ADDENDUM TO 52.212-1 INSTRUCTIONS TO OFFERORS--COMMERCIAL ITEMS (OCT 2015)

1. GENERAL INFORMATION

1.1. To ensure timely and equitable evaluation of proposals, Offeror(s) must follow the instructions contained herein. Offeror(s) are required to meet all solicitation requirements, including terms and conditions, representations and certifications, and technical requirements, in addition to those identified as evaluation factors or subfactors.

Failure to meet a requirement may result in an offer being ineligible for award. The response shall consist of two (2) separate volumes; Volume I - Price Proposal (SF 1449) and Volume II – Technical Proposal.

1.2. The proposal shall be clear, concise, and include sufficient detail for effective evaluation and for substantiating the validity of stated claims. The proposal shall provide convincing rationale to address how the offeror intends to meet these requirements. The Government will base its evaluation solely on the information presented in the offeror's proposal.

1.2.1. Proposals must demonstrate a comprehensive understanding of the nature and scope of work required. The Government will not search for data to cure problems or address inconsistencies in proposals. Each offeror shall submit a complete proposal in accordance with these instructions. Statements such as "will comply," "noted and understood," "in accordance with industry best practices/standards," etc. without supporting narrative are not acceptable. Cursory responses or responses that merely reiterate or paraphrase the Performance Work Statement (PWS) language will not be considered to satisfy the requirements of the RFP.

1.3. The contracting office Points of Contact (POCs) are the sole POCs for this acquisition. Questions pertaining to this RFP, or any of its attachments, shall be submitted via e-mail to all of the addressees below by the due date in AFFAR Provision 5352.215-9001(c).

1.4. Proposals must be submitted either in electronic or paper format as specified in Item 2, PROPOSAL PREPARATION INSTRUCTIONS. Telegraphic or facsimile offers will not be accepted. The sealed envelope or package used to submit your proposal must show the time and date specified for receipt, the Solicitation Number, and the Offeror’s POC name, address, and phone number.

1.5. Solicitation Documents and Additional Information. All referenced documents for this solicitation are available on the Federal Business Opportunities (FedBizOpps or FBO) website at https://www.fbo.gov/. Potential Offerors are encouraged to “register to receive notifications” and add this solicitation to their “Watch List” for real-time email notifications of information updates posted to the website regarding this solicitation. Additional information will be posted on FedBizOpps as applicable.

2. PROPOSAL PREPARATION INSTRUCTIONS

2.1. Submission of offers. Submit signed and dated offers to the office specified in this solicitation on or before the exact date and time specified in this solicitation. Offers shall be submitted using the Standard Form (SF) 1449 and Attachment 6 “Pricing Tool”. Please use Attachment 6 “Pricing Tool” in lieu of filling out each CLIN on the SF 1449. All other items of the SF 1449, with the exception of CLIN pricing, shall be filled out. In the “Pricing Tool” spreadsheet, only enter information into the “Price per Quantity” column on each “year” sheet (cells E4-29). The total price will be automatically calculated using pre-set formulas.

2.3. Binding and Labeling: Each offeror shall submit one Original and one copy of its proposal using three-ring binders. Volumes I and II may be submitted together within the same binding. A cover sheet should be placed at the beginning of each volume clearly marked as to whether it is the original or copy, and displaying volume number, title, solicitation identification, and the Offeror's name. Electronic files must also be labeled in the same format.

Electronic submission have a 5MB size limit, if this size limit is exceeded the email attachment will be removed from prior to the POCs receiving the email and will not be considered for evaluation as it will be considered an incomplete submission.

2.4. Page Format Restrictions and Limitations:

2.4.1. Page Format: Pages shall be 8.5 x 11 inches. Pages shall be typed. Except for the reproduced sections of the solicitation document, the font size shall be no less than 12 point Times New Roman. Margins on all four edges of each sheet will be at least one inch. Any proprietary statements, security markings, and page numbers can fall within the defined margin area. Pages shall be numbered sequentially by volume.

2.4.2. Page Count Limitations: Volume I will be the competed SF 1449 and Attachment 6 “Pricing Tool”. Volume II shall contain no more than 50 pages. Compliance with page count limits will be verified. If exceeded, the excess pages will not be read or considered in the evaluation of the proposal and will be destroyed as soon as practicable.

2.4.3. Indexing: Each volume shall contain a detailed table of contents to delineate the subparagraphs within that volume. Tab indexing shall be used to identify sections. The Table of contents and Index pages will not be counted towards the total page number for Volume II.

2.5. Documents submitted in response to this RFP must be fully responsive to and consistent with the following:

2.5.1. Requirements of the RFP (including Contract Line Item Numbers (CLIN) and Performance Work Statement (PWS) and Government standards and regulations pertaining to the PWS.

2.6. Electronic Format for All Proposal Volumes. If submitting electronic versions of proposals (5MB limit), Offerors should follow these instructions:

2.6.1. If submitting documents using Microsoft Office:

• Word documents – Set "Save as type" to "Word Document (*.docx)" only.

• Excel spreadsheets – Set "Save as type" to "Microsoft Excel Workbook (*.xlsx)."

• PowerPoint presentations – Set "Save as type" to "Microsoft PowerPoint 2010 Presentation (*.pptx)."

2.6.2. If submitting PDF documents, then use only Adobe Acrobat 9.0 or later. DO NOT submit any documents in PDF format that are copied as “images.” When creating PDF files, always create to enable textual search and copy functions.

3. SPECIFIC INSTRUCTIONS

3.1. Volume I – Price Proposal: Volume I shall be prepared IAW the instructions outlined in this solicitation and will be evaluated in accordance with the evaluation criteria in FAR Provision 52.212-2 as set forth herein.

3.1.1. Price Schedule: The offeror shall propose and insert a dollar unit price, for each CLIN of the Base Period and each Option Period on SF 1449. The sum total of all CLINs, for the Base Period and each Option Period, will form the offeror’s Total Evaluated Price (TEP). The proposed prices must be based on the offeror’s own technical approach submitted in the Technical Proposal (Volume II) and the Government’s PWS requirements.

3.2. Volume II – Technical Proposal: Volume II shall be prepared IAW the instructions outlined in this solicitation and will be evaluated in accordance with the evaluation criteria in FAR Provision 52.212-2 as set forth herein. Using the instructions provided below, provide a detailed approach to accomplish/satisfy all subfactors. Do not merely reiterate the objectives or reformulate the requirements specified in the RFP or PWS. By submitting a proposal, the offeror affirms that its organization will perform all requirements specified in the RFP and PWS. Volume II will consist of three “Tabbed” Sub-Factors, each of which will be assigned a rating of Acceptable or Unacceptable. A rating of Unacceptable in one or more Technical Sub-Factors will constitute an overall rating of Unacceptable for the Technical Factor. An offeror’s approach must clearly demonstrate its capability of and capacity to perform grounds maintenance by providing all information pursuant to Grounds Maintenance Plan, Quality Control Plan, and Management Control Plan as outlined in the Technical Sub-Factors in accordance with the Performance Work Statement (PWS) and Quality Assurance Surveillance Plan (QASP) that will be used in the performance of this contract (PWS and QASP are included as attachments to this solicitation). Volume II shall contain the following:

3.2.1. TAB 1 - Executive Summary – The Offeror shall provide an executive summary which describes its plan to provide an effective approach to Grounds Maintenance, Quality Control, and Management Control. The Offeror shall state its understanding of the entire requirement over the life of this contract. The Offeror’s executive summary shall also include the Offeror’s approach for working with other contractors in support of the 14 FTW mission.

3.2.2. TAB 2 - SUB-FACTOR 1 – Grounds Maintenance Plan: The Contractor shall prepare a Grounds Maintenance Plan. At a minimum, the plan shall describe how and when the tasks listed in the Service Summary (SS) of the Performance Work Statement shall be accomplished. A typed copy of the plan shall be submitted in response to this Sub-Factor.

3.2.2.1. TAB 2A - Approach to provide Grounds Maintenance services. This includes a detailed mowing and other grounds maintenance schedule for the base period of the contract. This schedule shall be in table format and address the requirements detailed in the Performance Work Statement (PWS); specifically, Appendix A, Grounds workload Estimates and Appendix B, Maps. The approach shall specify the days of the week, areas to be maintained, frequency of maintenance, and number of personnel to be working in each area. This schedule will be used for purposes of evaluation, but it should not be confused with the final schedule, which will be submitted by the successful offeror after award in accordance with the PWS.

3.2.2.2 TAB 2B – Chemical agents. The Contractor shall include its intended use of all chemical agents within this plan and shall specify what weed control methods (i.e., non-selective systemic herbicide use, selective systemic herbicide use, pre-emergent herbicide use, and/or mechanical weed control using shovels, hoes, etc.) shall be implemented. Special attention shall be placed on the following items:

• Athletic Area fertilizer application program

• Temporary Living Facilities (TLFs) maintenance plan

• Chemical trimming / Use of plant growth regulators

• Using herbicide in drainage ditches

3.2.2.3. TAB 2C – Mowing schedule. The proposal shall also include a proposed mowing schedule for all areas to include the airfield areas.

3.2.2.4. TAB 2D - Green procurement. List any type(s) of products to be used to perform grounds maintenance services.

3.2.3.5. TAB 2E – Process(es). Special attention shall be placed on the following items. Describe processes to be used for:

• Temporary Living Facilities (TLFs) maintenance plan

• Leaf removal process

• Prune/trim shrubs

• Managing Foreign Object Debris (FOD) on Airfield

• Maintaining Wet Areas on Airfield

• Access/Driving/Communications on Airfield

• Use of boom mower

• Using herbicide in drainage ditches

• Tree care and removal

3.2.2.6. TAB 2F- Changing requirements. Approach for adapting to new and/or changing requirements and workload (short and long term).

3.2.3. TAB 3 - SUB-FACTOR 2 – QUALITY CONTROL PLAN: The Contractor shall prepare a Quality Control Plan. At a minimum, the Plan shall provide a written description of Quality Control methods to include identification of the commercial practices and procedures to be used. The Plan should address procedures to identify, prevent, and correct deficiencies in performance regarding Service Summary (SS) items. The Plan shall specifically address strategies to provide quality workmanship, continual process improvement, and correction of deficiencies as appropriate.

3.2.3.1 TAB 3A - The Offeror’s Inspection Plan to include Self Inspection Methods and Documentation.

3.2.3.2. TAB 3B - The Offeror’s Hazardous Materials Handling Plan (i.e. chemicals and cleaning agents).

3.2.4. TAB 4 - SUB-FACTOR 3 – MANAGEMENT CONTROL PLAN: This subfactor evaluates the Offeror’s plan to provide grounds maintenance services. The plan shall provide a written description of Management Control methods to include required credentials and experience for Key Personnel.

The following Key Personnel must be included:

*Project Manager/On-Site Superintendent (NOT TO BE USED AS LABORER) *Quality Control Manager/Safety Officer (NOT TO BE USED AS LABORER)

3.3. Past Performance Information: Offerors shall submit the Past Performance Questionnaire (PPQ), Attachment 5, to a maximum of five sources. PPQ must be submitted to this office directly from external sources; PPQ submitted directly from offerors will not be accepted. Deadline for submission of PPQ by sources is the same as the due date for proposals as outlined in Block 8 of this SF 1449.

3.4. Additional Items:

3.4.1. Proposals shall remain valid for a period of 180 calendar days from the date of receipt of offers.

3.4.2. Compliance with these requirements is mandatory; failure to comply may render proposals ineligible for award.

52.212-2 EVALUATION--COMMERCIAL ITEMS (OCT 2014)

(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:

1. BASIS FOR CONTRACT AWARD

1.1. This acquisition will be evaluated using the Lowest Price Technically Acceptable (LPTA) method. The best value to the Government is expected to result from selection of the technically acceptable proposal with an acceptable past performance and lowest evaluated price. To be considered for award, an Offeror’s proposal must conform to all solicitation requirements and the Offeror must be deemed responsible. While the Government will strive for maximum objectivity, the evaluation process is subjective by nature and, therefore, professional judgment is implicit throughout the entire process.

1.2. The Government intends to award a contract without discussions with respective Offerors; therefore, the initial offer should contain the Offeror’s best terms from a price and technical standpoint.

1.2.1. An Offeror may be asked to clarify certain aspects of its proposal (e.g., the relevance of past performance information) or respond to adverse past performance information to which the offeror has not previously had an opportunity to respond. Exchanges conducted with regard to adverse past performance or to resolve minor or clerical errors will not constitute discussions, and the Government reserves the right to award a contract without the opportunity for proposal revision.

1.2.2. For the purpose of conducting evaluations, the Government will first rank all proposals by Total Evaluated Price (TEP) and then proceed with evaluation of the Technical Proposal (Volume II) and Past Performance of the Offeror presenting the lowest TEP. If the Technical Proposal and Past Performance of the Offeror with the lowest TEP are rated Acceptable and the TEP is determined fair and reasonable, then award shall be made to that Offeror without entering into discussions or conducting Technical or Past Performance evaluations on other proposals, as this represents the best value to the Government.

1.2.3. If the lowest priced proposal does not have acceptable technical and/or past performance or is not deemed fair and reasonable, the Government shall evaluate the Offeror presenting the second-lowest TEP in accordance with Item 1.2.2 above. If the second-lowest priced proposal does not have acceptable technical and/or past performance or is not deemed fair and reasonable, the Government shall evaluate the Offeror presenting the third-lowest TEP (and so forth) until the LPTA offer is identified. The Contracting Officer anticipates a high volume of proposals in response to this solicitation. Written notices shall be provided to unsuccessful Offeror(s) as appropriate.

1.2.4. Though it is the Government’s intent to award without discussions, the Government reserves the right to hold discussions if determined to be in its best interest. If, during the evaluation period, it is determined to be in the best interest of the Government to hold discussions, Offeror responses to Evaluation Notices (ENs) and the Final Proposal Revision (FPR) will be considered in making the source selection decision. If the Offeror’s proposal has been evaluated as acceptable at the time discussions are closed, changes or exceptions in the Final Proposal Revision are subject to evaluation and may render the Offeror’s proposal be determined unacceptable and ineligible for award.

2. EVALUATION FACTORS FOR AWARD

2.1. Evaluation of all offers will be made in accordance with the criteria outlined in this section. The proposals will be evaluated against the following three (3) factors:

Factor 1 – Price

Factor 2 – Technical Sub-Factor 1 – Grounds Maintenance Plan Sub-Factor 2 – Quality Control Plan Sub-Factor 3 – Management Control Plan

Factor 3 – Past Performance

2.2. Proposals will be evaluated for acceptability and will not be ranked using non-price factors/subfactors.

3. EVALUATION OF PRICE PROPOSAL

3.1. The Government will evaluate the price and price information submitted in each Offeror’s price proposal IAW FAR 15.404. Any other data provided by the Offeror with their price proposal that is not required IAW FAR Provision 52.212-1 will not be considered or evaluated.

3.2. Total Evaluated Price (TEP): Each Offeror’s Total Evaluated Price (TEP) will be utilized for award decision purposes. The TEP will be calculated as the sum of CLINs 0001 – 0026 for the base period and all option periods as reflected in the Offeror’s submissions on Attachment 6 “Pricing Tool”.

3.3. The contract will include FAR Clause 52.217-8, Option to Extend Services, with the potential to extend the contract an additional 6 months beyond the awarded contract performance period. In the event the Government requires continued performance, the Government's evaluation of prices proposed for the base and all option periods will suffice to be the Government's evaluation of prices under the extension of services (ref. 52.217-8). Should an extension of services be required, rates will be adjusted only as result of revisions to applicable prevailing Department of Labor wage rates.

4. EVALUATION OF TECHNICAL PROPOSAL

4.1. The Offeror’s proposal will be evaluated for the three (3) Technical Sub-Factors listed below. The Offeror’s proposal must demonstrate a clear understanding of the nature and scope of work required. Failure to provide a complete proposal may reflect a lack of capability to perform the work requirements and may result in a determination that the Offeror’s proposal is “Unacceptable.” Detailed instructions are included in the proposal preparation instructions section of this solicitation. During evaluation of each proposal, the Government will assign each subfactor a rating of either “Acceptable” or “Unacceptable” as shown below in TABLE A-1. NOTE: A rating of “Unacceptable” in one or more Technical Sub-Factors will constitute an overall rating of “Unacceptable” for the Technical Factor.

4.1.1. SUB-FACTOR 1: GROUNDS MAINTENANCE PLAN - This sub-factor will be evaluated to ensure the offeror’s approach clearly demonstrates the capability and capacity for appropriate, efficient and effective execution of Grounds Maintenance Requirements in the performance of this contract

4.1.2. SUB-FACTOR 2: QUALITY CONTROL PLAN - This sub-factor will be evaluated to ensure the offeror’s approach clearly demonstrates the capability and capacity for appropriate, efficient and effective execution of Quality Assurance and Support in the performance of this contract.

4.1.3. SUB-FACTOR 3: MANAGEMENT CONTROL PLAN - This sub-factor will be evaluated to ensure the offeror’s approach clearly demonstrates the capability and capacity for appropriate, efficient and effective Management and Oversight in the performance of this contract.

5. EVALUATION OF PAST PERFORMANCE

The Government will use the following procedures in evaluating past performance:

5.1. Evaluation Process. The past performance evaluation considers an Offeror’s demonstrated recent and relevant record of performance of services that meet the solicitation requirements and how well the Offeror performed on the past/present efforts. In conducting the Past Performance evaluation, the Government reserves the right to use both the information provided in the Past Performance Questionnaires as well as information obtained through the Past Performance Information Retrieval System (PPIRS).

5.2. The offeror’s past performance efforts will be assessed for: Recency and Relevancy. The Government will only evaluate Performance Quality on past efforts that are determined to be both recent and relevant. Performance Quality will be deemed Acceptable for any recent and relevant past performance efforts which feature no ratings below “Satisfactory” or equivalent (e.g. “Marginal” or “Unsatisfactory”).

5.3. Recency Assessment. To be considered recent, performance must have occurred within 36 months of the date this solicitation was issued.

5.4. Relevancy Assessment. To be considered relevant, work performed must consistent with the scope of this acquisition as defined in the applicable Performance Work Statement (PWS), to include projects/contracts with values over $650,000 per year, which include grounds maintenance services.

5.5. Performance Quality. The Government will evaluate recent and relevant past performance information for minimum “Satisfactory” ratings.

Unacceptable Proposal does not clearly meet the minimum requirements of the solicitation.

Note: In the case of an Offeror without a record of relevant past performance or for whom information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the

Offeror may not be evaluated favorably or unfavorably on past performance (see FAR 15.305 (a) (2) (iv)).

Therefore, the Offeror shall be determined to have unknown past performance. In the context of Acceptability/Unacceptability, “Unknown” shall be considered “Acceptable.”

5.6. Failure to receive an “Acceptable” past performance rating will result in the entire proposal being eliminated from the competition.

(b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).

(c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
