Performance Work Statement For

Combative Mats F2MTR36112AW01- 343 TRS, Bldg. 10334

1. SCOPE OF WORK:

1.1 SUMMARY: The work to be performed under this contract shall be in accordance with this Statement of Work. The Contractor shall provide all labor, tools, equipment, supplies, transportation; materials and supervision necessary to accomplish the installation of combative mats.

1.2 BASIC SERVICES. This is a one-time service contract. It provides for the removal of the current mats and the installation of new combative mats at building 10334 JBSA Lackland and building 150 Medina Annex, Texas. Combative mats shall be installed IAW the government design (Attachment 1). The following services are required:

a. Provide installation specifications and finishes review with customer immediately after contract award and prior to combative mat order factory submittal.

b. Provide new product.

c. Remove current combative mats and relocate to building 10263, government warehouse.

d. Installation of new combative mats ensuring installation is in accordance with design and all electrical outlets are visible.

e. Provide manufacturer warranty documentation after completion of work.

1.2.1 MATERIAL SPECIFICATIONS.

1.2.1.1 FLOOR MATS

a. The construction should be high-quality multi-density rebounded foam core for superior impact, and softness for lengthy rolling and grappling sessions

b. The surface should be a smooth, non-slip vinyl that eliminates mat burns and is easy to clean.

c. 1.5” thickness

d. 12# Density, re-bonded Polyurethane Foam

e. 18oz Fabric-reinforced Vinyl Cover

f. Anti-Skid Waffle Foam backing to minimize shifting

g. Blue, black and gray colors in an arrangement to be determined following award of contract

1.2.1.2 WALL PADS

a. Black in color

b. 72”H x 24” L x 2” D

c. 1/2” thick Novaply wood backing

d. 18oz Fabric-reinforced Vinyl cover

1.2.1.3 GRAPHICS

a. Building 10334

a. (4) Durable printed 5’ x 5’ USAF Security Forces Flash graphics for wall pads. Colors include black, blue, yellow, grey and white.

b. Building 150

a. (2) Durable printed 5’ x 5’ USAF Security Forces Flash graphics for wall pads. Colors include black, blue, yellow, grey and white.

1.3 SITE VISIT: Site visit shall be accomplished through coordination with the Contracting Officer.

1.4 ACCESS: All Contractor and Sub-Contractor employees working on Lackland AFB shall require a National Agency Check (NAC) background investigation. Workers entry must be coordinated in advance with the Contracting Officer.

1.5 HOURS OF OPERATION: Normal working hours are between 7:30 am and 4:30 p.m. Monday through Friday, excluding government holidays. Approval to work outside of normal working hours is at the Contracting Officer’s discretion.

1.6 PROJECT MANAGEMENT: Contractor shall provide a manager as a single point of contact to the government throughout all phases of work to be performed.

1.7 SPECIFICATION: Contractor shall specify systems from the attached concept plan.

a. All installations will comply with the National Fire Protection Association Codes (NFPA-101) and the Americans with Disabilities Act (ADA).

b. LAN and phone wiring, and electrical are not part of this project, however, work stations must be aligned to be able to utilize the outlets in place for connectivity to existing services.

c. Contractor shall not be responsible for any outside contract maintenance related items outside of manufaturer warranty period..

2. STORAGE OF PRODUCT DURING TEAR DOWN AND INSTALLATION: This is an occupied building. Materials shall be stored on site in such a manner that the continuing working operation of the 343 TRS function is not impaired. Removed matting will be relocated to building 10263 upon completion of removal of existing matting. Aisles must remain open and marked in compliance with OSHA29 CFR1910.22

3. ACCESSORIES AND INCIDENTALS: Contractor shall provide all incidentals necessary and all incidental hardware shall be installed in accordance with manufacturers’ recommendations. Adhesives, fasteners, nails, etc. shall be furnished by the Contractor.

4. ELECTRICAL; PHONE AND LAN SERVICES: Contractor shall install mats in a manner that does not cover any electrical, phone and LAN line. Contractor is not responsible for connectivity.

5. PROTECTION: The Contractor shall take precautions to prevent damage to government property.

The Contractor shall repair or replace damaged items at no additional cost to the government.

6. SITE MAINTENANCE: Work areas shall be maintained in a neat, clean, safe condition and all trash, debris, and garbage, which the Contractor generates shall be placed in appropriate containers daily.

7. DISPOSAL OF UNUSED PRODUCT AT THE END OF PROJECT: None

8. WARRANTIES: Installation shall be warranted from defect for one year. Commercial warranties on product shall be applicable.

9. COMPLETION OF WORK: The Contractor shall ensure that work for this project is performed in accordance with the criteria herein, and that all equipment shall be fully operational at the completion of work for this project.
