About this file
This is a Small Procurement Contract template from the State of Maryland Department of Health at Clifton T. Perkins Hospital Center (CTPHC). The contract serves as a framework agreement for the procurement of goods or services, with specific details to be inserted based on individual solicitations. The contract term is flexible and determined on a case-by-case basis, with specific start and end dates to be established for each engagement. The related state and local contract opportunity indicates that CTPHC is seeking a contractor to provide plumbing services as needed for the facility, though this template does not contain specifications unique to any particular plumbing services contract.
Compensation under this contract type shall not exceed $50,000, with payment due within thirty days of invoice receipt. The contractor must submit invoices in triplicate by the 15th of the month for the preceding calendar month, including required information such as contractor name, remittance address, invoice period, state-assigned contract number, and purchase order numbers. Victoria F. Bell is designated as the Procurement Officer for contract administration and coordination. The State reserves the right to terminate the contract for convenience without cause or for default if the contractor fails to fulfill obligations, and the contract automatically terminates if funds are not appropriated in subsequent fiscal years. All contractors must comply with Maryland and federal nondiscrimination laws and the Commercial Non-Discrimination Policy under State Finance and Procurement Article Title 19.
