STATEMENT OF WORK
PROJECT # MPLS 17-0023
REPAIR - RELOCATE ELECTRICAL
DISTRIBUTION, PHARMACY, B8410
JBSA LACKLAND
1.0 PROJECT SCOPE
1.1 General Requirements: The project consists of providing complete design and construction work necessary for a Design-Build (DB) project. It includes furnishing all materials, labor, and equipment required to relocate an existing high voltage primary duct back from underneath the Lackland Satellite Pharmacy Facility, Building 8410. This facility is currently under renovation and is located in the 8000 area of JBSA Lackland on Dimstead Avenue.
1.1.1 Background: The Lackland Satellite Pharmacy, Building 8410, was constructed in 1991.
The building is currently undergoing a major renovation which includes a 700 SF addition on the western side of the building which will expand capacity and improve the efficiency of the Satellite Pharmacy operation. The building was originally built on a main high voltage power line (duct bank) which supplies the Commissary and banking establishments, and other facilities on JBSA Lackland. This existing duct bank installation violates safety codes with regard to buried power lines and electric duct banks under a foundation. This project will relocate the underground duct bank from under Building 8410, so that the feeder installation will meet current codes and safety requirements.
1.1.2 Existing Conditions: The duct bank that runs under B8410 is a three phase, 13,200/7,620 volts feeder identified as Circuit 5, which originates out of JBSA Lackland Main Primary Switching Station. This circuit runs in an east/west direction and under the footprint of the proposed addition to B8410 and its existing facility. The National Electrical Code and Unified Facilities Criteria prohibit installing a building on top of an existing duct bank. In the interim period, to allow the continuation of the construction project on Building 8410, the power in the existing duct bank has been switched to an alternate base source, which has left the Circuit 5 run underneath the Satellite Pharmacy building un-energized (not live). However, the Using Agency for the Satellite Pharmacy will not be able to take beneficial occupancy of the renovated facility until Circuit 5 has been rerouted from underneath Building 8410, and power to the Satellite Pharmacy is restored on Circuit 5.
1.1.3 Contractor Responsibility: The contractor shall be responsible for coordinating all work efforts with the 502 Civil Engineer Squadron (CES) Project Manager (PM) to provide and maintain safe and secure access and working condition for government personnel that remain through the duration of this project. Upon the receipt of Notice to Proceed (NTP), the contractor shall coordinate the mobilization plan with 502 CES PM to ensure smooth startup. Prior to submitting a proposal for this DB, the contractor shall visit the project site and verify all existing conditions and dimension. Verification includes but not limited to, verifying cable sizes, and surveying existing conditions within manholes and switches in the electrical system. 1.3 SCOPE OF WORK
1.3.1 Provide a new pad mounted 600 amp, 15 KV, 3-way, Sectionalizer Switch Cabinet (J5-01) and intercept the existing high voltage underground 4-way concrete-encased duct bank of feeder circuit 5, which extend underneath the Satellite Pharmacy, Building 8410. The existing 4-way duct bank of Circuit 5 is composed of 4 inch PVC conduits with one set of 3#4/0 15 KV EPR cables, and 1# 4/0 600 volt ground in one of the duct. The housekeeping pad for the New Sectionalizer Switch shall be constructed of 3000lbs concrete and shall be 8 inch thick with 16” x 8” ground footage around the pad. Provide minimum 24” long conduit sleeves (stub-outs) in the pad. The pad shall extend 8” outside the perimeter of the Sectionalizer with a 1.5“ chamfer edge around the pad.
1.3.2 From the new Sectionalizer Switch (J5-01), construct a new two-way duct bank and extend the duct bank line, via new manholes EM 171 A and EM171 B, to the existing high voltage Switch SW 5-04 via existing manhole EM172. The new duct bank shall consist of 2- 4 inch scheduled 40 PVC ducts with 3000 PSI concrete encasement with a minimum of 3.5” around the duct bank and 3” spacer outer of the duct. Install 1 set off 3 # 4/0 copper 15kv cables, 133 insulation level, EPR (Ethylene Propylene Rubber) with 1/3 concentric neutral in one of the ducts, the other duct shall be a spare duct with a pull cord in it. The top of the new ductbank shall be 36” below grade, with a 6 mil warning tape labelled “High Voltage” placed 18” along the duct bank and 18” below grade. The Sectionalizer shall have the ID J5-01 placed on it cabinet enclosure. Color of the Sectionalizer cabinet shall be green to match existing Sectionalizer units installed on the base. The 15kv cables running through manholes shall be fireproofed using 30 Mil Scotch 77 tape and Scotch 69 tape or with approved fireproofing spray. The fireproofing shall

extend a minimum distance of 24” inside the ducts. High voltage potential testing shall be accomplished on the new cable after installation at 65,000 volts.
1.3.3 New manholes shall be of the pre-cast concrete structure type. Strength of manholes shall conform to the requirements of ASTM C478. The minimum interior dimensions of manholes shall be 8ft. X 8ft and 7ft in height. There shall be a minimum of 12-inch X 12-inch deep sump in the floor of the manhole. A ground rod shall be installed at each manhole. The ground rod May be installed so that approximately 4 inch of the ground rod will extend above the manhole floor or the ground rod may be below the manhole floor and a no 1/0 AWG ground conductor brough into the manhole through a watertight sleeve in the manhole wall. Ground rods shall be ¾” x 10’ copper clad steel conforming to UL 467. Manholes covers shall be gray heavy weight cast iron commercial units with the word “Electric” engraved in the cover.
1.3.4 Demolition work in the project shall include, but not limited to, trenching and removing asphalt surfaces for the new duct bank installation, cutting existing duct bank outside the footprint of the Satellite Pharmacy foundation on both ends, east side and west side of the building, removing high voltage cables from the duct bank running under Satellite Pharmacy, and filling duct left in grade and underneath building with grout or cement, and capping ducts at both ends, and abandon inactive duct bank sections in place. The contractor shall patch and repair all pavement, grass, sidewalks cut and removed to install the new duct bank.
Striping of parking lot areas shall be redone where disturbed.
1.4 DESIGN
1.4.1 The contractor shall identify and comply with all applicable (1) federal, state, local as well as Department of Defense (DoD), Air Force, and JBSA - Lackland statutes, instructions, manuals, handbooks, codes, regulations, guidance, policy letters, and rules (including all changes and amendments), and (2) Presidential Executive Orders, in effect on the date of issuance of this contract. It is the responsibility of the contractor to obtain and maintain these books and/or documents for the duration of the project. The Government does not assume the presence of any hazardous materials.
1.4.2 As a minimum design drawings for this project, shall also include location and site maps, a complete routing layout of the new underground primary electrical duct bank, duct bank details, manhole details, Sectionalizer details, duct connection detail from existing duct banks to the new Sectionalizer, duct connection detail from new duct bank to existing manhole, trenching and backfilling details, legend and notes.
1.4.3 The contractor shall be responsible for developing all design documents (i.e. Design Analysis, Plans, and Specifications) to accomplish the provisions of this SOW.
1.4.4 The Designer of Record (DOR) shall use the Unified Facilities Guide Specifications (UFGS) and the requirements contained in this SOW to develop the technical specifications and construction drawings. The technical criteria contained and cited in this SOW and UFGS establish minimum standards for the construction quality.
1.4.5 The contractor shall be responsible for preparing, filing, testing, and paying for any fees

necessary to obtain all necessary permits and tests to meet the provisions of this SOW. All testing and/or permitting fees shall be included as part of the base bid.
1.4.6. Design and installation shall be completed in strict compliance with the most current versions of NFPA National Electrical Code, and with the United States Unified facilities Criteria and component manufacturer’s instructions.
1.4.7 The Contractor shall provide signed and stamped drawings with the original electronic or embossed seal of a Texas licensed Professional Engineer.
1.4.8 A final copy of the 100% design, drawings and specifications shall be furnished at the completion of the design phase of the design-build project.
1.4.9 The contractor shall submit for government review and approval, bound paper copies and electronic copies of 50%, and 100% complete design document packages consisting of detailed design analyses, plans and specifications for the accomplishment of all construction work described in section 1.4 below. Contractor shall submit a signed and sealed i s su e d f o r c o ns t r u c t i on package incorporating all corrections from 50%, and 100% reviews.
Mobilization and construction effort will only be allowed to begin upon the receipt and approval of signed and sealed 100% design package. All electronic copies shall be provided to the government in b o t h the auto cad file and PDF formats.
1.4.10 The contractor shall coordinate with the 502 CES PM and receive approval 21 calendar days in advance prior to mobilizing for construction.
1.5 CONSTRUCTION
All construction work shall be in strict conformance with design documents and shall include the furnishing of all materials, labor, plant, tools, equipment, and services necessary to accomplish the provisions of this SOW. The work performed by the contractor shall result in complete and fully functioning systems consisting of, but not limited to, the following principal items:
1.5.1 The Contractor shall ensure the adherence to the following exceptions:
1.5.2 All circuits and workmanship shall be in accordance with the latest edition of the National Electrical Code, all applicable UFC’s, NFPA’s, Electrical Safety Standards, and be to current industrial standards.
1.5.3 Prior to commencing demolition activities, the contractor shall coordinate with the 502 CES PM to coordinate any materials/equipment government wishes to salvage upon demolition so as to not damage the materials/equipment during demolition. Contractor shall request for coordination a minimum 14 calendar days prior to commencing work.
1.5.4 Items designated by 502 CES PM for salvage shall remain the property of government at the conclusion of this project. The salvaged property shall be segregated, itemized, delivered and off-loaded at a site designated by the government, distance of the site from the project location not to exceed 2-miles. Any unsalvageable items and/or equipment at the conclusion of the

project will become the property of the contractor and shall be removed off-site and disposed in accordance with (IAW) the provisions of this SOW and all applicable local, state, and federal laws.
1.5.5 Contractor shall divert all recyclable materials to appropriate recycling facilities and shall make all provisions to minimize the amount of debris sent to landfills as a general principle.
This includes cardboard, paper, plastic and glass (except tempered).
1.5.6 Contractor shall protect government equipment and property from damage during construction. Contractor shall be responsible for repairing any damages to government property due to contractor’s negligence, at its own cost.
1.5.7 At the conclusion of the project contractor shall perform a final cleanup and return the project site back to the government in the same or better condition than at the beginning of the project, ready for immediate use by the government.
1.5.8 All work shall meet applicable codes and Air Force standards. It is the contractor’s responsibility to obtain and adhere to codes and standards throughout design and construction.
1.6 CODES AND REGULATIONS
All work shall be accomplished IAW with the latest edition of applicable codes, standards, regulations and other requirements. The contractor shall provide and install a completely finished, operating, and fully functioning high voltage electrical duct bank system. The system shall be finished to current and jurisdictional industrial, federal, DoD, State, and local standards and ready for instantaneous use by the Government. Such standards and regulations shall include, but not be limited to International Building Code (IBC), National Fire Protection (NFPA) Codes, NFPA Life Safety Code, and Unified Facility Criteria (UFC) Guidelines.
1.7 GENERAL
Applicable provisions of the U F G S , t h i s S O W a n d t h e A F F A R S w i l l g o v e r n work under each section of these specifications.
1.8 GENERAL REQUIREMENTS
1.8.1 Site Plan
The contractor shall prepare a site plan indicating the approved location and dimensions of any area to be isolated/fenced for use by the contractor, the number of trailers to be used, avenues of ingress/egress to the project area, and laydown/storage area. Any areas, which may have to be graveled to prevent the tracking of mud, shall also be identified. The contractor shall also indicate if the use of a supplemental or other staging area is desired. Please see Appendix A for general location of the site. Contractor site plan shall be submitted for approval by the government 14 calendar days in advance prior to mobilization for construction. Supplemental or other desired staging areas shall be submitted for approval by Group staff.
1.8.2 Identification of Employees

The contractor shall be responsible for furnishing to each employee, and for requiring each employee engaged at the work site to display, identification as approved and directed by the Contracting Officer. Prescribed identification shall immediately be delivered to the Contracting Officer for cancellation upon release of any employee. When required, the contractor shall obtain and provide fingerprints of persons employed on the project. All contractor and sub-contractor personnel shall wear identifying markings on hard hats and safety vests clearly identifying the company for whom the employee works.
1.8.3 Employee Parking
Contractor employees shall park privately owned vehicles in an area designated by the JBSA Civil Engineer and the Contracting Officer. This area will be within reasonable walking distance of the construction site. Contractor employee parking shall not interfere with existing and established parking requirements of the military installation. Please see the Appendix A for general location.
1.8.4 Job Site Tools and Equipment
All hand tools and equipment shall be maintained in good working conditions and properly stored when not in use. Construction attire and PPE shall be IAW applicable OSHA regulations.
All tools and equipment shall be at hand prior to the contractor commencing work for the day.
1.8.5 Project Signage
Contractor shall furnish and erect a project construction sign as specified in Appendix F.
Coordinate final signage copy and location at site with 502 CES PM.
1.8.6 Protection of Government Property:
The contractor shall furnish and maintain adequate barricades, and warning signs, on the job site.
The contractor shall clean up debris on a daily basis, Contractor generated refuse shall be stored in a covered waste (tarped) container provided by the contractor and hauled away from construction site to a disposal area, selected by the Contractor, located outside the physical boundaries of the installation. The construction site shall be kept clean, ordely, and safe for workers at all times. On site- dumpster furnished by the Contractor shall be coordinated with the Government inspector.
2.0 AVAILABILITY AND USE OF UTILITY SERVICES
2.01 Sanitation
The contractor shall furnish their own portable toilet facilities located within the construction laydown area. No tracking of dirt and/or rubbish shall be in the existing pharmaceutical facility.
2.02 Telephone
The contractor shall make all necessary arrangements and pay all costs for telephone facilities if desired.
2.1 Payment for Utility Services
The government will make all reasonably required utilities available to the contractor from existing outlets and supplies, as specified in the contract. The contractor shall carefully conserve

any utilities furnished without charge. The contractor, at its expense and in a manner satisfactory to the Contracting Officer with Base Civil Engineer, shall provide and maintain necessary temporary connections and distribution lines.
2.2 CONTRACTOR'S TEMPORARY FACILITIES
2.2.1 Administrative Field Offices
The contractor shall provide and maintain any necessary administrative field office facilities within the construction area per approved Site Plan. Government office and warehouse facilities will not be available to the contractor's personnel.
2.2.2 Storage Area
The contractor shall construct and maintain temporary 6-foot high chain link fence around the exterior of the project site perimeter, trailers and stored materials. The fence shall include woven privacy fabric, 90%-density, bound edging with brass 1”-grommets fastening locations; factory cut wind-holes; color: dark brown, so that visibility through the fence is obstructed. Fence fabric shall be installed on the “public” side of the site’s construction fence and maintained in a neat and tight fashion onto the fence. Fence fabric damaged during the construction phase shall be replaced at the direction of the Contracting Officer. Fence fabric material color shall be dark brown and submitted to the Contracting Officer for approval. Fence posts shall be driven, in lieu of concrete bases, where soil conditions permit. Contractor shall pin all temporary fencing such that it does not move with a 50 knot wind. Trailers, materials, or equipment shall not be placed or stored outside the fenced area unless such trailers, materials, or equipment are assigned a separate and distinct storage area by the Contracting Officer away from the vicinity of the construction site but within the military boundaries. Trailers, equipment, or materials shall not be open to public view with the exception of those items which are in support of ongoing work on any given day. Materials shall not be stockpiled outside the fence in preparation for the next day's work. Mobile equipment, such as tractors, wheeled lifting equipment, cranes, trucks, and like equipment, shall be parked within the fenced area at the end of each workday.
2.2.3 Appearance of Trailers
Trailers utilized by the contractor for administrative or material storage purposes shall present a clean and neat exterior appearance and shall be in a state of good repair. Trailers, which, in the opinion of the Contracting Officer, require exterior painting or maintenance will not be allowed on the military property and can be sent, back at the discretion of the Contracting Officer.
2.2.4 Maintenance of Storage Area
Fencing shall be kept in a state of good repair and proper alignment. Should the contractor elect to traverse with construction equipment or other vehicles, grassed or unpaved areas which are not established roadways, such areas shall be covered with a layer of gravel as necessary to prevent rutting and the tracking of mud onto paved or established roadways; gravel gradation shall be at the contractor's discretion. Grass located within the boundaries of the construction site and 5’ outside the boundaries of the construction site shall be mowed by the contractor for the duration of the project. Grass and vegetation along fences, buildings, under trailers, and in areas not accessible to mowers shall be edged or trimmed neatly. At no time shall vegetation be allowed

to be taller than 6”.
2.2.5 Security Provisions
Adequate outside security lighting shall be provided at the contractor's temporary facilities. The contractor shall be responsible for the security of its own equipment; in addition, the contractor shall notify appropriate law enforcement agency requesting periodic security checks of the temporary project field office.
2.2.6 Removal of Temporary Fence
The contractor shall remove the temporary fence upon completion and acceptance of the project.
In case the fence needs to be removed for the completion of obstructed areas, such as landscaping, the site shall remain in neat appearance with all proper barricades, as specified.
2.3 BULLETIN BOARD, PROJECT SIGN, AND PROJECT SAFETY SIGN
2.3.1 Bulletin Board
Immediately upon beginning of work, the contractor shall provide a bulletin board displaying the Equal Employment Opportunity poster, a copy of the wage determination contained in the contract, Wage Rate Information poster, and other information approved by the Contracting Officer. The bulletin board shall be located at the project site in a conspicuous place easily accessible to all employees, as approved by the Contracting Officer. Legible copies of the aforementioned data shall be displayed until work is completed. Upon completion of work the bulletin board shall be removed by and remain the property of the contractor.
2.3.2 Project and Safety Signs
The contractor shall prominently display any safety signs necessary to prevent any accidents to contractor’s employees, government employees, or any other visitors in the project area. The signs shall be posted within 10 working days after receipt of the notice to proceed (NTP). Upon completion of the project, the signs shall be removed from the site.
2.4 TEMPORARY PROJECT SAFETY FENCING
As soon as practicable, but prior to the commencement of work, the contractor shall furnish and install temporary project safety fencing at the work site. The contractor shall also furnish and install hazard safety fencing at temporary hazards and work site areas considered to be hazardous to the public. Caution tape shall not be used as a means of fencing. In lieu of caution tape sawhorse traffic barricades (Type I or II) with tightly strung yellow rope may be used on a limited basis. The safety fencing shall be a high visibility OSHA orange colored, high density polyethylene 1.5” x 3” oval grid or approved equal, a minimum of 48-inches high, supported and tightly secured to steel posts located on maximum 6-foot centers, constructed at the approved location. The temporary safety fencing shall be maintained by the contractor during the periods listed below. Upon completion and acceptance of the work, the fencing shall remain the property of the contractor and shall be removed from the work site.
2.5 Duration of Temporary Safety Fencing
The following durations for the use of specific safety barriers shall be used as a guideline. The Contracting Officer may dictate longer or shorter periods as required for project specific work.

2.5.1 Sawhorse traffic barricades with tightly strung yellow rope not to exceed a 24-hour period.
2.5.2 Traffic cones not to exceed 3-days (excluding road construction).
2.5.3 Traffic barricades and safety fencing not to exceed 14-days.
2.6 CLEANUP
At the end of each working day, contractor shall clean up the work site by the removal of all construction debris, waste materials, packaging material and the like. If the aforementioned items could become airborne, they shall be placed in secure refuse containers immediately. Clean up of the work site shall include the construction areas, material storage areas, parking and eating areas, and any other areas affected by the construction process. Any dirt or mud, tracked on to any street, parking area, sidewalk, patio, driveway, or building shall be cleaned away daily.
Materials resulting from demolition activities that are salvageable shall be stored within the laydown area described above or at the supplemental storage area. All stored material, whether new or salvaged, shall be neatly stacked when stored and protected from weather damage.
2.7 RESTORATION OF GOVERNMENT AREA
Areas used by the contractor for the storage of equipment or material, or other use, shall be restored to the original or better condition prior to returning to the government.
2.8 EXISTING UTILITIES
Record drawings showing existing underground utilities shall be provided to the winning contractor. The contractor shall avail himself of the drawings. Any utility line shown on the record drawings (or made known to the contractor) and damaged during construction work shall be repaired immediately by the contractor at no cost to the government. It shall be the responsibility of the contractor to protect the utility markings made by the government during the issuance of Digging Permit throughout the duration of the contract period. The cost of subsequent request to remark the site shall be $200 per visit and shall be deducted out of the contract amount.
2.9 SUBMITTALS
During the design phase the DOR shall update the provided Material Submittal Register with additional items for government approval to comply with the intent of this SOW, these items shall include all newly installed products and/or reference material that are to remain government property at the conclusion of this project. Contractor shall utilize the updated Submittal Registry to provide submittals on AF IMT 3000 Material Approval Submittal Form for all listed items, prior to work or installation of materials. Do not purchase or conduct work until the approved submittal form has been returned from Contracting Officer. The contractor shall allow 14 calendar days for government review of the submittal for approval or disapproval.
2.10 Electrical Equipment and Systems:
Contractor shall mean to provide in good faith current up to date quality, and well made products

and materials from established manufacture. The provision, installation or submission of obsolete, inferior, poorly designed (and unrecognized by national bodies such as UL, ANSI etc.) and end of model closeout products shall be considered a breach of good faith.
2.11 PRE-CONSTRUCTION SITE VISIT
The contractor shall visit the job site to verify location, conditions, and dimensions prior to commencing work.
2.11.1 PRE-CONSTRUCTION MEETING
The contractor shall request for a preconstruction meeting with the Contracting Officer prior to the commencement of any construction work. The preconstruction meeting shall be attended by at minimum the contractor’s Project Manager, Site Superintendent, Quality Control, and Site Safety and Health Officer.
2.12 ENVIRONMENTAL MANAGEMENT
The contractor shall comply with all provisions of JBSA-Lackland Environmental Protection specification as applicable to this contract. Appendix C includes a copy of the JBSA-Lackland Environmental Protection specification. No separate payment will be made to the contractor to ensure compliance with the provisions of the environmental specifications; all cost shall be part of the contractor’s base bid
2.13 QUALITY ASSURANCE
Provide a Quality Control (QC) person at the work site at all times for continuous surveillance of project activities to ensure workmanship, methods, materials, and equipment conform to contract requirements. QC shall have the “stop work” authority to prevent or stop performance inconsistent with the contract requirements. The QC shall be a separate function from the superintendent and thus not one in the same
2.13.1 The contractor shall submit a Quality Control Plan (QCP) for government approval to the Contracting Officer at least 14 calendar days prior to commencing work. Work cannot proceed without an approved QCP. At minimum, the QCP shall discuss the following topics:
• Introduction: task order description, and quality commitment statement
• References
• Project Organization: key project personnel and responsibilities
• Quality Control Plan Elements
• Submittals
• Quality Management Procedures
• Testing / Inspection / Monitoring
• Closeout Inspection
• Documentation and Control
• Forms and Records

2.14 GENERAL SAFETY REQUIREMENTS

2.14.1 Provide a Site Safety and Health Officer (SSHO) at the work site at all times to perform safety and occupational health management, surveillance, inspections, and safety enforcement for the contractor. The contractor’s QC can be SSHO on this project.
2.14.2 The contractor shall submit a site specific Accident Prevention Plan (APP) for government approval to the Contracting Officer at least 14 calendar days prior to commencing work. Work cannot proceed without an approved APP. All changes to the approved APP shall be made with the knowledge and concurrence of the Contracting Officer and 502 CES PM. At minimum, the APP shall discuss the following topics:
• Site Background
• Project Organization: key project personnel and responsibilities
• Site Hazards: chemical, physical, biological
• Exposure Monitoring
• Site Control
• Personal Protective Equipment
• Decontamination
• Safety Policy and Procedures
• Emergency Response Plan
• Training
• Medical Surveillance
• Recordkeeping
2.14.3 Obey the posted base speed limits. Wear Seat belts on base IAW AFI 91-207. JBSA Lackland speed limits is 30 mph unless otherwise posted by the base security. If troops are marching (2 or more troops) in formation, the speed limit is a maximum of 10 mph.
2.14.4 Report contractor accidents involving injury to Air Force personnel or damage to government property to the Contracting Officer, 502 CES PM, 502 CES Project Inspector (PI) and the 502 ABW OL-A/SEG, Ground Safety Officer immediately.
2.14.5 The contractor shall familiarize and comply with the general safety requirements IAW DOL regulations, OSHA 29 CFR Part 1926, as well as applicable AFOSH standards and AFRs.
These standards and AFRs are available in the 502 ABW OL-A/SEG, Ground Safety Office, and building 2484.

2.15 BARRICADES AND WARNING SIGNS
Adequate barricades, warning signs and flares shall be furnished, installed, and maintained by the contractor. Barricades and signs shall meet the requirements established and in the National Manual on Uniform Traffic Control Devices for Streets and Highways, pertaining to safety and barricades. Barricades shall have warning lights with low-intensity electric flashers for night application.
2.16 DIGGING PERMIT
2.16.1 Obtain a blank AF Form 103, BASE CIVIL ENGINEERING WORK CLEARANCE REQUEST (often called a digging permit) from 502 CES PM. Contact 502 CES/CEOI, (210) 671-3533 to schedule a digging permit (AF Form 103) to excavate on JBSA-Lackland.
Contractor shall request a digging permit at least 14 calendar days in advance
2.16.2 The dispatcher will assign a Job Order Number and schedule a time for the base agencies to meet at the job site with the job superintendent. The superintendent must contact Dig Test at 1-800-344-8377 for Time Warner and SBC and Base Communications Office (210) 925-0607.
Utilities are only located in housing areas; base communications is located everywhere else.
After each agency including Time Warner and Southwestern Bell has signed the AF Form 103, deliver the form to the 502 CES PI for final approval signatures. The 502 CES PI will return a copy of the completed and approved AF Form 103 to the contractor.
2.16.3 It shall be the responsibility of the contractor to document and preserve markings until they are no longer needed. The government will not be responsible for re-marking an area already covered by an existing digging permit. If absolutely necessary, the cost of subsequent request to remark the site shall be $200 per visit and shall be deducted out of the contract amount.
2.16.4 All excavations other than building foundations, must have the site, completed within 14 calendar days (excluding approved rain delay days) after excavation has been initiated.
2.17 UTILITY OUTAGES

2.17.1 Utility outages is defined as a stoppage in electric, gas, potable water, sewer, chilled or hot water, steam, telephone, Energy Monitoring and Control System (EMCS), or any other similar utility whether serving all or a part of a single facility or of several facilities.
2.17.2 All utility outages shall be coordinated with the Using Agency through the Contracting Officer, the 502 CES PM. Notice of the necessity for an outage shall be provided by the contractor through the Contracting Officer to the JBSA-CE, in writing, not less than 14 calendar days prior to the time of the required outage and written approval for the outage will be issued by the Base Civil Engineer via Contracting Officer. All work shall be coordinated and arranged to ensure that the outage will be of minimum duration.
2.17.3 Once a utility outage is arranged and work begun, work shall continue until all utilities are restored to the affected facility or facilities.
2.17.4 In the event a scheduled utility outage is cancelled by the government, notification will be given to the contractor at least 24 hours in advance of the time for the outage to start.
Rescheduling of a cancelled outage must be coordinated with written notification as described above.
2.18 WELDING/OPEN FLAME OR SPARK PRODUCING ACTIVITIES
Any of these activities shall not be performed without first obtaining a hot work permit issued by the JBSA Fire Department at 210-671-2921
2.19 CONTRACTOR'S USE OF ELECTROMAGNETIC EMISSION DEVICES ON
BASE
2.19.1 The Contractor shall comply with DOD 4650.1 Policy and Procedures for Management and use of Electromagnetic Spectrum, AFI 33-580 and AR5-12, Army use of the Electronic Magnetic Spectrum. In specific as interpreted by 502 CES/SCTR AFI 33-580:
• AF Contractors must submit frequency requests in direct support of AF contracts through the AF representative to the MAJCOM responsible for administering the contract. The contractor must obtain frequency assignments from FCC for requirements not in direct support of the contract.
• Multiple service Contracts. Contractors must submit frequency requests in support of a multiple service contract through the appropriate spectrum management channels to the military department that is the executive service for the contract. The contractor must obtain frequency assignments from the FCC for requirements not in direct support of the contract.
• The FCC & NCTIA hold license legally and financially responsible in cases involving interference, so frequencies in use must be approved by requisite authority(The requisite authority in this Contract is the DOD, USAF, & JBSA-LAK.)
• Chapter 6- Consequences & Risks of Non of Compliance Notice of apparent liability: The FCC Regulations and the provisions of the NTIA Manuals are found in 47 CFR 300. Therefore, FCC and NTIA provisions, rules, and technical standards are mandatory for Army civilians, military and contractor personnel. In general, punitive measures resulting from violations of the CFR are applicable to intentional and unintentional instances of noncompliance of FCC rules and the NTIA Manual reference to 47 USC 503 regarding punishment

of any person willfully or repeatedly fails to comply with any of the provisions stated.
2.20 RADIOACTIVE EQUIPMENT
Use of radioisotopes or radiation producing equipment (density gauging, NDE weld testing, etc.)
requires a Nuclear Regulatory Commission (NRC) Radioactive Material License, with all documentation submitted and approved prior to bringing the material on base. To obtain authorization, contact the Base Radiation Safety Office (SRPO), at 559 AMDS/SGPB, Bio- Environmental Engineering Office, Bldg 7065, phone (210) 671-7061.
2.20.1 Contractor shall also comply with all requirements of AFI 40-201, Lackland AFB Supplement 1, AFI 40-201, and AFI 48-148 which can be reviewed in the Base Bio-Environmental Engineering Office.
2.20.2 Contractor shall forward application to the Contracting Officer 30 calendar days prior to commencing operations using radioactive materials.
2.20.3 Contractors possessing Agreement State Licenses must also submit an NRC Form 241 to NRC in compliance with 10 CFR 150.20. Request shall include the following:
• Description of proposed activities on NRC Form 241.
• Procedures established to ensure radiological health and safety of all personnel.
• Name of responsible contractor representative.
• Current copy of application for NRC or Agreement State License. The license must specifically state the installation by name or approval for temporary job sites anywhere in the United States where the NRC or Agreement State maintains jurisdiction.
• The part of the Air Force contract describing work to be done at the base and the inclusive dates of such work.
• An acknowledgement that the base RPO can make periodic checks to ensure the contractor is following applicable radiological health and safety practices.
• Contractors requiring more than 180 calendar days per calendar year of operation must possess an NRC license.
2.21 FIRE REGULATIONS AND PREVENTION PRACTICES
The contractor shall comply with fire prevention practices as set forth by the National Fire Protection Association, other recognized fire prevention agencies, and base regulations.
2.22 EXPLOSIVE OPERATED HANDTOOLS
The contractor shall comply with OSHA Standard 1910.215(c) when utilizing explosive operated hand tools. Storage of explosive cartridges on the installation shall be in metal containers and limited to one days' supply. The contractor shall provide adequate controls to prevent loss/theft of cartridges used and stored on the installation.
2.23 PREVENTION OF FUEL SPILLS
Contractor may use temporary fuel tanks upon approval by the Contracting Officer. Tanks shall be surrounded by an earthen berm, which provides 110% of the tank capacity. The berm shall be lined with a 40-mil polyethylene sheet held in place with weights located on the outside of the

berm. All berms shall be inspected by the 502 CES PM and the JBSA FD prior to filling fuel tanks.
2.24 GENERATED REFUSE
The contractor shall take positive action to prevent work-generated refuse from entering the sewer system. All contractor generated refuse and waste shall be hauled from the construction site daily to a disposal area located outside the physical boundaries of the installation. This refuse and waste material shall be hauled in trucks with tight-fitting beds and covers to prevent spillage on roadways. The construction site shall be kept neat, orderly, and safe for workmen at all times.
All required manifests shall be obtained prior to the transportation of refuse off-site.
2.25 WATER CONSERVATION
Conserve water to the greatest extent possible.
2.25.1 If JBSA-Lackland implements water usage restrictions, the contractor is required to comply with these restrictions, at no additional cost to the government. In the event the contractor cannot comply with water usage restrictions and still meets other contract requirements (particularly when installing sod or hydro-mulching), then he can request permission from the Contracting Officer for relief from the restrictions.
2.25.2 Waivers: Submit request for waivers to water conservation restrictions in writing. The Contracting Officer will consider such cases on an individual basis and grant waivers only in extreme cases. Submit detailed requests with appropriate manufacturer’s/supplier’s data to show unavoidable noncompliance with the water usage restrictions. In the event a restriction is conditionally waived, use only the minimum amount of water necessary to comply with contract requirements.
2.26 ENERGY REQUIREMENTS
Design of new construction and renovations shall be in full compliance with ANSI/IESNA/ ASHRA Standards with specific attention to ASHRA Standard 90.1-2004 or latest edition. In addition, new construction for existing facilities must comply with Energy Policy Act of 2005 (EPACT 05), Executive Order 13423 and energy conservation standards under Unified Facilities Criteria, UFC 3-400-01 1 July 2013, UFC 3-410-01FA 1 July 2013, UFC 3-410-02 1 May 2012, UFC 3-440-01 14 Jun 2002, UFC 3-470-01 1 May 2012 or latest edition for all at the time of contract award. Air Force Design Sustainability shall be applied in the development concepts of planning, design, construction, environmental management, operation, maintenance and disposal of facilities and infrastructure project, consistent with budget and mission requirements. Design Energy Compliance Analysis (ECA) must be provided with the design package.
2.27 INVENTORY OF INSTALLED PROPERTY
The contractor shall prepare and submit a list of new equipment or units of equipment that require gas, water, or electrical utilities connections per the attached JBSA-Lackland Environmental Protection specification, see Appendix C. (Examples: AHUs, fans, air conditioners, compressors, condensers, chillers, boilers, furnaces, heat exchangers, pumps, cooling towers, tanks, fire hydrants, sinks, water closets, urinals, lavatories, shower stalls, and other large plumbing fixtures, lighting fixtures, electric heating equipment, motors, and other

electrical equipment). The list will be reviewed periodically by the government to verify completeness and accuracy. Completed list shall be turned over to the 502 CES PM at least 21 calendar days prior to the pre-final inspection.
2.28 INSPECTION AND ACCEPTANCE
Inspections and final acceptance will be made as completed by the contractor with the following additional provisions:
2.28.1 The contractor shall give the government a minimum of three (3) working days’ notice for all requested inspections. Inspections shall be staggered such that two inspections will not occur at the same time.
2.28.2 The contractor shall make sure that the work is ready for all inspections. The presence of an unreasonable number of deficiencies, as determined by the 502 CES PM, may cause the inspection to be rescheduled. A final inspection shall not have more than 10 deficiencies or it will have to be terminated and rescheduled when the contractor is actually ready.
2.29 INSPECTION OF CORROSION PROTECTION INSTALLATION
2.29.1 All metallic, above or underground, water or fuel tanks and all metallic underground water, gas, fuel, or electric piping require corrosion protection. The installation shall be coordinated with the 502 CES PM prior to commencement.
2.29.2 All work shall be coordinated and arranged by the contractor to ensure the corrosion protection installation is inspected by the 502 CES PM IAW the following sequence:
2.29.3 Initial Inspection: All work on the corrosion protection installation is in the process of being completed and is properly uncovered for inspection by the 502 CES PM or PI.
2.29.4 Final Inspection: All work on the corrosion protection installation has been satisfactorily completed, all deficiencies, if any, corrected and is properly uncovered for final inspection by the
502 CES PM.
2.29.5 Corrosion protection installation shall be completed IAW the plans and specifications and certified prior to approval/acceptance by the government.
2.30 DETERIORATED CONSTRUCTION AND FINISH
The 502 CES PM will be the governing authority in determining if existing construction and finishes are deteriorated and require repair and/or replacement.
2.31 AS-BUILT DRAWINGS
The contractor shall provide complete detailed as-built drawings representing the entire project as constructed under the contract. The drawings shall be drawn to appropriate scales and dimensioned completely and accurately. Drawings shall be delivered to the government on ANSI Size D 22” x 34” hard copy, standard black line quality paper and electronically on AutoCAD release 2014 or compatible format and as PDF on CD. Preliminary as-built drawings shall be submitted with an AF Form 3000 for government review/approval. The preliminary as-builts

shall be submitted concurrently with the request for pre-final inspection. Upon government approval of the preliminary as-builts, the contractor shall submit the entire final set of hard copy and electronic as-builts drawings for final approval and acceptance. As-builts shall be included as a separate item in the progress schedule AF Form 3064. The final set of hard copy and electronic as-built drawings shall be submitted within 10-working days of the final acceptance of the project.
2.32 DD FORM 1354
The contractor shall prepare and submit DD Form 1354, Transfer and Acceptance of Military Real Property prior to acceptance of the project by the government. The contractor will use the checklist provided in Appendix D, and complete all applicable information to develop the DD Form 1354. A preliminary copy of the form along with the checklist shall be submitted for review by the government to verify completeness and accuracy at least 30 calendar days prior to the final inspection. The completed DD Form 1354 along with the checklist shall be turned over to the 502 CES PM at least 14 calendar days prior to final inspection. A final copy of the DD Form 1354 shall be submitted within 10 calendar days after the final acceptance of the project.
2.33 DELIVERABLES
All of the contract deliverables and supporting documentation becomes the property of the Government upon delivery and shall remain property of the Government at the end of the contract period.
2.34 CONTRACT DRAWINGS
Drawings shall be prepared on AutoCAD release 2014 or compatible format. All contract drawings shall be well prepared, complete, and accomplished in accordance with the best professional practice to show clearly and concisely the type and extent of work to be performed.
The drawings shall be drawn to appropriate scales and dimensioned completely and accurately.
Extensive explanatory notes on the drawings should be held to a minimum or placed in the specifications. Dimensions shall be shown in Standard English units. Standard building material indications and symbols for architectural items and for mechanical and electrical equipment shall be used to the greatest extent possible. Drawings shall be delivered in hard copy, standard black line quality paper, at review stages in the number listed in paragraph on SUBMITTALS. All review drawings shall be ANSI D size 22” X 34” in size.
2.35 DRAWING STANDARDS
Drawings shall be prepared on AutoCAD release 2014 or compatible format. All drawings shall utilize A/E/C CAD Standards, Rel 4.0 or latest release and SDSFIE 2.61 for compliance with the installation standards. Both hard copies and electronic copies of the drawings shall be provided to the Contracting Officer for review at the 50%, and 100% design phases. Upon the acceptance of the 100% design drawings, the contractor shall provide to the Contracting Officer signed and sealed 100% design submittal. Extensive explanatory notes on the drawings should be held to a minimum. Dimensions shall be shown in Standard English units. Standard building material indications and symbols for architectural items and for mechanical and electrical equipment shall be used to the greatest extent possible.

2.36 DESIGN SERVICES
2.36.1 S i t e D e s i g n : The contractor shall perform field reconnaissance, surveys, and site investigations required to obtain engineering information and design data necessary to comply with the provisions of the contract documents IAW the requirements of this SOW.
2.36.2 Field Measurements: The contractor shall be required to make its own field investigations to verify dimensions and other information shown on government furnished reference drawings.
2.36.3 SCHEDULE OF DELIVARIBLES
