Replace Elevator Tile on all 11 elevators 1 06/25/18

1.0 Purpose:

Elevators personnel cab need the floor tile replaced at Peter W. Rodino Federal Building 970

Broad Street, Newark NJ 07102. The elevators tile replacement work will be done at night. Only one elevator per bank will be allowed to be out of service at a time.

2.0 Scope of Work:

Ensure appropriate materials for the removal and installation of the elevator cab tile floor project are received and staged on-site prior to commencing any work or installation. Materials shall be inspected satisfactory prior to the commencement of any work. There are 11 elevator cab floors to be replaced 4 low rise, 6 high rise, and 1 garage car elevator.

Specifically:

1. Lock out one elevator car per bank.

2. Remove elevator cab floor tiles.

3. Clean and level floor in elevator cab

4. Prepare elevator cab floor for tile installation.

5. Install elevator cab floor tile( samples provided by Contractor for GSA selection).

6. Return Elevator to service.

7. Remove all trash; area must be secure and cleaned nightly.

8. All work to be scheduled through the Rodino building manager.

9. Perform ten (10) more times to complete a personnel elevator cabs.

Restore all systems controls to the normal operating configuration, perform area clean-up, secure work areas and inform appropriate buildings personnel of work completion.

3. Deliverables:

Project deliverables shall include:

Specifically:

1. Lock out one elevator car per bank.

2. Remove elevator cab floor tiles.

3. Clean and level floor in elevator cab

4. Prepare elevator cab floor for tile installation.

5. Install elevator cab floor tile.

6. Return Elevator to service.

7. Remove all trash; area must be secure and cleaned nightly.

8. All work to be scheduled through the Rodino building manager.

9. Perform ten (10) more times to complete a personnel elevator cabs.

4.0 Period of performance:

All work on this contract is to be completed within 30 days after receipt of the Notice to

Proceed.

Hours of Work: All work to be performed After the normal building hours of 7:00 am to 4:00 pm, M – F, or as directed by GSA management, excepting those evolutions creating significant noise and or disruption to building activities. Additionally, all work shall be specifically coordinated through the appropriate facility building manager.

5.0 Standards, Codes and Regulations:

MATERIALS SHALL BE SUPPLIED CONSISTENT WITH, OR EXCEEDING, THE QUALITY OF

EXISTING COMPONENTS AND SHALL BE CONSIDERED ACCEPTABLE REPLACEMENTS

BY A FACTORY CERTIFIED EQUIPMENT REPRESENTATIVE. REFURBISHED EQUIPMENT

MEETING THESE STANDARDS IS SPECIFICALLY ALLOWED IF A COMPARABLE AND

COMPLETE EQIUPMENT AND INSTALLATION WARRANTY ACCOMPANIES SAID

MATERIALS.

All work activities shall be performed in a manner consistent with guidance as set forth by the IBEW and OSHA for safety of personnel and equipment. Work areas shall be established consistent with the standards for work on or around energized equipment and all appropriate electrical safety considerations shall be taken.

6.0 Products and Workmanship:

The contractor shall protect adjacent areas at all times during the construction. Any damage caused as a result of the contractor’s activities shall be repaired at no additional cost to the Government and shall in no way adversely affect required schedules and guarantees.

Dimensions as referenced are approximated and shall be used for determining size and location of the work. The contractor shall field verify all existing dimensions, sizes and field conditions, utilize such as basis for their work and notify the government of any potential issues prior to the commencement of work.

All materials shall be verified of appropriate quality and condition prior to installation.

Appropriate actions shall be taken to ensure adequate protection to existing equipment and finishes.

Workmanship shall conform to the practices of the trade and to be in accordance with all Federal, State and local codes.

WARRANTY: Full warranty on all labor and materials shall be provided upon installation and carry through a period of one year.

7.0 Special and / or Specific Requirements

USE OF BUILDING FACILITIES: The contractor may use utilities, services and toilet facilities of the buildings, subject to the approval of the Facilities Manager. The Contractor shall prohibit his employees from disturbing papers on desks, opening desk drawers or cabinets, or using telephone or office equipment provided for official Government use. The Contractor shall require his employees to comply with the instructions pertaining to conduct and building regulations issued by duly appointed officials.

FIRE HAZARDS: The contractor shall take every precaution to prevent fires during the performance of this contract. The Contractor shall furnish a covered metal receptacle to be kept on the exterior of the building to place therein all combustible material such as oily rags, waste, etc., used in the performance of the work. Paint product containers are to be kept sealed at all times except when in use. The Contractor must provide and have available at all times, portable fire extinguishers, suitable for potential hazard(s) involved. He must also obtain from the PBS Buildings Manager, a separate daily permit for any welding or cutting on a day-to-day basis (GSA Form 1755).

CONTRACTOR EMPLOYEES: Each employee of the contractor shall be a citizen of the United States of America, or an alien who has been lawfully admitted for permanent residence as

Replace Elevator Tile on all 11 elevators 4 06/25/18 evidenced by Alien Registration Receipt Card Form I-l2 or, who presents other evidence from the Immigration and Naturalization Service that employment will not affect his immigration status.

EXTRAS: Except as otherwise provided in the contract, no charge for extra work or materials will be allowed unless the same has been ordered in writing by the contracting officer and the price stated in such order.

INDEMNITY: The contractor shall save and keep harmless and indemnify the Government against any and all liability claims, and costs of whatsoever kind and nature of injury to or death of any person or persons and for loss or damage to any property (Government or otherwise) occurring in connection with or in any way incident to or arising out of the occupancy, use, service, operations, or performance of work in connection with this contract, resulting in whole or in part from the negligent acts or omissions of the contractor.

SECURITY PROVISIONS: All contractor and subcontractor employees will be required to furnish information for security clearances and comply with security regulations as imposed by the occupying agencies. The contractor will make it a specific provision of his subcontracts that these regulations be accepted. Security provisions now required are portion to the HSPD-12 program and the contractor may apply suitable clearances, under this program, as may have been met at alternate facilities or agencies.

8.0 Contract Description

This contract is for the replacing 11 Elevator cab flooring at 970 Broad street Newark, NJ. 07102.

Due to the minimal dollar value of the contract, partial payments shall not be allowed, with full payment to be made following satisfactory inspection and receipt of all deliverable items.
