer of food.

2.8.7 Management Plan (GA): The Contractor shall have a management plan outlining the Phase 2 work.

2.8.8 After Western Winds is vacated and prior to Phase 3 NTP, starting construction operations, the Contractor shall fence-in and secure the lay down area. The lay-area site shall be the parking lot south of Western Winds, reference Division 1 Specification Section 01010. The lay down area shall provide for office space and material storage. New materials shall be secured and protected in this area. This fence shall be minimum 6’-0” high portable chain link with portable interlocking panels (no concrete footings) pedestrian gate(s), and vehicular gate(s). Any existing site improvements altered by the presence of the fence shall be restored to their original condition and appearance. All Contractor storage areas shall be maintained including yard/grass regularly cut/trimmed by the Contractor. All Contractor trailers shall maintain a professional appearance. The Contractor shall at all times maintain accessibility, including parking, to the Medical Admin facility B246 for those who work there. Refer to Attachment “D”

2.9 PHASE 3: Phase 3 shall proceed with and complete all work demolition and construction as described in the specifications and on the plans. Note, there are no facility security systems in Western Winds.

2.9.1 The Contractor shall provide all necessary support to protect all existing interior floors and other nearby spaces/areas from paint spills and other damages. Also, the Contractor shall provide all necessary dust covers to protect all remaining furnishings identified to remain in-place. Refer to Sheet ASH-602 for Floor Finish Schedule.

2.9.2 All interior spaces shall be properly ventilated to comply with all applicable OSHA Regulations, and the Contractor's safety plan required by this contract specification. The Contractor shall minimize dust particles during construction operations to the best of his ability.

2.9.3 The new installed HVAC systems shall not be used to heat/cool Western Winds during construction. Reference ASHRAE 52.2 for additional information.

2.9.4 All facility cleaning should be finalized prior to HVAC flush-out.

Final test and balancing should be completed and HVAC controls functional prior to return of occupants into Western Winds.

2.9.5 Refer to Attachment “E“, Project Closeout Checklist. The Contractor shall complete this checklist prior to the completion of Phase 3. The Government will only accept Beneficial Occupancy Date (BOD) after approval/acceptance of all checklist items, unless otherwise noted as not applicable to the work. For each Phase regarding all drinking water disruptions, refer to Utilities Specification Section 01020, para. 3.3.

2.9.6 After acceptance of the work as part of the final construction inspection punchlist items, the Contractor shall remove all protective floor coverings.

2.10 PHASE 4:

2.10.1 The Contractor shall, with the Contracting Officer’s Designated Representative, perform a return inventory of all storage furnishings and other items using the previously completed exit inventory documentation.

2.10.2 At the completion of the project, after Western Winds is back in operation, the Contractor shall remove temporary covered shelter(s) and utilities, and restore temporary construction site and the construction staging area to same or better condition than prior to covered shelter installation. The Contractor shall deliver all crated covered shelter equipment to cargo storage facility B736. The Contractor shall hot patch repair all asphalt parking lot pavements that have been disturbed by this contract.

2.10.3 Management Plan (GA): A management plan outlining, the Contractor’s methods of achieving the Phase 4 work shall be provided within 30 calendar days after start of Phase 3 work. Contracting Officer must approve this plan before Contractor can commence any site demobilization/ or demolition work. The management plan shall include but not be limited to, demolition plan, demobilization plan, utility outages, and dumpster locations.

2.10.4 CLIN 0011 Parking Lot Lighting: If awarded, this work

shall accomplished in Phase 4.

3. WORK SCHEDULE:

Working hours for the Contractor will normally be between the hours of 7:30 A.M. and 4:30 P.M. excluding Saturdays, Sundays, and Federal holidays. AETC will have Family Days(Off Days) that are in conjuntion with Federal Holidays. The dates are a follows 27 Nov 15, 24 Dec 15, 31 Dec 15, 27 May 16, 05 Jul 16, 02 Sep 16, 25 Nov 16, and 27 Dec 16. If the Contractor desires to work during periods other than above, a request must be made to the Contracting Officer in writing four (4) calendar days in advance of his/her intention. If the required base personnel are reasonably available, the Contracting Officer may authorize the Contractor to perform work during periods other than normal duty hours/days.

4. SAFETY AND HEALTH:

4.1 All Contractor operations shall be conducted and performed in accordance with Department of Labor, OSHA requirements found in 29 CFR 1910 and 29 CFR 1926, Air Force Instruction 91-203, AFOSH standards including but not limited to ANSI, NFPA, ASTM, and CPSC as applicable, and handbooks, and the Corps of Engineers (COE) Safety Manual 385-1-1 all editions in effect on the date of the solicitation. The Contractor shall comply with all mandatory requirements for lockout/tag out, confined space, hazard communication, fall protection, personnel protection equipment, scaffolding, powered construction equipment, and all project related safety and health requirements.

4.2 All companies who conduct business within the state of Texas must, in accordance with Texas Worker’s Compensation laws (Texas House Bill 62), have an approved company safety policy and an Accident Prevention Plan. The plan, approved by the Texas Worker’s Compensation Commission (TWCC) and meeting all of the requirements of the COE Safety Manual, shall be submitted For Information Only (FIO) in accordance with paragraph 16, SUBMITTAL

REQUIREMENTS.

4.3 All holes/pits/trenches/manway openings, etc, that are to be left open shall be surrounded with a 48 inch high mesh fence with highly visible orange plastic coating with warning signs posted “Keep Out” and blinking light from dusk to dawn. Guardrails, fences, barricades, and warning lights or other illumination shall be provided in roadwork construction or near vehicular traffic areas. Fence shall be securely anchored with tension wires and posts as required to prevent sagging and located a minimum of 3 feet from the opening so as to prevent an individual, should they fall across the fencing, from falling into the opening. All exposed trenches/holes shall also be covered, when not being worked in, with three quarter inch plywood or a metal grating that will prevent anyone from entering the trench/hole.

4.4 Radiation Permits and Authorizations: Civilian Contractors or any other agency bringing a radioactive device, e.g., soil or asphalt density

SECTION 01010 Page 10 meter, on Goodfellow AFB must have prior approval from the Base Radiation Safety Officer (RSO). An application to bring a radioactive device on Goodfellow AFB shall be forwarded to the Base RSO through the Contracting Officer at least 7 days prior to the anticipated use. The application package shall include:

a. Copy of the appropriate Nuclear Regulatory Commission License or State Permit to operate or own a radioactive device.

b. Copy of the operator’s qualifications and radiation safety training.

c. Radiation dosimetry results for the operator for the prior calendar year, if applicable.

d. Statement of the expected storage and security requirements or other particular needs of the Contractor.

e. Copies of the last two leak tests, as required.

f. Under no circumstances shall an unlicensed radioactive device be used on Goodfellow AFB. Any questions on Contractor’s responsibilities regarding this requirement should be directed to the Base RSO, 17 MDOS/SGOAB, 325-654-3126, prior to the Contractor starting work.

4.5 Confined Space Entry:

All operations involving entry into confined spaces are performed by the Contractor and shall meet the requirements of OSHA 29 CFR 1910.146, AFOSH Std 91-25 (may be viewed at the website http://www.e-publishing.af.mil//shared/media/epubs/AFOSHSTD91-25.pdf), shall be briefly described during the pre-construction meeting with the Contracting Officer, and specifically comply with the following:

a. All entry supervisors, attendees, and confined space entrants shall have been properly trained in the safety hazards, proper use of Personal Protective Equipment (PPE), entry procedures, and self-rescue. Records of this training must be readily available.

b. Entry supervisors shall maintain a Master Entry Plan (MEP) consisting of:

(1)Descriptions of confined spaces to be entered including location, classification, and acceptable entry conditions.

(2)Designation of authorized entry supervisors, entrants, and attendees.

(3)Identification of the types of tasks to be performed in the confined space including duration.

(4)Procedures for entry and emergency rescue.

(5)Identification of Personal Protective Equipment (PPE), communication equipment, rescue equipment, testing equipment, and monitoring equipment; conditions under which they will be used; and verification of condition of equipment.

(6)Designation of frequency and type of atmospheric monitoring.

(7) Designation of controls required (e.g., lockout/tagout, ventilation, etc.).

(8) Procedures for communication during confined space operations.

c. All AFOSH Std 91-25, chapter 7 requirements will be met and documented. If both Contractor and Government will be accomplishing confined space entry, all procedures (permits required, operations plan, and procedures) shall be documented by the Entry Supervisor prior to operations. The Government will brief known hazards and the Fire Chief will approve or disapprove rescue.

d. All confined spaces shall be tested by a qualified person using a properly calibrated monitor for percent oxygen, lower explosive limit (LEL), and toxicity each time before entry and periodically during operations that have the potential to alter atmospheric conditions.

4.6 Welding, Cutting, and Brazing: Inspection of all welding, cutting, and brazing operations shall be completed by Fire Protection prior to any operation. The Contractor shall provide the appropriate operable fire extinguisher. Fire extinguishers shall comply with NFPA 10. Contractor shall comply be with OSHA STD29 CFR 1910.252 Welding, Cutting, and Brazing (General Requirements) and AFOSH 91-5 Welding, Cutting, and Brazing, which may be viewed at the following website: http://www.e-publishing.af.mil/shared/media/epubs/AFOSHSTD91-5.pdf. Air Force Form 592 USAF Welding, Cutting, and Brazing permit will be issued prior to any operation and shall be kept on site till completion of operation or permit expires. Contact Fire Protection at 325-654-3532/33/34 for issuance of permit.

4.7 Fire Alarm System: The installation of fire alarm systems must be certified by a licensed professional possessing a current Texas Alarm Certificate of Registration (ACR). The installation of fire sprinkler systems and hood and duct fire systems must be certified by a licensed professional possessing a current Texas Fire Sprinkler Certificate of Registration. A copy of these certificates shall be submitted. Prior to performing any work or disconnecting or shutting off any fire alarm, fire sprinkler system, or hood and duct system, the Civil Engineer Alarms shop at 325-654-3436 and Fire Protection 325-654-3532/33/34 shall be notified by the Contractor. After completing work on any fire protection alarm system, sprinkler, or commercial cooking suppression system, the proper completion documents and/or inspection and test documents, shall be completed and submitted to the Contracting Officer, that comply with NFPA 72, NFPA 13, NFPA 96, or NFPA 24 as applicable.

4.8. Base Fire Regulations: The Contractor shall comply with Base Fire Regulations as set forth in GAFB Instruction 32-2001, titled “Base Fire Prevention Program” in effect as of the date of this solicitation. All work shall be in strict compliance with NFPA-101. The Contractor shall use no explosives or fire in performing the work.

5. STREET CLOSINGS: This paragraph not used. Project will not require street closings.

6. DISPOSITION OF WASTE AND EXCESS MATERIALS:

6.1 The Contractor shall make waste determinations for all wastes generated in the performance of this contract, in accordance with the provisions set out in 40 Code of Federal Register (CFR) Part 261, at the time and point of

The Contractor shall properly sample, analyze, or by use of process knowledge classify all wastes in accordance with Title 30 Texas Administrative Code (TAC), Chapter 335, Subchapter R at no additional cost to the Government. Unless the Contractor can demonstrate that a waste is non-hazardous, the Contractor shall manage the waste as a hazardous waste until sample test results prove otherwise. All non-hazardous wastes, special wastes, and hazardous wastes (including but not limited to construction debris, material containers, material residues, and unwanted excess materials) resulting from the performance of work under this contract shall be removed from and disposed of off Goodfellow AFB by the Contractor at no additional cost to the Government and in accordance with all applicable Federal, State, and local laws, rules, and regulations. Under no circumstances shall the Contractor dispose of wastes or excess material in trash dumpsters, storm sewers, sanitary sewers, creeks, streams, or other property of Goodfellow AFB. The Contractor and the Government will be co-generators of all wastes resulting from the performance of this contract.

Refer to Section 01400-Environmental Protection, paragraph 3, DISPOSAL OF WASTES, for special requirements on disposal of waste types.

6.2 Dumping/cleaning out of concrete trucks on Goodfellow AFB is prohibited. Concrete truck chutes only may be rinsed at the construction site. Wastewater and concrete from this rinse shall be collected in a high-density polyethylene (HDPE) plastic-lined box or pit provided by the Contractor at the site. At the end of pouring operations, the Contractor shall excavate all the waste and liner and properly dispose of same. The pit shall be completely backfilled and the site restored to original conditions.

6.3 Upon issuance of NTP for Phase II, the Government will provide a list of equipment and materials to be submitted for turn-in. All equipment and materials to be removed from the project site not specifically identified for turn-in to the Government shall become the property of the Contractor upon issuance of the Notice to Proceed (NTP). The Contractor shall turn-in all materials specifically designated for turn-in to the Government to a location at Goodfellow AFB, as designated by the Contracting Officer. The Contractor shall obtain a receipt from the Government employee responsible for receiving the returned equipment or material as evidence of compliance.

A copy of the receipt(s) shall be submitted to the Contracting Officer prior to final inspection of the project. Following is a list of equipment or materials to be turned-in.

7. STORAGE AREA (I.E. TEMPORARY FIELD OFFICE, STAGING AREAS, TOOL/JOB

SHACKS, AND OTHER CONSTRUCTION FACILITIES):

There are no Government furnished covered or secure storage areas. Limited storage may be permitted at the discretion of the Contracting Officer and on a space available basis. The location on Goodfellow AFB of the Contractor’s temporary field office, storage, and other construction buildings required temporarily in the performance of the work, shall require written approval of the Contracting Officer. Plans showing temporary field office, storage, and other construction buildings shall be submitted for Government Approval (GA). Utilities at the storage area may or may not be available for Contractor use. The Government implies no responsibility for lost or stolen materials, equipment, or tools, the security of which lies solely with the Contractor. Contractor shall keep his storage areas clean, neat, and orderly. Contractor shall mow grass and weedy vegetation when it reaches a height of 6 inches. Mowing shall be to a height of 3 inches. Mowing shall

SECTION 01010 Page 13 be accomplished with a rotary mower that leaves the clippings evenly distributed on the soil surface. Mowing shall be accomplished during periods and in a manner that the soil and grass will not be damaged. Towed or self-propelled riding mowers shall not be operated within 3 feet of shrubs or trees. Contractor shall mow areas adjacent to shrubs and trees with hand propelled mowers. Temporary fencing used by the Contractor to delineate construction sites shall be securely anchored with tension wires and posts as required to prevent sagging and an unsightly appearance.

Fencing shall be maintained by the Contractor in this manner throughout the life of the contract. Due to high winds in west Texas, Contractor shall take every precaution to preclude trash and materials from blowing off site.

8. TOILET FACILITIES: There are no toilet facilities available for Contractor use. Contractor shall provide his own portable/temporary toilet facilities.

9. CLEAN-UP:

The Contractor shall at all times keep the construction site, construction trailer(s)/building(s), and storage area(s) in a clean, neat, workman like condition, free from accumulation of waste, rubbish, weeds, overgrown grass, or construction debris, to the satisfaction of the Contracting Officer. All loose or light weight materials shall be secured to prevent blowing or scattering. The burning of trash or construction debris is strictly prohibited on Goodfellow AFB. Prior to final inspection, the Contractor shall remove all construction debris, tools, equipment, and materials not the property of the Government. Upon completion of the work, the Contractor shall leave the work site and storage area(s) in a clean, neat, and workmanlike condition satisfactory to the Contracting Officer. Refer to Section 01400 - Environmental Protection, paragraph 2.4.6, Post-Construction Cleanup or Obliteration.

10. FINAL INSPECTION:

The Contractor shall advise the Contracting Officer in writing of the Contractor’s desired final inspection date seven (7) calendar days in advance of that desired date to permit proper coordination. The date selected shall provide adequate time for Contractor performed corrections of final inspection deficiencies within the contract performance time. The Contracting Officer will be the final authority for determining whether or not the Contractor’s performance is sufficiently advanced to warrant a final inspection.

11. TESTING:

11.1 Costs of all tests, unless specifically indicated as being performed by the Government, shall be at the Contractor’s expense. The Contractor shall schedule all tests and notify the Contracting Officer or his/her representative in a timely manner prior to any required testing. All test results shall be submitted to the Contracting Officer on AF Form 3000, Material Approval Submittal. Where test reports are to be submitted to the Contracting Officer within 24 hours after the tests are performed, the results may be faxed to the Contracting Officer at a number provided at the preconstruction conference.

11.2 Testing laboratories must be licensed to operate in the State of Texas and must meet the following:

- “Recommended Requirements for Independent Laboratory Qualification”, published by American Council of Independent Laboratories

- Basic requirements of latest edition of ASTM E329 “Standard Recommended Practice for Inspection and Testing Agencies for Concrete, Steel, and Bituminous Materials Used in Construction”

- Latest edition of ASTM E 548, “Qualifications for Testing Labs”

12. AS-BUILT DRAWINGS AND REAL PROPERTY DOCUMENTATION:

12.1 As-Builts (GA):

a. The Contractor shall maintain two sets of project drawings with red-line “as-built” notations and markings. Prior to the final inspection, the Contractor shall transfer these red-lined changes to a CADD format that is fully compatible with Autodesk’s AutoCAD version 2000 or greater. The Contractor shall submit one full size hard copy set of as-built maps along with electronic copies in pdf and CADD formats. In addition, the Contractor shall develop and submit GIS data sets on all improved areas. The government will provide the contractor with a copy of Goodfellow’s most current data sets. These data sets shall be compatible with ArcGIS v9.3 format and shall have a minimum accuracy of one meter. The coordinate system for both AutoCAD and GIS files shall be in WGS 1984, UTM Zone 14. The Contractor shall submit two copies of electronic files on a CD or DVD under the cover of an AF Form 3000. The CD or DVD label shall contain, at a minimum, the following information: (1) Brand and version of the CADD software used to generate the drawing files; (2) Short description of the contents including a cross reference of the drawing file names on the CD or DVD and the project drawings sheet sequence numbers or sheet titles; (3) Statement marking the CD or DVD as “as-built” drawings;

and (4) any directions required to open files.

b. The Contractor shall submit an interim/draft DD FORM 1354 (GA) with all relevant data 30 days prior to the final inspection and the final DD FORM 1354 (GA) shall be provided at the project final inspection as prescribed in UFC 1-300-08. Additional information/instructions on completing the DD FORM 1354 will provided by the Government at the pre-construction meeting.

12.2 Instruction Manuals:

Required instruction manual(s) shall be provided in three-ring binder(s) with tabs and an index/table of contents (FIO). Provide permanent label on front and side with project title, project number, facility number, street address, Contractor/subcontractor name, address, phone number(s), and manual title/contents description. Include all wiring diagrams and parts lists.

13. SECURITY REQUIREMENTS:

13.1 Goodfellow Air Force Base is a closed/controlled access base. As an effort to minimize peak traffic flows at the entry gates, the Contractor, his employees, and his subcontractors are highly encouraged to avoid

SECTION 01010 Page 15 scheduling deliveries through the entry control gates from 7:00 A.M. to 8:00 A.M. Monday through Friday as there could be significant delays.

The Government will conduct security/background checks on all construction personnel as required. Security requirements will be briefed at the pre-construction conference and handouts outlining specific requirements will be provided. Screening measures are in place and suitability for employees to access the installation will be determined during the conference.
