About this file

This document is an official protocol for incident reporting by service providers working with the Arizona Department of Juvenile Corrections (ADJC). The document outlines specific notification procedures for service providers when unusual or significant incidents occur involving juveniles under ADJC jurisdiction, including dual-ward cases that involve both delinquent and dependent youth. The protocol applies to service providers across Arizona and provides detailed guidelines for reporting to ADJC staff during both regular business hours and after-hours periods.

The document defines a comprehensive list of reportable incidents, which range from serious events like juvenile death or injury to programmatic issues such as AWOL status, substance use, law enforcement contact, and transportation-related events. Service providers are required to make verbal reports within two hours of an incident and submit written documentation within 24 hours, with specific reporting requirements including juvenile identification, detailed incident description, staff information, and follow-up actions. The protocol mandates reporting through designated contact numbers, including the Community Services Office (602-542-4157) during business hours and the After-Hours Administrator (602-526-6063) outside standard business times.
