About this file

This is a Cost Detail Attachment for a Request for Proposal issued by the Minnesota Department of Health (MDH) seeking proposals to develop, calculate, and report on healthcare affordability standards for Minnesota. The project consists of seven core tasks: (1) kick-off meeting, workplan, and project management; (2) engagement of the Health Care Affordability Advisory Task Force; (3) development of options; (4) facilitation of a Technical Panel; (5) finalization of affordability standards options; (6) calculation of standards and Minnesota's performance against them; and (7) communications. The contract will be structured as a deliverables-based contract for Tasks One through Seven, plus any selected optional additional deliverables, with Task Eight operating as a time and materials contract at the State's discretion. Responders must provide estimated hours and total costs for each task completion, with proposals remaining open for acceptance until contract execution, solicitation cancellation, or 180 days after the due date, whichever occurs first.

All costs including travel expenses, mass mailings, fees, commissions, equipment, and supplies must be incorporated into the rate structure or itemized as separate line items for travel in accordance with the current Commissioner's Plan. Travel costs must be broken out separately while all other expenses are to be included as part of tasks and deliverables. Responders are required to submit a personnel list identifying all individuals expected to perform work under the contract, including their job titles and maximum hourly rates, which will be used during contract negotiation. Cost information must be submitted as a separate document from the technical proposal, and the State will only make payments for services performed or work delivered after completion. The State reserves the right to negotiate additional tasks within the scope of the solicitation with the highest-scoring responder.

ATTACHMENT C: COST DETAIL

Responders must submit Attachment C “Cost Detail” form as their Cost Proposal. The rate(s) identified in the Cost Proposal must include all costs, including but not limited to: travel expenses (e.g., flights, accommodation, meals), mass mailings, fees, commissions, compensation, indirect costs, equipment, supplies, and other charges. Travel costs are to be broken out separately on a line item, while all other costs must be included as part of tasks and deliverables. Deliverables correspond to the Required Tasks and Deliverables in Section 2 of the Request for Proposal (RFP).

Identify the level of the State’s participation in the contract and details of cost allowances for this participation. The State does not make regular payments based solely upon the passage of time; it only pays for services performed or work delivered after it is accomplished.

Submit the Attachment C: Cost Detail as a separate document(s) from your technical response for all copies of the Proposal. Do not include any cost information in the Technical Proposal part of the response. The Proposal must be open for acceptance until a contract is executed, the Solicitation is cancelled, or 180 days after the due date and time of the Solicitation, whichever comes first.

Responder must complete each of the tables below. The contract resulting from this solicitation will be a deliverables-based contract including Tasks One through Seven and any selected optional additional deliverables (as described in Section 3 of this Solicitation) and a time and materials contract for Task Eight. The State reserves the right to negotiate additional tasks within the scope of this solicitation with the highest scoring responder.
