1. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR

Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued.

2. Solicitation number F2MTJP5033AM03 is issued as a Request for Quote (RFQ).

3. The solicitation documents and incorporated provisions and clauses are those in effect through Federal

Acquisition circular 2005-79 and DFARS Change Notice 20150129.

4. This acquisition is a 100% Small Business set-aside competitive acquisition in accordance with FAR Subpart

19.502-2. Interested vendors shall refer any and all eligibility questions to their respective Business Development

Specialist at their respective servicing SBA District Office. The associated NAICS is 484210; size standard is

$27.5M.

5. See the attached solicitation Bid Schedules (Reference Atch 2-6) for applicable contract line items (CLINS), quantities, and units of measures.

6. Nonpersonal Services: 502 CONS/JBKCB is requesting quotes from small businesses in awarding a service contract for the preparation of personal property of Department of Defense (DOD) personnel for shipment and/or storage and related services through the DPM for Schedule III (Intra-City/Intra-Area) services in the San Antonio area. The scope of work is defined by the Performance Work Statement (PWS) (Reference Atch 1).

7. Period of Performance: 1 Apr 15 -31 Dec 15, basic plus 4 (four) option years. The service areas are identified in the Bid Schedules (Reference Atch 2-6).

8. The provision at 52.212-1, Instructions to Offerors – Commercial Items, applies to this acquisition.

ADDENDA TO 52.212-1 QUOTATION PREPARATION INSTRUCTIONS

(a) Period for Acceptance of Offers. The paragraph is tailored as follows: “The quoter agrees to hold the prices in its offer firm for 120 calendar days from the date specified for receipt of quotes.” The quoter shall make a clear statement in the quotation documentation that the pricing is valid until this date.

(b) To assure timely and equitable evaluation of quotations, quoters must follow the instructions contained herein.

Quoters are required to meet all solicitation requirements, including terms and conditions, representations and certifications, and technical requirements, in addition to those identified as evaluation factors. Failure to meet a requirement may result in an offer being ineligible for award. Quoters must clearly identify any exceptions to the solicitation terms and conditions and provide complete accompanying rationale.

(c) Multiple Awards: Due to the large volume of local moves during the summer moving season (May –

September) in the San Antonio area, this schedule is divided up for one primary contractor and one secondary contractors. The primary contractor will receive an estimated 75% of all annual tonnage and the secondary contractor will receive an estimated 25% of all annual tonnage. Quoters may place a quote to be either the primary contractor or a secondary contractor. Quoters placing quotes in both categories will be considered for both but will not be awarded both contracts. Quoters submitting such quotes need only to submit technical quotations supporting their primary contractor quote. Such technical quotation will be considered sufficient to meet secondary contractor requirements. All procedures indicated in the performance work statement applies to all contractors. A tonnage distribution roster will be maintained by the ordering officer to ensure that each contractor receives their required percentage monthly. This tonnage distribution roster will be made available to each contractor for review with prior notice given to the ordering office.

(1) The Government shall make award by Area to the best value quoter under each schedule to the extent of the quoter's stated guaranteed daily capability as provided in this solicitation and the estimated quantities schedule. The best value criteria and process that will be used to select awardees is described in the paragraph of provision 52.212-2.

(2) The government reserves the right to make an award of two or more areas to a single quoter if such award will result in an overall lower estimated cost to the Government.

(3) The Government reserves the right to award additional contracts, as a result of this solicitation, to the extent necessary to meet its estimated maximum daily requirements.

(d) Submission of quotes. Submit quotes, to include all fill- in clauses to the office specified in this solicitation at or before the exact time specified in this solicitation.

(e) Address any questions, clarifications or concerns you may via email to: SSgt Nick Griswold, nicholas.griswold@us.af.mil, (210) 652-9094 and provide a copy to Ms. Gina F. Garcia, gina.garcia@us.af.mil,

(210) 652-5131.

(f) Exceptions taken to terms and conditions of the RFQ, to any of its attachments, or to other parts of the RFQ shall be identified. Provide rationale in support of the exception and fully explain its impact, if any, on the performance, schedule, cost and specific requirements of the RFQ. Failure to comply with the terms and conditions of the RFQ may result in the quoter being removed from consideration for award.

9. The provision at 52.212-2, Evaluation -- Commercial Items, applies to this acquisition.

ADDENDUM TO FAR 52.212-2, EVALUATION PROCEDURES

Reference FAR 52.212-2, paragraph (a) is hereby tailored as follows:

(a) Best value determination will be based on a Lowest Price Technically Acceptable approach. The Government will award a single contract resulting from this RFQ to the responsible quoter whose services are technically acceptable, quotes the lowest overall price with inclusion of option years, and has a past performance rating of acceptable. The following factors shall be used to evaluate quotes:

(i) Technical

(ii) Lowest Price with Inclusion of Option Years

(iii) Past Performance

If five (5) or fewer quotes are received, the Government will evaluate all quotes for technical acceptability. If more than five (5) quotes are received, the Government will then evaluate the five (5) lowest priced quotes for Technical

Acceptability. However, the Government reserves the right to evaluate additional quotes for technical acceptability if deemed to be in the best interest. If additional quotes are evaluated for technical acceptability, they will be evaluated in the order of increasing total evaluated price, when the agency determines that none of the initial five (5) lowest priced quotes are qualified as acceptable. The agency will reject and not consider for award any quote found technically unacceptable and ineligible for award under the terms of this RFQ, in the absence of discussions.

The evaluation process shall proceed as follows:

1. Technical Acceptability. The government technical evaluation team shall evaluate the quotations on a pass/fail basis, assigning ratings of Acceptable or Unacceptable. The quotations shall be evaluated against the following sub factors:

(a) Sub Factor 1. Capability. The quoter shall complete the Guaranteed Daily Capability on page 2 of

Attachments 2, 3, 4, 5 and 6 for each area in which the quoter proposes. Quotations not meeting minimum acceptable daily capability for an area as specified on page 2 of Attachments 2, 3, 4, 5 and 6 could indicate a lack of understanding concerning mission requirements shall result in the entire quotation being determined technically unacceptable or being eliminated.

(b) Sub Factor 2 - Contractor Facilities. The quoter shall provide documentation and planned location of the facilities that will be used for storage and processing of shipments as outlined in Appendix D of DoDR 4500.9-R

(Reference PWS paragraph A.11.). The Government will review all provided documentation and perform a site visit of the proposed facilities to determine it acceptable or unacceptable.

2. Price:

(a) Total Evaluated Price: The Government will evaluate quotes for award purposes by adding the total price for all options to the total price for the basic year for each area. The quoter shall accurately complete the Bid

Schedules (Reference Atchs 2-6) for the areas in which the quoter proposes for the basic and each option year.

Incomplete Bid Schedules or quotations could indicate lack of understanding of the requirement and may result in the entire quotation being determined unacceptable and eliminated from further consideration.

(b) Options:

i. The contract will include four (4) one-year options.

ii. The Government may determine that an quoter is unacceptable if the option prices are significantly unbalanced. Unbalanced pricing exists when, despite an acceptable total evaluated price, the price of one or more contract line items or sub-line item(s) appear to be significantly overstated or understated as indicated by the application of cost or price analysis techniques. Therefore, a quotation may be rejected if the Contracting Officer determines the lack of balance poses an unacceptable risk to the Government. Evaluation of options shall not obligate the Government to exercise the option(s).

(c) Price Reasonableness: The Government may use any of the following price analysis techniques IAW

FAR 15.404-1(b) to determine price reasonableness: comparison of proposed prices received, comparison of previously proposed prices, and previous Government and commercial contract prices, use of parametric estimating/rough yardsticks, comparison with competitive published price lists or market prices, comparison with independent Government cost estimates, comparison with market research, and analysis of pricing information provided by the quoter.

3. Past Performance

Past performance will demonstrate contractor/vendor’s ability to provide the services IAW the PWS (Reference

Atch 1) and the Bid Schedules (Reference Atchs 2-6). The Government will evaluate the quality and extent of quoter’s performance deemed recent and relevant to the requirements of this RFQ. The Government will use information submitted by the quoter and any other sources of information available to the Government to access past performance. Provide at least one (1) and not more than two (2) references of the most relevant contracts performed for Federal agencies and/or commercial customers within the last three (3) years from the issue date of this RFQ or currently being performed. Relevant performance includes performance of efforts involving moving and storage services that are similar or greater in scope, magnitude, and complexity than the effort described in this solicitation.

The rating of Acceptable or Unacceptable will be assigned based on the responses provided from the references provided. Furnish the following information for each reference listed:

(1) Company/Division Name

(2) Product/Service

(3) Point of Contact, e-mail, and phone number

(4) Contract Number

(5) Contract Dollar Value

(6) Period of Performance

The above information shall be provided as part of the RFQ package. Past performance will be rated as acceptable/unacceptable based on the response from the references provided and any other sources of information available to the Government to access past performance. The Government reserves the right to utilize past performance information gained from other sources (i.e., past performance databases, other government sources, etc.) to aid in evaluating past performance. If you have any questions, please contact Nick Griswold at nicholas.griswold@us.af.mil.

The Government will award a single contract resulting from this RFQ to the responsible quoter whose services are technically acceptable, quotes the lowest overall price with inclusion of option years, and has a past performance rating of acceptable. The Government intends to award a contract without discussions with respective quoters. The

Government, however, reserves the right to conduct discussions if deemed in its best interest.

10. Quoter(s) is(are) required to include a completed copy of the provisions at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with their quotation. A quoter shall complete only paragraphs (b) of this provision if the quoter has completed the annual representations and certificates electronically via https://www.sam.gov. If a quoter has not completed the annual representations and certifications electronically at the SAM website, the quoter shall complete only paragraphs (c) through (o) of this provision.

11. The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition and also

Addendum to FAR 52.212-4.

12. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders --

Commercial Items (Deviation), all FAR clauses cited in the clause are applicable to the acquisition.

13. Additional contract requirement(s) or terms and conditions: None

14. Defense Priorities and Allocations System (DPAS) and assigned rating: N/A

15. Complete quotation is due NLT 11:00 AM CST on 23 February 2015

16. Complete quotations are to be delivered to 502 CONS/JBKCB via email or delivered to 395 B Street W, Suite

2, JBSA Randolph, TX 78150-4525, ATTN: SSgt Nick Griswold, nicholas.griswold@us.af.mil, 210-652-9094 or

Ms. Gina F. Garcia, gina.garcia@us.af.mil, 210-652-5131.

NOTE: The Government reserves the right to cancel this RFQ either before or after the closing date. In the event the Government cancels this RFQ, the Government has no obligation to reimburse any quoter for any costs.

52.209-5 CERTIFICATION REGARDING RESPONSIBILITY MATTERS (APR 2010)

(a)(1) The Offeror certifies, to the best of its knowledge and belief, that-

(i) The Offeror and/or any of its Principals-

(A) Are ( ) are not ( ) presently debarred, suspended, proposed for debarment, or declared ineligible for the award of contracts by any Federal agency;

(B) Have ( ) have not ( ), within a three-year period preceding this offer, been convicted of or had a civil judgment rendered against them for: commission of fraud or a criminal offense in connection with obtaining, attempting to obtain, or performing a public (Federal, State, or local) contract or subcontract; violation of Federal or State antitrust statutes relating to the submission of offers; or commission of embezzlement, theft, forgery, bribery, falsification or destruction of records, making false statements, tax evasion, violating Federal criminal tax laws, or receiving stolen property (if offeror checks “have”, the offeror shall also see 52.209-7, if included in this solicitation); and

(C) Are ( ) are not ( ) presently indicted for, or otherwise criminally or civilly charged by a governmental entity with, commission of any of the offenses enumerated in paragraph (a)(1)(i)(B) of this provision.; and

(D) Have , have not , within a three-year period preceding this offer, been notified of any delinquent

Federal taxes in an amount that exceeds $3,000 for which the liability remains unsatisfied.

(1) Federal taxes are considered delinquent if both of the following criteria apply:

(i) The tax liability is finally determined. The liability is finally determined if it has been assessed. A liability is not finally determined if there is a pending administrative or judicial challenge. In the case of a judicial challenge to the liability, the liability is not finally determined until all judicial appeal rights have been exhausted.

(ii) The taxpayer is delinquent in making payment. A taxpayer is delinquent if the taxpayer has failed to pay the tax liability when full payment was due and required. A taxpayer is not delinquent in cases where enforced collection action is precluded.

(2) Examples. (i) The taxpayer has received a statutory notice of deficiency, under I.R.C. Sec. 6212, which entitles the taxpayer to seek Tax Court review of a proposed tax deficiency. This is not a delinquent tax because it is not a final tax liability. Should the taxpayer seek Tax Court review, this will not be a final tax liability until the taxpayer has exercised all judicial appeal rights.

(ii) The IRS has filed a notice of Federal tax lien with respect to an assessed tax liability, and the taxpayer has been issued a notice under I.R.C. Sec. 6320 entitling the taxpayer to request a hearing with the IRS Office of Appeals contesting the lien filing, and to further appeal to the Tax Court if the IRS determines to sustain the lien filing. In the course of the hearing, the taxpayer is entitled to contest the underlying tax liability because the taxpayer has had no prior opportunity to contest the liability. This is not a delinquent tax because it is not a final tax liability. Should the taxpayer seek tax court review, this will not be a final tax liability until the taxpayer has exercised all judicial appeal rights.

(iii) The taxpayer has entered into an installment agreement pursuant to I.R.C. Sec. 6159. The taxpayer is making timely payments and is in full compliance with the agreement terms. The taxpayer is not delinquent because the taxpayer is not currently required to make full payment.

(iv) The taxpayer has filed for bankruptcy protection. The taxpayer is not delinquent because enforced collection action is stayed under 11 U.S.C. 362 (the Bankruptcy Code).

(ii) The Offeror has ( ) has not ( ), within a three-year period preceding this offer, had one or more contracts terminated for default by any Federal agency.

(2) Principal, for the purposes of this certification, means an officer, director, owner, partner, or a person having primary management or supervisory responsibilities within a business entity (e.g., general manager; plant manager;

head of a division or business segment; and similar positions).

(b) The Offeror shall provide immediate written notice to the Contracting Officer if, at any time prior to contract award, the Offeror learns that its certification was erroneous when submitted or has become erroneous by reason of changed circumstances.

(c) A certification that any of the items in paragraph (a) of this provision exists will not necessarily result in withholding of an award under this solicitation. However, the certification will be considered in connection with a determination of the Offeror's responsibility. Failure of the Offeror to furnish a certification or provide such additional information as requested by the Contracting Officer may render the Offeror nonresponsible.

(d) Nothing contained in the foregoing shall be construed to require establishment of a system of records in order to render, in good faith, the certification required by paragraph (a) of this provision. The knowledge and information of an Offeror is not required to exceed that which is normally possessed by a prudent person in the ordinary course of business dealings.

(e) The certification in paragraph (a) of this provision is a material representation of fact upon which reliance was placed when making award. If it is later determined that the Offeror knowingly rendered an erroneous certification, in addition to other remedies available to the Government, the Contracting Officer may terminate the contract resulting from this solicitation for default.

(End of provision)

52.209-7 INFORMATION REGARDING RESPONSIBILITY MATTERS (JULY 2013)

(a) Definitions. As used in this provision--

(1) The total value of all current, active contracts and grants, including all priced options; and

(2) The total value of all current, active orders including all priced options under indefinite-delivery, indefinite-quantity, 8(a), or requirements contracts (including task and delivery and multiple-award Schedules).

Principal means an officer, director, owner, partner, or a person having primary management or supervisory responsibilities within a business entity (e.g., general manager; plant manager; head of a division or business segment; and similar positions).

(1) Whether the offeror, and/or any of its principals, has or has not, within the last five years, in connection with the award to or performance by the offeror of a Federal contract or grant, been the subject of a proceeding, at the

Federal or State level that resulted in any of the following dispositions:

(i) In a criminal proceeding, a conviction.

(ii) In a civil proceeding, a finding of fault and liability that results in the payment of a monetary fine, penalty, reimbursement, restitution, or damages of $5,000 or more.

(iii) In an administrative proceeding, a finding of fault and liability that results in--

(A) The payment of a monetary fine or penalty of $5,000 or more; or

(B) The payment of a reimbursement, restitution, or damages in excess of $100,000.

(iv) In a criminal, civil, or administrative proceeding, a disposition of the matter by consent or compromise with an acknowledgment of fault by the Contractor if the proceeding could have led to any of the outcomes specified in paragraphs (c)(1)(i), (c)(1)(ii), or (c)(1)(iii) of this provision.

(2) If the offeror has been involved in the last five years in any of the occurrences listed in (c)(1) of this provision, whether the offeror has provided the requested information with regard to each occurrence.

(d) The offeror shall post the information in paragraphs (c)(1)(i) through (c)(1)(iv) of this provision in FAPIIS as required through maintaining an active registration in the System for Award Management database via https://www.acquisition.gov (see 52.204-7).

52.209-9 UPDATES OF PUBLICLY AVAILABLE INFORMATION REGARDING RESPONSIBILITY

(1) The non-public segment, into which Government officials and the Contractor post information, which can only be viewed by--

(i) Government personnel and authorized users performing business on behalf of the Government; or

(ii) The Contractor, when viewing data on itself; and https://www.acquisition.gov/ https://www.acquisition.gov/

(iii) Information that is withdrawn during the 14-calendar-day waiting period by the Government official who posted it in accordance with paragraph (c)(1) of this clause.

(c) The Contractor will receive notification when the Government posts new information to the Contractor's record.

(1) If the Contractor asserts in writing within 7 calendar days, to the Government official who posted the information, that some of the information posted to the non-public segment of FAPIIS is covered by a disclosure exemption under the Freedom of Information Act, the Government official who posted the information must within

7 calendar days remove the posting from FAPIIS and resolve the issue in accordance with agency Freedom of
