PERFORMANCE WORK STATEMENT (PWS)
HVAC Preventive Maintenance Services
GLAHS-WLA South Campus
1. General. It is the intent of the Government to enter into a single award contract for performance-based PMI services on Government Owned HVAC equipment and Siemens Apogee Building automation System in accordance with the terms and conditions contained herein.
1.A. Background. With the implementation of the Siemens Apogee Building Automation system it has been proven to be effective to have a comprehensive solution to the maintenance of the entire HVAC system because all the elements of the system are integrated. The objectives of this contract are to:
1.A.1. Provide a clean and healthy atmosphere which is compliant with all hospital regulatory agencies such as the Joint Commission, CAP, CARF, OMI.
1.A.2.Maintain continuous operation of the HVAC and building automation system 24/7/365 days.
1.A.3.Maintain efficient operation of equipment.
1.A.4.Control temperatures through-out the building to satisfy facility requirements.
1.A.5.Control Air Pressures relationships and air exchange rates to meet facility requirements.
1.A.6.Ensure operation of HVAC equipment 24/7/365.
1.B. Scope. The contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items to perform HVACR service as defined in this Performance Work
Statement. The Contractor shall perform on-site maintenance, repair, calibration, performance verification and quality assurance to keep the equipment covered by this contract in optimum operating condition. Services are to be proactive in nature and shall include continual updating, maintenance and surveillance of the Siemens Apogee
Building Automation System for optimal performance in controlling the building HVAC systems.
1.C. Specific Tasks. Contractor will conduct a site survey and perform initial inspection of all systems covered under this contract. Contractor will provide written report within the first 120 days of services performed, inspection findings, actions taken, and recommendations for any further actions.
1.C.1. HVACR Systems. The contractor shall provide services for Preventive maintenance of the HVACR systems in the buildings listed below:

1.C.1.A. Preventive maintenance shall be performed in accordance with the terms and conditions contained herein and the manufacturer’s recommended maintenance procedures. It is the sole responsibility of the servicing contractor to schedule and perform all preventive maintenance inspections at the intervals specified, at a time mutually agreeable with the Contracting Officers Representative (COR). The VA Greater Los Angeles
Healthcare System does not assume any responsibility for reminding the servicing contractor that a preventive maintenance inspection is due. A copy of the PM inspection shall be provided to the COR at the completion of each inspection.
1.C.1.B. The Contractor shall perform system testing periodically. The Contractor shall inspect and test all systems to ensure the proper efficiency, reliability, and the safe performance of all equipment. Work shall be in accordance with applicable codes, equipment manufacturer’s recommendations, and the specifications herein.
1.C.1.C. All tests shall be a part of a Master Schedule. A copy of the Master schedule shall be provided to the
COR within the first 30 days of award of the contract. The Contractor shall confirm these test dates to the COR seven (7) days prior to the test. Tests that are disruptive to normal utilization of the building system such as electrical, heating, cooling, etc., shall be performed on weekends or during off hours on workdays at no additional cost to the government. The COR may exercise the option of witnessing these tests.
1.C.1.D. All inspections, tests, cleaning, routine repairing, and other preventive maintenance activities shall be performed in accordance with schedules required by the manufacturer’s recommendations, operating history, and
Facility requirements.
1.C.1.E. Contractor shall furnish all labor, materials, parts, tools, equipment software, firmware, and supervision to clean, inspect, test, adjust, diagnose, and maintain all items and components associated with the Apogee
Automation System (The Building Automation System, BAS) located throughout the VAGLAHS South Campus.
1.C.1.F. Contractor shall perform on site preventive maintenance inspections. Preventive maintenance shall be inclusive of all O.E.M. recommended checklists for all HVAC and Automation related equipment. These tasks include but are not limited to:
1.C.1.F.A. Annual battery replacement.
1.C.1.F.B. Monthly panel memory checks
1.C.1.F.C. Monthly panel programming unresolved code check
1.C.1.F.D. Semi-Annual network diagnostic communication test and report
1.C.1.F.E. Weekly failed points check
1.C.1.F.F. Weekly operator override check
1.C.1.F.G. Weekly server error report check
1.C.1.F.H. Semi-Annual seasonal loop tuning
1.C.1.G. Contractor shall at a minimum perform monthly checks to ensure the automation controls are in calibration, working optimally, and properly reflect any changes in the configuration of the building, system and varying climatic conditions. Checks will be performed more often if needed, as determined by the COR.
1.C.1.H. Analysis and Optimization will be performed on the software, network, automation controls and control loops a minimum of twice per year. A full report of the tests run, deficiencies found, and corrections and adjustments made will be provided to the COR. Optimization will be performed in May and November, or more frequently to assure optimum operation of the Apogee Building Automation System. A report will be provided within 5 working days of the optimization work being completed.
1.C.1.H. The contractor shall furnish and install INSIGHT software updates to maintain or improve present performance within the functional capabilities of this system. INSIGHT software updates shall be installed within
90 days of new version availability. Documentation will be provided for all updates in both hard copy and digital formats.
1.C.1.I. The contractor shall install all firmware updates as they become available and notify the COR of the updates having been made.
1.C.1.J. Software licenses shall be updated on an annual basis during the contract period. The update shall include the latest revision on INSIGHT, which is available for the Apogee Automation System. The contractor shall have updated the software to the latest revision level available from the manufacturer (if not already current).
The contractor shall coordinate the software update with the COR to accommodate any revision GLAHS may wish to make.
1.C.1.K. In the event of hardware failure, the contractor shall reload the graphics database and system file from its current backup copy.
1.C.1.L. All Siemens Apogee Insight workstation software and field panel databases will be backed up a minimum of twice per year to contractor provided resources of storage of the database. The database remains the intellectual property of the VA. The contractor shall back-up the INSIGHT graphics database monthly. In the event of catastrophic failure, the stored backup copy will be provided, and the system reprogrammed and tested within twenty-four hours.
1.C.1.M. VAGLAHS shall provide an IP address for a site to site connection to allow the contractor to connect to the BAS to correct deficiencies. The IP number shall be given to the contractor once the contract is awarded.
1.C.1.N. The contractor shall offer 24 hours of off-site diagnostic services. The contractor, from his facility, must be able to access the system via modem/computer access control to perform system evaluation and operation as part of this contract..
1.C.2. Mechanical Systems.
1.C.2.A. Contractor will perform routine maintenance services during normal business hours. The technicians shall provide inspection and analysis, preventive maintenance, assure continuous operation, repair services and simple modification and improvements.
1.C.2.B. All typical repair and maintenance materials are included in this contract. All materials or parts specified to be replaced during routine services according to manufacturer’s service guidelines shall be included as part of this contract.
1.C.2.C. Contractor shall provide a written plan to replace or upgrade mechanical systems and subsystems to maintain the buildings HVAC systems in an optimal state when economy of repair and efficiencies are no longer reasonable. Reasonable is determined when the cost of repair exceeds 35% of the replacement cost. The plan will include both a simple scope of work and a magnitude of cost. The final decision to implement suggested improvements shall be made by the Contracting officer and the COR.
1.C.2.D. Visually inspect, clean, tighten, adjust, replace worn components annually or more frequently as needed to assure optimum performance.

Painting and surface preparation as required, to prevent corrosion and deterioration of exterior equipment surfaces
Equipment tasking:

-Check operation of damper(s), clean and lubricate damper linkages

Adjust water distribution and maintain nozzles and orifices for even flow
Monitor the VFD and fan controls at least monthly to ensure that all fans are operating in synchronization
1.C.2.G. Basic Building system Mechanical repairs shall include but are not limited to:
1.C.2.G.1. Chilled Water Piping: Insulation repairs of rubber or foil/paper/Plastic covered/ faced fiberglass insulation replacement up to 4” diameter pipe, where repair materials will match those of original. Hard pipe insulation such as Polyisocyanurate with metal or paper jacketing is not included in this contract.
1.C.2.G.2. Field Piping- Building heating system: Contractor is responsible for piping between the last isolation valve and the point of use in the HVAC system, such as between the isolation valve and the terminal re-heat unit, including the electronic control valve, strainer, related parts, and the return line to the isolation valve.
1.C.2.G.3. Electrical: Contractor is responsible for complete PMI and service of the electrical portion of the
HVAC system from the Load Terminals of the disconnect switch controlling the HVAC equipment. The VA is responsible to provide power to the line side of the disconnect switch and the disconnect switch mechanism.
1.C.2.G.4. Duct work: Contractor is responsible for maintaining all duct work in a secure condition with minimal leakage. Damage caused to ductwork over 12” will be addressed on a case by case basis and is outside the scope of this contract. No new or replacement duct will be permitted to have insulation exposed to the airstream. The duct insulation will either be external or fully encapsulated, for example: Astrofoil, double wall duct or similar.
1.C.2.G.5. Contractor is responsible for controlling and monitoring the operating performance of the chillers and related equipment. Any abnormality in operation of this equipment is to be brought to the attention of the COR immediately.
1.C.3. Air Handling Units, Solar Inverters and Refrigeration Equipment. Air Filters: Filter service to be performed according to the type and frequency as described in the attached VA GLA Air Filter schedule for filter specification and frequency of service. Refrigeration Equipment: Includes all refrigerators, freezers, Nutrition and
Food Service equipment, reach in coolers, cold stations.
1.C.3.A. Operation Inspection:
• Lock out and tag out equipment as required
• Check all electrical connections. Tighten as needed
• Check all motor starter contactor surfaces for wear
• Lubricate air handling unit motor bearings and fan bearings if bearings are not sealed type.
• Check belt tension and sheave alignment. Adjust as required
• Check and vacuum clean evaporator coils for excessive dirt and debris
• Check and vacuum clean condenser coils
• Check and clean condensate drains and pans, ensure proper drainage
• Inspect air handling unit fan assembly
• •Check structural integrity of unit
• Check all mounting hardware
• Check Delta T’s
• Check Delta P’s
• Check operating controls for proper operation
1.C.3.B. Annual Inspection:
• Lock out and tag out equipment as required
• Check al electrical connections. Tighten as needed
• Check all motor starter contactor surfaces for wear
• Clean electrical controls enclosures
• Lubricate air handling unit motor bearings and fan bearings
• Check belt tension and sheave alignment. Change belt as required
• Clean evaporator coils as required
• Check and clean condensate drains and pans
• Inspect air handling unit fan assembly
• Check structural integrity of unit
• Check all mounting hardware
• Check operation of crankcase heater
• Meg-Ohm motor from starter and record
• Contractor shall replace the Motor contactor/starter with new OEM every time a motor is replaced, including refrigeration compressors, condenser fan motors, AHU motors etc.
1.C.4. Chiller Maintenance. Contract shall provide preventive maintenance service and inspection to chillers in bldg. #213, 215 and 501 VAGLAHS 11301 Wilshire Blvd. Los Angeles, CA 90073. Services will be performed during normal working hours of 6AM-4PM

1.C.5. Air Compressor Maintenance: Full services are to be performed, includes all parts, labor and equipment:
Lubricant and oil filter changes, air filter changes, oil separator changes, filter mat changes, electrical system checks, filter cartridge replacement, safety control system check, differential pressure check, scavenger line filter check and change, V belt tension check/replace, minimum pressure check valve, inspect and clean coolers, switch over driers, and precoolers semiannually and any other manufacturer recommended preventive maintenance services. All services should be performed at quarterly, semiannual and annual intervals in accordance with manufacturer’s recommendations. Equipment is in buildings B295 and B501. Each building is a restricted access building and advanced notification is required for entry. Hearing protection is required in each building.

1.C.5.A. Operations and Air Flow Testing, Monitoring, and Reporting: Contractor shall perform regular testing, trend data and prepare reports on a regular basis including but not limited to monitoring spaces such as Negative pressure rooms, Operating rooms, procedure rooms and any space of interest as requested by the facility.
Pressurized (positive and negative) rooms shall be smoke tested for directional air flow and documented in the first week of February, March, May, June, August, September, November, December. Incorrect air flow will be reported to the COR and corrected as soon as possible by the contractor. A report of the results and any corrections required shall be turned in to the COR by the 15th of each month listed, or the next business day following the 15th.
1.C.5.B. Quarterly Space Differential Air Pressure Testing. Contractor shall provide certified reports for air flow and differential pressure according to the attached schedule. Spaces for this testing schedule may change according to the facility requirements. The facility shall be provided 3 bound copies of each report. Differential
Air Flow reports shall be provided within 7 calendar days after completion of testing. Testing shall be performed according to the standards published by the Associated Air Balance Council.
1.C.5.C. Annual Crucial Area Validation: The Crucial Area Validation reports shall contain detailed diagrams including the values of each supply and return/exhaust grill, a drawing of the area being tested with the differential air pressure value and a directional arrow indicating air flow. Differential pressure for every door in the subject space shall be tested and documented. The facility shall be provided 3 bound copies of the reports of areas tested within 7 calendar days of completion of the testing. Testing shall be performed according to the standards published by the Associated Air Balance Council.
1.D. Contractor Qualifications:
1.D.1. Apogee Building Automation System Field Service personnel must be factory trained and certified with a minimum of three years’ experience servicing and maintaining Apogee systems.
1.D.2. Contractor must be able to provide all manufacturer software updates, flashes and other software.
1.D.3. The contractor shall ensure that all employees performing maintenance and repair services are fully qualified, competent Field Service Engineers (FSE) who have been trained to perform work on the specific equipment in accordance with the OEM’s PMI/ repair procedures. “Fully qualified” is defined as factory-trained or equivalent and experienced to work on the equipment assigned.
1.D.4. Contractor personnel performing work under this contract must have a favorable basic security check at time of the proposal submission and must maintain the level of security required for the life of the contract.
1.D.5. Site FSE’s will be fully versed in the operation and maintenance of the Siemens Apogee Building automation system as well as all common industrial HVAC equipment. All site engineers will function at the journeyman level and must be graduates of an accredited trade school and / or a certified apprenticeship program.
1.D.5. Contractor shall have provided similar services to other customers of similar size and scope for at least 5 years.

1.E.1. Documentation and Quality Assurance: The contractor shall document each on-line and on-site service call and furnish the VA with a copy showing time, date, and a brief description of activity. Contractor shall meet with the VA monthly to evaluate system performance and VA satisfaction with the quality of service that is being provided under the contract.
1.E.2. Quality Control: The contractor shall develop and maintain an effective quality control program to ensure services are performed in accordance with this PWS. The contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services, and include a plan when staff is out due to illness/vacation.
1.F. Equipment: The Contractor shall maintain Government provided computers, servers and work-stations for the Apogee system. All Computers and system components shall meet VA requirements.
1.G. General Requirements
1.G.1. Physical Security. The contractor shall be responsible for safeguarding all government equipment, information and property provided for contractor use. At the close of each work period, government facilities, equipment, and materials shall be secured.
1.G.2. Key Control. The Contractor shall establish and implement methods of making sure all keys/key cards issued to the Contractor by the Government are not lost or misplaced and are not used by unauthorized persons.
NOTE: All references to keys include key cards. No keys issued to the Contractor by the Government shall be duplicated. The Contractor shall develop procedures covering key control that shall be included in the Quality
Control Plan. Such procedures shall include turn-in of any issued keys by personnel who no longer require access to locked areas. The Contractor shall immediately report any occurrences of lost or duplicate keys/key cards to the Contracting Officer.
1.G.2.A. In the event keys, other than master keys, are lost or duplicated, the Government, at its option, may replace the affected lock or locks or perform re-keying. When the replacement of locks or re-keying is performed by the Government, the total cost of re-keying or the replacement of the lock or locks shall be deducted from the monthly payment due the Contractor. In the event a master key is lost or duplicated, all locks and keys for that system shall be replaced by the Government and the total cost deducted from the monthly payment due the
Contractor.
1.G.2.B. The Contractor shall prohibit the use of Government issued keys/key cards by any persons other than the Contractor’s employees. The Contractor shall prohibit the opening of locked areas by Contractor employees to permit entrance of persons other than Contractor employees engaged in the performance of assigned work in those areas, or personnel authorized entrance by the Contracting Officer.
1.G.3. Lock Combinations: The Contractor shall establish and implement methods of ensuring that all lock combinations are not revealed to unauthorized persons. The Contractor shall ensure that lock combinations are changed when personnel having access to the combinations no longer have a need to know such combinations.
These procedures shall be included in the Contractor’s Quality Control Plan.
1.H. Post Award Conference/Periodic Progress Meetings: The Contractor agrees to attend any post award conference(s) convened by the contracting officer or COR in accordance with Federal Acquisition Regulation
Subpart 42.5. The contracting officer, Contracting Officers Representative (COR), and other Government personnel, as appropriate, may meet periodically with the contractor to review the contractor's performance. At these meetings the contracting officer will apprise the contractor of how the government views the contractor's performance and the contractor will apprise the Government of problems, if any, being experienced. Appropriate action shall be taken to resolve outstanding issues. These meetings shall be at no additional cost to the government.
1.J. Contractor Personnel: All contract personnel attending meetings, answering Government telephones, and working in other situations where their contractor status is not obvious to third parties are required to identify themselves as such to avoid creating an impression in the minds of members of the public that they are
Government officials. They must also ensure that all documents or reports produced by contractors are suitably marked as contractor products or that contractor participation is appropriately disclosed. Contractor employees will wear government furnished ID badges at all times while on the facility. Contractor employee will wear uniforms with company name/logo on the shirt
1.J.1. The follow personnel are considered key personnel by the government: Contractor Manager, Field Service
Engineer Lead Technician, Field Service Engineers. The contractor shall provide a contract manager who shall be responsible for the performance of the work. The name of this person and an alternate who shall act for the contractor when the manager is absent shall be designated in writing to the contracting officer and COR. The contract manager or alternate shall have full authority to act for the contractor on all contract matters relating to daily operation of this contract. The contract manager or alternate shall be available between 7am to 4:30pm, Monday thru Friday except Federal holidays or when the government facility is closed for administrative reasons.
Qualifications for all key personnel are listed below:
1.J.1.A. Contract Manager: A thorough understanding of the Joint Commission and Environment of Care requirements in addition to a thorough knowledge of HVACR maintenance and repairs.
1.J.1.B. Lead Service Engineer: Able to ensure all scheduled maintenance is performed and documented on time, reports are provided to the COR on time.
1.K. Government Furnished Items and Services. All items furnished to the contractor by the government will be used only in direct support of this contract.
1.K.1. The Government will provide telephone, office space, a parts cage/area for storage of necessary parts and supplies, electricity and air conditioning, keys for access, and internet access. Parking is provided in the
Employee parking lot. Except for delivery or pickup of tools or materials, parking in the area immediately south of Building 500 and Building 501 is strictly prohibited for the contractor and their sub-contractors
1.K.2. Utilities: All utilities in the facility will be available for the contractor’s use in performance of tasks outlined in this PWS The Contractor shall instruct employees in utilities conservation practices. The contractor shall be responsible for operating under conditions that preclude the waste of utilities, which include turning off the water faucets or valves after using the required amount to accomplish cleaning vehicles and equipment.
1.L. Smoking is strictly prohibited on the grounds of any VHA facility. Per VHA Directive 1085 dated March 5, 2019, it is VHA policy that all VHA health care facilities, including hospitals, community clinics, administrative offices and Vet Centers, will be smoke-free for patients, visitors, contractors, volunteers, and vendors effective
October 1, 2019. There will no longer be designated smoking areas.
1.M. Normal Working Hours. Normal working hours are 6am-4pm Monday through Friday. Service hours on the 1st Wednesday of each month shall be from 5am to 4pm to provide support for monthly emergency generator testing at building 501/500. Emergency on-call service shall be provided twenty-four hours per day, seven days per week including holidays.
1.M.1. Any overtime or holiday pay that may be entitled to the Contractor’s employees shall be the sole responsibility of the Contractor and shall not be billed to nor reimbursed by the Government.
1.M.2. The Contractor is not required to provide service on the following U.S. Government holidays nor shall the
Contractor be paid for these days:

1.M.4. If the holiday falls on a Sunday, the following Monday will be observed as a National holiday. When a holiday falls on a Saturday, the preceding Friday is observed as a National holiday by U.S. Government Agencies, and any day specifically declared by the President of the United States of America.
1.N. Insurance Coverage.
1.N.1. The Contractor agrees to procure and maintain, while the contract is in effect, Workers Compensation and
Employee’s Public Liability Insurance in accordance with Federal and State of California and/or Nevada laws. The
Contractor shall be responsible for all damage to property, which may be done by him, or any employee engaged in the performance of this contract.
1.N.1. The Government shall be held harmless against any or all loss, cost, damage, claim expense or liability whatsoever, because of accident or injury to persons or property of others occurring in the performance of this contract.
1.N.1. Before commencing work under this contract, the Contracting Officer shall require the Contractor to furnish certification from his/her insurance company indicating that the coverage specified by FAR 52.228-5 and per FAR
Subpart 28.307-2 has been obtained and that it may not be changed or canceled without guaranteed thirty (30) day notice to the Contracting Officer.
1.N.2. Contractor is required to provide copies of proof of Workers Compensation and Employee Public Liability
Insurance within fifteen (15) calendar days after notification of contract award.
1.O. Contractor is responsible for repairing damages incurred during performance of this scope of work to include removal/installation or moving materials and tools in and out of the work area.
1.P. Changes. The awarded Contractor is advised that only the Contracting Officer acting within the scope of the contract has the authority to make changes which affect the contract in terms of quality, quantity, price or delivery. In the event the
Contractor effects any such change at the direction of any person other than the Contracting Officer, the change shall be considered to have been made without authority and no adjustment shall be made in the contract price to cover any increase in costs incurred as a result thereof.
