About this file

This document is an Addendum to the original Proposal submitted by J Michael Consulting, LLC for the Oregon Health Authority's Focused Market Research for Public Health Lab Newborn Screening and Laboratory Information Management System (LIMS) project. The addendum outlines additional budget line items for ad hoc consulting services on a Time and Materials (T&M) basis up to $12,000, as well as a Travel and Expenses (T&E) budget cap of $4,000. This flexible budget will allow the Oregon State Public Health Laboratory to request additional tasks or services from subject matter experts that fall within the general scope of work but exceed the expected level of effort described in the original Proposal.

The total proposed budget and not-to-exceed cost for this project is $150,000. J Michael Consulting will work with the Oregon State Public Health Laboratory to submit change requests through the OHA's internal change request process to allocate the T&M and T&E budgets. This approach aims to provide cost predictability while maintaining flexibility to bring in additional resources if needed to complete the project.

Oregon Health Authority Focused Market Research for Public Health

Lab Newborn Screening and Laboratory Information Management System (LIMS)

May 3, 2022

Addendum 1 Submitted by: J Michael Consulting, LLC

Exhibit A-1 Proposal Addendum 1

Addendum – OHA Focused Market Research

8/17/2016 2

J Michael Consulting (JMC) submits this addendum to our Oregon Health Authority Focused Market Research for Public Health Lab Newborn Screening and Laboratory Information Management System (LIMS) Proposal, dated April 6, 2022. While JMC anticipates completing the deliverables specified under this contract via the fixed price quote, the scope of a complex informatics project and the services needed to complete it can be difficult to accurately predict. JMC therefore recommends two additional budget line items to provide ad hoc consulting services; a Time and Materials (T&M) budget cap of $12,000 and a Travel and Expenses (T&E) budget cap of $4,000. With this flexible budget, Oregon state Public Health Laboratory can request additional tasks or services that fall within the general scope of work but exceed the expected level of effort of specific tasks and services described in the original SOW in our Proposal. The T&M and T&E budgets will be accessible through a change request process to be outlined in the contract and at the request of the Oregon State Public Health Laboratory (OSPHL). See Table 1 below for summary of original Proposal and Table 2 for this Addendum 1.

In order to allocate these funds, JMC will work with OSPHL to submit a change request through OHA’s internal change request process. The work would be awarded by a mutually accepted change order that would outline the specific services, tasks, and deliverables that would be performed

We believe the approach of combining a fixed price contract with a more flexible T&M/T&E budget will be to Oregon’s benefit in that overall costs are capped, and the budget will be predictable, but we will still have the flexibility to provide additional services or to bring in additional subject matter experts (SME), if necessary. These SMEs include LIMS configuration analysts, system integration experts, Rhapsody programmers, and standards and interoperability specialists. However, if the project does not require these extra hours, OHA may gain some cost savings.

JMC’s full labor rates for all labor categories under GSA SIN 54151S IT are listed below; this list is also available on GSA Advantage.
