About this file
This document is an Exhibit from the Arizona Department of Child Safety (ADJC) detailing a Service Providers Unusual/Significant Incident Report Protocol. The document specifically outlines the reporting requirements and responsibilities for service providers working with juveniles under ADJC jurisdiction, including those who are dual wards (both delinquent and dependent). The protocol applies to service providers during regular business hours (8:00 AM - 5:00 PM, Monday-Friday) and after hours (5:01 PM - 7:59 AM weekdays, and all weekend/holiday hours).
The protocol mandates that service providers report incidents within two hours of occurrence through verbal and written communications. Reportable incidents include a wide range of events such as juvenile death, serious injury, use of restraint, sexual incidents, law enforcement contacts, absences without leave, assaults, property damage, substance use, and other significant events. The reporting process requires providers to contact specific ADJC offices (Community Services Office or After Hours Administrator) and submit detailed written reports within 24 hours, including comprehensive information about the incident, juvenile details, staff involvement, and any follow-up actions. The document provides specific contact numbers, reporting scripts, and expects thorough documentation of each reportable event.
