About this file

This document is an incident reporting protocol for the Arizona Department of Juvenile Corrections (ADJC), specifically outlining service providers' responsibilities for reporting unusual or significant incidents involving juveniles under ADJC jurisdiction. The protocol applies to service providers working with ADJC juveniles, including both delinquent and dependent youth with an assigned ADJC Parole Officer, and involves the ADJC Office of Procurement located in Phoenix, Arizona.

The protocol details specific reporting requirements for incidents, mandating verbal notifications within 2 hours and written reports within 24 hours through designated communication channels. Reportable incidents include a comprehensive list of 17 event types such as serious injuries, use of restraint, sexual allegations, law enforcement contacts, AWOL status, substance use, and other safety or programmatic impacts. The protocol provides explicit contact information for reporting during regular business hours (602-542-4157) and after-hours (602-526-6063), with specific script guidelines for initial notifications and requirements for subsequent detailed written documentation to be submitted via fax or email.
