Attachment 2 Statement of Work
Purchase and deliver sea land containers to the Barry M Goldwater Air
Force Range
The contractor shall purchase and provide the following Sea Land containers:
60 ea 40’X 8’X 8’ containers.
40 ea 20’X 8’X 8’ Containers.
High cubes are allowed in either size.
All containers shall be delivered to the water well 6 miles west of Range 1 approximately
50 miles south of Gila Bend AFAF.
Contractor Access. All personnel delivering containers to the BMGR will be required to view a Range orientation video (approximately 20 min.), receive strict orders and procedures on entering and exiting a live bombing range, and sign a hold harmless agreement prior to entering any range. Contractors (other than those holding
Government ID cards) who require unescorted access to Gila Bend AFAF and the BMGR must complete LAFB Form 338, Installation Access Affidavit, and attach copies of 2 forms of identification. Additionally, your government sponsor must submit a letter including reason for access, dates, and personnel list with Luke 338(s) for processing.
This process includes a background check, and applicants must begin the process at least
45 days in advance of desired entry date. After successful processing, these forms are presented to GBAFAF security for issue of AF Form 75 Visitor Pass. Please return pass to security when no longer needed. Every authorized contractor individual shall provide a valid driver's license, current vehicle registration and valid vehicle insurance certificate to obtain a vehicle pass. Contractor shall provide Entry authorization listing (EAL) indicating names of individuals delivering containers with the last 4 digits of their
SSANs.
Total price shall include loading, shipping, and downloading at destination point. All containers must be lead free, asbestos free and be rated A or B for hazardous material, constructed of steel (no aluminum or plastics). Wood floors are acceptable. Containers must not be painted with lead-based paint or be lined with insulation or asbestos.
Containers shall have no shelves, dividers or roofing materials.
Containers must be structurally sound, however, they may have up to 4 inch
Diameter holes with no more than 3 per container. Delivery is to be made FOB destination during normal working hours; which
Are 6:30 AM -3:00 PM, Mon-Fri, however, contractor may work outside of these hours and days with proper coordination in advance with the Contracting Officer.
Contractor shall have an individual on site that is responsible for the delivery of containers to correct location. Contractor shall download and catalogue all containers and provide a detailed drawing and report to the government listing container serial numbers and locations in respect to each other for accurate, user friendly validation after all have been delivered. Contractor shall provide a forklift or crane for downloading containers.
The government will not provide any personnel or equipment for offloading of containers or any other type of support. Period of performance is 90 days.
