STATEMENT OF WORK

For Replace Carpet Location: Building: 1425 By: Roland Hartz

WORK ORDER NUMBER: 65090

DATED: 11 July 2013 REVISED: 06 July 2015

1. GENERAL INFORMATION:

This project is intended to replace carpet in accordance with all applicable National, Air Force, State and local building codes and regulations. This work will consist of supplying/furnishing all labor, materials, tools, equipment, shop drawings, and expenses necessary to accomplish the work described below in compliance with the Commonwealth of Massachusetts Building Code and Massachusetts General Law requirements, state and federal codes, base standards and manufacturer’s instructions. Where there is a conflict between nationally recognized codes and local codes, the most stringent shall apply.

2. SCOPE: Replace carpet in multiple rooms, no hardwood floors being done at this time

· Entry way 16’2” x 18’8” Approximately 292 SF Remove existing carpet including stairs

Install new carpet on floor and stairs (color TBD) Tested for ACM No ACM found

· Heritage Room 20’11” x 33’6” Approximately 704 SF Remove existing carpet

Install new carpet (color TBD) Tested for ACM No ACM found

· Musket Room 23’7” x 42’8” Approximately 1032 SF Remove existing carpet including stairs & ramps in the Lincoln Ball Room

Install new carpet including stairs (color TBD) Tested floor tile under carpet contains asbestos material, conducted pull test, no tiles pulling up at this time, if tiles start pulling up with carpet stop work and notify CE.

· Lodging Room 32’5” x 53’1” Approximately 1723 SF Remove existing carpet in room

Install new carpet in room (color TBD) Tested for ACM No ACM found

· Lincoln Ball Room 67’2” x 79’1” Approximately 4853 SF Remove existing carpet Install new carpet (color TBD) Tested for ACM No ACM found

· Den 44’10” x 50’3” Approximately 1830 SF Remove existing carpet Install new carpet floor (color TBD)

Behind bar not to be done Tested for ACM No ACM found

3. GENERAL REQUIREMENTS:

Work includes, but is not limited to the following:

a. Provide resolutions of any technical problems that arise during the construction.

b. Should any change in scope be required, it must be coordinated with the Project POC and approved as a change order prior to performing the change in work.

c. Provide updates to Project POC. Any construction issues, changes to schedule, or schedule slippages with justification must be reported at the periodic updates.

d. Provide warranty letter

e. Period of Performance with be 120 days after contract award
