About this file

This document is a Texas Family Code Schedule for Solicitation No. 601440000050247, issued by the State of Texas, relating to ineligibility to receive state grants, loans, or contract payments. The schedule requires respondents to provide information about ownership and child support obligations, with a specific focus on entities or individuals holding at least a 25% ownership interest in the business submitting a response.

The document mandates disclosure of social security numbers for owners meeting the ownership criteria, with a privacy notice explaining that this information will be used to identify potential child support obligations. Respondents must complete the schedule accurately, as failure to do so may result in the response being considered non-responsive or potentially lead to contract termination. The schedule was revised on September 27, 2023, and must be signed and returned with the response, with provisions for entities that do not meet the specified ownership criteria to indicate such by checking a designated box.
