About this file

This document is a service provider protocol for the Arizona Department of Child Safety (ADJC), outlining reporting requirements for incidents involving juveniles under ADJC jurisdiction. The protocol applies to service providers working with ADJC juveniles, including those who are dual wards (adjudicated delinquent and dependent), and establishes specific notification procedures for both regular business hours and after-hours situations.

The protocol details comprehensive incident reporting requirements, mandating verbal notifications within 2 hours and written reports within 24 hours for a wide range of incidents including, but not limited to: medical emergencies, use of restraint, sexual assault allegations, law enforcement contacts, substance use, weapons violations, AWOL status, and other safety or programmatic events. Service providers must submit reports via fax or email to the ADJC Community Services Office, including specific details such as juvenile identification, incident description, environmental factors, and follow-up actions. The protocol provides a standardized script for verbal notifications and specifies contact numbers for different reporting scenarios.
