About this file
This document is an exhibit detailing the Unusual/Significant Incident Report Protocol for the Arizona Department of Child Safety (ADJC), specifically outlining service providers' reporting responsibilities for incidents involving juveniles under ADJC jurisdiction. The protocol applies to service providers working with ADJC juveniles, including dual wards who are both adjudicated delinquent and dependent, and involves reporting procedures for the Community Services Office and ADJC Parole Officers.
The protocol establishes comprehensive reporting requirements for a wide range of incidents, including but not limited to deaths, serious injuries, restraint use, sexual incidents, law enforcement interactions, absences, assaults, property damage, substance use, and other critical events. Service providers must make verbal reports within two hours of an incident during both regular business hours (8:00 AM - 5:00 PM Monday-Friday) and after hours (5:01 PM - 7:59 AM weekdays, weekends, and holidays), followed by a detailed written report within 24 hours. The written report must include specific information such as juvenile identification, incident details, staff information, and any follow-up actions, to be submitted via fax or email to the designated ADJC contact information.
