PERFORMANCE WORK
STATEMENT
FOR
INSPECTION OF No. 6
FUEL OIL TANKS
PREPARED BY:
66 ABG/ CENMP
04 December 2017
1. DESCRIPTION OF SERVICES:
1.1. Scope: The contractor shall provide all labor, tools, equipment, hoses, gauges, test equipment, hearing protection, eye protection including prescription safety glasses, materials, supplies and parts, transportation and other incidentals necessary to travel to Hanscom AFB, MA, to clean all materials from (2) No. 6 Fuel Oil Tanks, and replace heaters. The contractor shall be responsible for insuring all services and parts provided under this contract are in accordance with the terms and conditions outlined herein and conform to the manufacturer’s equipment specifications and this Performance Work Statement (PWS).
1.2. Requested Service: Coordinate and supervise cleaning, inspection of No. 6 Fuel Oil Tanks, and replace oil heaters located at Building 1201 Hanscom AFB, MA 01731.
1.2.1. Tank Cleaning, Inspection, and repair Phasing: The fuel tank cleaning, heater repair, and inspection will be phased for cleaning, repairing, inspecting one (1) tank at a time. One tank is almost empty and the fuel will need to be transferred into the other tank or disposed of off-site to allow for the cleaning of one tank to commence. Once one tank is cleaned and inspected, the contractor will fix the oil heater and transfer the oil to the clean tank. Then the remaining tank will be cleaned, oil heater replaced inspected.
1.2.2. Tank Cleaning: Consists of pressure washing/ scrubbing interior walls, ceiling, and base of the fuel tanks to remove sludge and build up from interior of the tank. All sludge and build up waste will be removed from the tank and disposed of properly. All the tank’s interior surfaces shall be cleaned to allow for adequate inspection complying with the latest edition of API 653, UFC 3-460-03, 502 CMR 5.00, 527 CMR 9.00, and NFPA 30 Flammable and Combustible
Liquids Code.
1.2.3. Oil Heater Replacement/ Repair: The tow (2) oil heaters inside the tanks are well over
5 years old and need to be replaced. Currently one (1) heater is broken. Both heaters will be replaced with Government Furnished Heaters located on site. Each heater will be replaced after the tank has been cleaned/ inspected but prior to filling with new oil. The current oil heaters in the tank shall be clean, repaired, refurnished, packaged, and returned to the Government for future use.
1.2.4. Tank Inspection: Tanks inspections shall comply with the latest edition of API 653, UFC 3-460-03, 502 CMR 5.00, 527 CMR 9.00, and NFPA 30 Flammable and Combustible
Liquids Code.
1.2.5. Inspection Report: The tank inspection report shall include the following but is not limited to:
• Settlement Evaluation: Examine whether the tank is tilting or settling in the ground improperly (Out of plane, Rigid Body Tilting, Uniform, Dish Type, Edge Settlement, Localized Dispersions).
• The minimum remaining thickness, minimum required thickness, and the maximum corrosion rate shall be noted for the roof, shell, and bottom of the tank.
• Inspection type performed, reasoning on why specific type was selected, and other method used to verify measurements/ results (Ultrasonic spot or can, liquid penetration, penetrating oil, magnetic particle radiography, magnetic flux scan, vacuum box, or tracer gas).
• Verification of general tank information (tank specifications, tank construction, release detection method, spill control).
• Include repair recommendation for critical repairs, improving tanks, and extending the life of the tanks. Include repair type, quantity of repair, cost to repair the tanks, and criticality of repair.
1.2.6. Confined Space: This contract has the potential for entry into permitted and non-permitted confined space areas. Permit required confined space areas will require, IAW AFI
91-203, The U.S. Air Force Mishap Prevention Program, and AFFARS Clause 5352.223-9001, Health and Safety on Government Installations, a pre-brief prior to the start of any service which includes the contractor, COR, Safety Office and Fire Department. It is the contractor’s sole responsibility for compliance with OSHA (Public Law 91-956. The pre-brief will inform the contractor of any known hazards and the contents of the space. Additionally, the contractor will be briefed on precautions and procedures implemented by 66 ABG/CE to protect Air Force workers. The Fire Chief will coordinate the confined space entry contract and either approves or disapproves use of the rescue team supplied by the installation FES
Flight. At the conclusion of the contract, an after-action brief will be required where the con-tractor will provide information regarding hazards encountered or created in the space(s).The
COR will contact the CO and ABG/SE immediately if an unsafe act or procedure is per-formed by the contractor.
1.2.7. Records and Documentation: The contractor shall provide all necessary documents, tickets, receipts, and records within three to five business days. Document all safety hazards on Fuel Tanks. The amount (gallons) disposed should be transmitted back to the
Contracting Officer for environmental metrics required by the Environmental Branch. Fill out and return PMTL check list to COR.
1.3. Quality Assurance: According to the contract’s inspection clause, the government will evaluate the contractor’s performance under this contract. The COR is a representative of the
Contracting Officer (CO) and will participate in the administration of this contract. Any matter concerning a change to the scope, prices, terms or conditions of this contract shall be referred to the CO. All services to be performed by the contractor will be subject to review by the CO or COR. The contractor shall provide an answer to a submitted Corrective Action Re-port (CAR) and Customer Complaint Form from the COR within one (1) working day from receipt. The contractor shall provide written response addressing the root cause, corrective action and preventive actions (s).
1.4. Continuation of Essential Services during Crisis: This requirement has been deter-mined not to be essential and does not require continued support during a crisis as defined in DFARS Clause 252.237-7023.
1.5. Period of Performance (POP): The POP for this services is Ninety (90) calendar days.
1.5.1. POP Detail: The contractor has s 15 day period from notice to proceed to submittal all required documentation and 85 days to clean, inspect, provide a report for the tanks, replace tank heaters, and repair old heaters.
1.6. Hours of Operation: The contractor shall perform work Monday thru Friday, under this contract during the following hours: 07:00 a.m. until 3:30 p.m., excluding Federal Holidays.
Work required to be performed outside of these hours shall only be done after approval from the Contracting Officer.

1.7. Contractor Personnel: The contractor shall designate in writing the name of the Con-tract Manager (CM) and any alternate(s) who shall act for the contractor when the manager is absent and their telephone numbers where they may be contacted any time to the CO and the
COR within five (5) calendar days after contract award. The CM or alternate shall have full authority to act for the contractor on all matters relating to operation of this contract at Hans-com AFB. The CM or alternate shall be available to meet on the installation with government personnel designated by the CO to discuss problem areas.
1.7.1. Listing of Contractor Employees: The contractor shall provide a CM who can read, write, speak, and understand English fluently. The contractor shall provide an initial listing of contractor employees’ names and position titles to CO and COR prior to contract start date.
The contractor shall also provide an updated listing of personnel to the CO and COR within ten (10) work days when employees affecting the work of this contract have a status or position change.
1.7.2. Employment: The contractor shall not employ persons on this contract if such employee is identified to the contractor as a potential threat to the health, safety, security and general well-being or operational mission of the installation and its population. The contractor shall not employ any person who is an employee of the United States Government if the employment of that person would create a conflict of interest, nor shall the contractor employ any person who is an employee of the Department of the Air Force, either military or civilian, unless such person seeks and receives approval according to the DoD 5500.7-R Joint Ethics
Regulations (JER). In addition, the contractor shall not employ any person who is an employee of the Department of the Air Force if such employment would be contrary to the policies contained in AFI 64-106, Air Force Industrial Labor Relations Activities. The contractor shall be prohibited from employing off-duty COR’s who are surveilling any contracts/subcontracts awarded to the Contractor. All contract employees depending on particular condition, occasion or place, or job assignment shall require professional, and suitable attire. All contractor employees shall conduct themselves in a professional, courteous manner. The selection, assignment, reassignment, transfer, supervision, management, and control of contractor employees in performance of this PWS shall be the responsibility and prerogative of the contractor; however, the con- tractor shall comply with the requirements set forth in this PWS and Hanscom AFB regulations concerning conduct of employees as referenced herein. The contractor shall employ and maintain a technically trained and experienced work force.
1.7. Environmental Protection and Safety: The contractor shall be knowledgeable of and comply with all applicable federal, state and local laws, regulations and requirements regard-ing environmental protection and occupational safety and health. The contractor shall comply with any laws or regulation changes during the term of this contract. The contractor shall fol-low manufacturer’s guidelines and professional recommendations for control of humidity, temperature, cleanliness and materials handling (including hazardous materials).
1.8. Hazardous Material (HAZMAT): The contractor shall not store chemicals/HAZMAT on Hanscom AFB. All chemicals/HAZMAT shall be approved prior to being transported on base for use by any contractor. The contractor shall prepare and submit to the COR, ten (10)
Calendar Days: after contract award date, one (1) each, Chemical/Hazardous Material Re-quest Authorization Form (Air Force Form 3952) and Safety Data Sheets (SDS) for each chemical, material and substance proposed to be used in performance of this PWS. The COR will forward the completed AF Form 3952(s) and SDS to 66 ABG/CEIE for approval. The contractor shall not transport or use any chemical, material or substance on Hanscom AFB before written approval is provided by 66 ABG/CEIE. Contractor shall maintain copies of
SDS(s) for all chemicals. The SDS(s) shall correspond with the manufacturers of the chemicals on hand. Manufacturer, material, and SDS shall match. NOTE: SDS refers to manufacturer
SDS. Once material is approved for use, contractor shall contact base Environmental
Management (EM) (66 ABG/CEIE) and provide a list of all material. 66 ABG/CEIE shall determine any additional material requirements based on the hazard code of the material.
1.9. Hazardous Waste: Hazardous waste (e.g., chemical solutions and used rags in waste drums) is not to be disposed to solid waste dumpsters. For assistance in making determinations as to what waste is hazardous, please contact the Environmental Branch. If wastes is determined to be hazardous, then the hazardous waste manifests need to be signed by the Environmental Branch.
1.9.1 Hazardous Waste Cont. : Massachusetts requirements. 310 CMR 15.230. Mass. State law for grease traps. Per Massachusetts requirements, traps require cleanout if over 25% of the effective depth of the trap, or at least three months, whichever is sooner. Oil/grease is regulated by the State of Massachusetts as a hazardous waste under 310 CMR 30.000, The contractor must manifest it on a hazardous waste manifest form to be signed by CEIE under
310 CMR 30.000.
1.10. Discharges: Any discharges into industrial waste treatment plant (IWTP) or sanitary sewer shall be done in accordance with Hanscom Industrial Waste Water Discharge Permit as instructed by 66 ABG/CEIE. The contractor shall maintain records of written authorization issued by 66 ABG/CEIE for discharges into IWTP and sanitary sewer.
1.11. Spills: Upon occurrence of any spills that could enter the storm system or cause any harmful environment effects, the contractor shall immediately call 911 to report the incident.
When possible, the contractor shall use a base telephone to receive help from Hanscom AFB
911. If using a cell phone, request Hanscom AFB 911. The contractor shall submit a verbal notification immediately after the incident is discovered, not to exceed 30 minutes so that the
Environmental Branch can meet state 2-hour notification requirements. The contractor should not respond unless RCRA certification allows for them to cleanup and respond to a spill. The contractor shall report all environmental violations to the COR.
1.12. Environmental and Safety Training: The contractor personnel shall have Resource
Conservation Recovery Act (RCRA) environmental and safety training required by the state and federal government. The training shall be IAW the following regulations: Resource
Conservation Recovery Act (RCRA) 40 Code of Federal Regulation (CFR) 265.16 “Personal
Training”, Occupational Safety and Health Administration (OSHA) Hazardous Waste
Operations and Emergency Response Standard Title 29, CFR Part 1910.120 “Hazardous
Waste Operations and Emergency Reponses”, Hazardous Waste Management Instruction, Section 13, and Air Force Instruction (AFI) 32-7042, “Solid and Hazardous Waste
Compliance”, Section 2. The contractor shall provide the environmental and safety training certificate for all employees to the CO and COR, after contract award date. The
Environmental Branch will review the RCRA certificates to make a determination of their ability to respond to spills prior to their arrival.
1.13. Forms and Publications, TO, AFI and AFMCI: The contractor shall obtain the forms and publications expressly required to perform the work in this contract via internet at www.e-publishing.af.mil In the event that a form or publication is not available online, the
COR will provide the form, instruction, or publication to the contractor upon written request
(e-mail is acceptable) by the contractor. In the event that laws, regulations, TO’s, AFI, AFMCI’s change during the term of this contract, the contractor is required to comply as such changes come into effect. Contractors delivering or handling official USAF records shall comply with the applicable records management regulations AFI 33-322, Air Force Manual
(AFMAN) 33-363, and AFI 37-138 (in their entirety).
1.14. Housekeeping: The contractor shall keep the work areas clean and neat in accordance with Industry Standards, Occupational, Safety and Health Administration (OSHA), fire and safety standards and this PWS.
1.15. Tool Control and Accountability: The contractor or contractor’s personnel shall inventory the container or bag of tools at the beginning of each work day and at the end of each work day. Contractor personnel shall immediately notify the COR of any tools that are http://www.e-publishing.af.mil/ missing or lost from the work area or container (i.e. tool or job box or tool bag). The COR will report to appropriate government personnel of any missing contractor’s tool(s) not located within one hour after notification of lost or missing tools.
1.16. Privately Owned Motor Vehicles (POV): Contractor personnel shall comply with the directives pertaining to operation of POVs on Hanscom AFB per AFI 31-204 (in its entirety), Motor Vehicle Traffic supervision.
1.17. Visitor Pass:
The contractor shall comply with requirement and procedures IAW FAR 5353.242-9000, Contractor Access to Air Force Installations. Prior to coming to Hanscom AFB to perform any work, Contractor shall contact the Civil Engineering Government Representative to coordinate access to the base as well as to the building where the work shall be performed. A list of the names of the workers will be required to gain access, and passes shall be distributed at the visitor’s building at Vandenberg Gate. Workers shall have proof of identification as well as a valid license. If bringing a vehicle on base, the contractor will need proof of registration and insurance of that vehicle. Contractor employees must have identification that readily identifies them as contractors. During the course of the contract, certain areas/sites may be restricted or closed to contractor personnel. The contractor shall comply with all requirements for entry. In addition, unscheduled gate closures may occur at any time and personnel entering or exiting the installation may experience delays.
In order to maintain accountability once gaining access onto Hanscom AFB, Contractor shall immediately sign the logs at Building 1810, indicating time and date of arrival. Before departure, Contractor shall again sign the logs indicating the departure time, as well as the work performed and the buildings worked in. A copy of a signed work receipt on Contractor’s letterhead detailing work performed shall also be given to the Civil Engineering Government
Representative or left in the log book prior to departure.
1.18. Foreign National(s): The contractor shall notify the COR before sending a Foreign
National representative to perform services. Special rules apply for foreign national(s) visiting a US Government facility.
1.19. Health and Safety on Government Installation: In performing work under this con-tract on a Government installation, the contractor shall comply with federal, state and local regulations regarding occupational safety and health and the specific health and safety requirements established by this contract. The contractor shall notify the COR of any injuries that occur to contractor personnel while on the installation. The contractor shall notify the
COR of damage to government property or equipment during the execution of the contract.
1.20. Emergency Procedures: Contractor employees shall follow the direction of
Government employees in regards to emergency evacuation procedures, i.e. fire, tornado, active shooter and bomb threats. It is the contractors’ responsibility to account for their employees following an emergency evacuation. Contractor personnel shall participate with government personnel in all fire and tornado drills and at no additional cost to the government.
1.21. Partnering Agreement: The contracting officer may require a partnering agreement between the government and the contractor to ensure joint cooperation and a sound partner-ship of all parties involved in the execution of this contract. Partnering is the creation of a government-contractor relationship that promotes achievement of mutually beneficial goals.
It involves an agreement in principle to share the risks involved in completing the project and to establish and promote a nurturing partnership environment. Representatives from each organization are encouraged to participate in developing the partnering agreement. Suggested representation is the civil engineer manager, the government inspector, the government contract administrator, the contractor’s manager and the contractor’s quality control person.
All costs for the partnership agreement should be shared equally between the government and contractor. This group is responsible for developing a formal partnering agreement that should be signed by all parties involved. The agreement should contain as a minimum:
specific goals to be reached and a list of objectives to reach the goals, a set of metrics to evaluate the objectives, a frequency for meetings to review the metrics, and a statement of cooperation to execute the terms of the agreement.
SECTION 2
Performance Assessment
The contractor shall be aware that in the absence of any contract requirement from the
Service Summary does not detract from its enforceability nor limit the rights or remedies of the government under any other provision of the contract. The contractor shall ensure the following service summary requirements are accomplished.

SECTION 3
GOVERNMENT-FURNISHED PROPERTY AND
SERVICES
3.1. Government Furnished Property (GFP): Government Furnished Property is applicable to the in the performance of this contract. The Government will be providing two (2) heater tube bundles, each tube bundles has 38 tubes. The manufacturer and item number for the heater tube bundles is Yula #OSU-2D-120B.
3.2. Reserved.
3.3. Government Furnished Services: Government Furnished Services are applicable during the performance of this contract.
3.3.1. Security Police: The government will provide general on base Security Forces service. The Security Forces’ telephone number is 911 for emergencies, for crime in progress 781- 225-5613 and 781-225-5613 for non-emergency calls.
3.3.2. Fire and Emergency Services: The government will provide general on-base fire protection services. The contractor shall comply with fire prevention rules and regulations enforced by the Hanscom AFB Fire Prevention Branch. Call 911 for emergencies. For routine fire prevention matters and/or calls to dispatch, call 781-225-
3330.
3.3.3. Emergency Medical Service: The government will provide 24 hour on-base state certified paramedic level emergency response and patient transport services for contractor personnel who are injured or become seriously ill while on Hanscom AFB.
The telephone number is 911 for emergencies.
Note: Calling 911 from a cell phone will be received by off base emergency 911 systems, causing a delay in response time by emergency personnel. When possible use a base tele- phone to receive on base help.
3.3.4. Utilities: The government will not furnish utility services. I t shall be the contractor’s responsibility to provide all required utilities to perform the work.
3.3.5. Refuse Collection: The government will not provide refuse collection services.
The Contractor will be responsible for disposing all generated wasted under this service.
3.3.6. Workspace: The government will provide adequate workspace around the equipment area.
