About this file

This is a piggybackable contract (P22-005-O26) between the State of Wisconsin, acting on behalf of the Board of Regents of the University of Wisconsin System for the University of Wisconsin-Milwaukee, and Mauk Swanson, LLC. The contractor is located in Green Bay, Wisconsin, and provides sign specification writing and consulting services. The contract includes email correspondence documenting an insurance waiver approval by the university's Risk Manager.

The contract covers onsite consultation and technical specification services for UWM's procurement processes, including planning, design, production drawings, artwork, and written technical specifications for signage projects. The maximum blended hourly rate is $74.00, with no price escalation permitted during the contract term. The initial contract period runs from February 15, 2022, through February 14, 2023, with four automatic one-year renewal options unless either party provides 90-day notice of non-renewal. The contract allows multiple awards and is available to all UWM departments including outlying locations. Payment terms include acceptance of charge cards with no fees and purchase orders with net 30 terms, though the contractor prefers checks or direct deposit for invoices over $1,000. Notably, insurance requirements were waived due to the contractor being a sole proprietor working from a home office with minimal risk exposure.

From: Pamela K Loignon Sent: Monday, January 24, 2022 1:57 PM To: James J Olson <olson69@uwm.edu> Cc: alswnsn@gmail.com <alswnsn@gmail.com> Subject: Insurance QuesƟon Regarding a Bid Award

Hi Jim, I am in the process of awarding a bid for a Sign SpecificaƟon Writer / Consultant to assist Mike with upcoming projects on campus.

When I put together the bid, I used our standard language:

Insurance: The Contractor shall maintain Standard Insurance levels as required here:

http://uwm.edu/risk-management/insurance/certificates-of-insurance/.

A certificate of insurance must be provided upon award and provided on an annual basis throughout the term of the Contract. The Contractor shall add: “The Board of Regents of the University of Wisconsin System, its officers, employees and agents as an ‘additional insured’ under the commercial general and automobile liability policies.” The certificate holder shall be listed as the University of Wisconsin-System Administration or System campus for their respective purchases.

We would like to award to Mauk Swanson LLC and one other company,. I have been working with Andrea in the CC above from Mauk Swanson LLC.

As projects come up, one or the other company could be used, or Mike may choose to go outside of our contract. ( More than likely not, but it could happen). The contract is for a total of 5 years.

Andrea currently does not meet the insurance requirements.

2 of 3 2/9/2022, 8:04 AM

Per an email from Andrea, this is what she has stated:

I am the sole employee of Mauk Swanson, LLC.

My standard homeowners and auto insurance (plus umbrella) has been enough coverage for the type of work I

do. I work out of my home office, and I drive my personal vehicle to site visits. I do not plan on using any motorized equipment or tools while onsite.

Jim, is there any type of excepƟon that can be made? I have included Andrea in the email in case you need documents or have quesƟons.

Thank you so much!!
