Security
Security requirements as outlined in the following “Security Instruction” titled “Security Instruction number 1” shall be strictly adhered to.
Security Instruction number one
DATE: November 10, 2014
TO: All Contracting and Vendor Personnel
| FROM: | Captain Harry Bolton | |
| Company Security Officer, California Maritime Academy |
This is to inform all persons that may visit, conduct work, provide technical or sales services or have other official business aboard or in any way need access to be alongside or aboard the training ship GOLDEN BEAR , that International and Coast Guard regulations requires stringent access control to all U.S. vessels by all persons in accordance with those regulations. This includes the training ship, whether she is in a foreign or in any domestic port. Additionally, the California Maritime Academy may invoke additional requirements to gain access to the campus if it is determined by the U.S. Coast Guard that the CMA is a “port facility” under the new regulations.
These security requirements will require that all visitors, contracting personnel, vendors, sales-persons and any technical consultants comply with the requirements outlined herein before being allowed to board or approach the training ship. Failure to comply in whole or in part of these regulations may lead to the expulsion of personnel from the vessel until full compliance is achieved. Any and all additional costs resulting from this eventuality, including delays in work, denial of access for subcontractors or any other interruption of contracted services, is to be completely borne by the contracting or vendor companies. Only pre-authorized persons will be allowed onboard. This includes visitors.
All contracting, vendor, or visiting personnel, must at all times, comply with any and all requirements and rules promulgated by the ship’s Security Officer, which in the case of the GOLDEN BEAR, is the vessel’s Chief Officer. These requirements are those made by the training ship and the California Maritime Academy. Any additional security procedures, rules or requirements promulgated by MARAD will be in addition to these requirements.
Any or all of the following rules and procedures must be complied with depending on the official national maritime threat level:
1. Campus access and parking may be controlled by the school’s Police Service department.
2. All personnel wanting unescorted access on the TSGB must possess and produce a valid TWIC card. All personnel wanting entry into a “Restricted Area” must be authorized by the Captain, Chief Mate, Chief Engineer or COTR. The primary contractor shall be responsible for issuing all of their direct employees and as well as their designated sub-contractor’s employee’s identity verification and authorized company photo ID cards. Non-photo company identification cards will not be accepted. These identification cards are required to be worn in a visible location. Subcontractors and employees without company ID shall be required to obtain from the CMA/GOLDEN BEAR a daily visitor pass in the form of a generic ID card that authorizes the holder to be onboard. The daily visitor cards will be required to be worn in a conspicuous manner so that each person is readily identifiable by ship’s officers and security personnel. The cards will be used to log workers and vendors on and off of the vessel using the ship’s logging system. These Golden Bear Visitor cards must be issued and surrendered at the beginning and end of each day for logging in and out. Failure to surrender the visitor card at the end of the day shall be treated as a security violation.
3. Outside company employees newer than six months and all temporary visitors not known to the Academy are subject to security background inquires by the CMA consistent with current laws before they will be permitted to board the vessel.
4. Materials or parts delivered directly to the campus/ship are subject to search and screening by the ship’s Security Officer. Deliveries of equipment, parts and materials must be declared to the ship Security Officer at least 24-hours in advance of the delivery and must be scheduled for screening before being loaded aboard.
5. Materials and supplies shipped in advance or drop shipped to CMA’s facility must pass through the campus warehouse for screening. This screening process shall be conducted by the ship’s security officer (Chief Mate) and an authorized person designated by the contractor. This inspection and shipping documentation screening must be accomplished before any materials can be loaded onto the vessel.
1. Contractor vehicles will not be allowed onto the pier without first being screened by the vessel’s security officer. This may be done daily. The same applies for contractor tool boxes or other equipment when first loaded aboard. Ship security personnel shall work closely with contractor’s management personnel to achieve workable but effective security concerns.
7. Depending on the national security threat level, all persons needing access to the vessel may be subject to random personal and bag checks. If the vessel is at a heightened security level, ALL persons may be subject to these searches or screenings.
8. The contractor and shipboard management supervisors must attend a one-hour security orientation on or before the first day of a contract performance period. This orientation will be conducted onboard the vessel by the Ship’s Security Officer. It must be emphasized that contracting personnel or their sub-contractors who have not been authorized to be onboard the vessel will be denied access to the vessel.
9. In conjunction with the orientation, a short security briefing will be conducted before the contract begins. This briefing shall be attended by:
· MARAD COR
· Company Security Officer
· Ship Security Officer
· Director of Campus Public Safety
· Warehouse Supervisor
It must be emphasized to contractors and visitors that these requirements are mandatory, and that any disruption or recalcitrance demonstrated by visiting or contracting persons onboard concerning these requirements will be denied access to the vessel until full cooperation is achieved. If necessary, persons who are being uncooperative or belligerent will be removed from the vessel permanently and reported to national authorities. It is the desire of the CMA and the training ship that all work is accomplished in the spirit of security awareness and cooperation and every accommodation that can be given while maintaining compliance with the regulations is encouraged.
Regards, Captain Harry Bolton CMA Security Officer

Inspection Report All Cal Equipment Services, Inc.- Scott Webster
Subject: Safety Inspection of Trident 10,000lb capacity port and starboard stores cranes
M/N: TMP20-10-23T38
Dear Sirs, The Scope of Work Accomplishments: Port Trident 10,000 lb capacity unit:
The unit was inspected visually for OSHA standards compliance.
Deficiency: Boom Heel Pin- the pin at the articulating portion of the base boom is seized into the boom. No presence of lubrication presented. The pin retaining washers are corroded beyond standards for simple washers. The pin protrudes out of the left side of the boom mounting ear on the operator’s left hand side. This condition requires attention immediately.
Deficiency: Boom Heel pin mounting on the upper turret section- the mounting ear on the operators left side of the turret has excessive TIR. The movement within the mounting ear is visual. The mounting ear is devoid of lubrication. This condition requires attention immediately, continued use will degrade the mounting ears as the crane continues to be used in normal operations.
Deficiency: Hydraulic extensible boom- large corroded debris accumulating at the turret below the rear exit of the boom. Visual inspection of the interior of the boom presents further excessive steel pack rust and de-lamination at the interior visible portion of the boom steel structure. Further inspection would be required to confirm the condition of the box steel boom sections. Condition would be considered serious, and at the minimum would need an internal inspection for structural integrity.
Deficiency: Hoisting winch supporting structure- The operators left side hoist side support plate presents heavy corrosion and minor pack rust. Coating removal necessary, after removal, inspection to determine condition of steel structure. If condition left unattended condition will degrade, creating a possible unsafe condition.
Deficiency: Crane load chart- Illegible; unit does have a variable load chart. Standards require a clearly visible load chart to be present to the operator. Unit has a Hydraulic control to further effect chart.
Deficiency: Crane controls- controls present to the operator not clearly indicated. Standards require all controls to be clearly marked when presented to the operator.
Deficiency: Crane controls- active oil leak at operator’s console. Oil leak present at operators left side. This condition presents an unnecessary operation condition that may at some point adversely affect the operator. If condition left unattended it may degrade to affect crane performance and safety.
Deficiency: Crane control- system pressure gauge, is not intact. The needle is indicating pressure when in fact no pressure is present. Gauge face has become foggy and difficult to read. To the above mentioned oil leak; the gauge has a valve to allow operation. The valve has the actual leak, and has been “duct” taped to prevent the oil leak.
Deficiency: Crane Boom Tip- upper boom tip sheave has excessive TIR at the cross shaft. The excessive TIR is most notably transverse to the axle. Indicating that the bushing / bearing has excessive wear.
Deficiency: Load block and system- Excessive play in load hook swivel pin. Load hook safety latch operates incorrectly. Standards require regular inspections, Manufacturers maximum wear tolerances to be adhered to, and deficient devices removed from service Load hook safety latch is not held within the interior of the hook throat. This presentation would indicate a condition that may require a new safety latch to become compliant.
Deficiency: Boom Hoist upper connection point- the rod end of the boom hoist cylinder is retained by a fixing pin. The boom structure has additional backing half shells. These half shells have excessive pack rust development. Recommend, measures be taken to reduce the effect of the corrosion and possible re-coating of the components.
Deficiency: maintenance- attempts were made to lubricate the existing grease points. Many points will not receive lubrication. The unit presents no recent maintenance
Starboard Trident 10,000 lb capacity unit:
The unit was inspected visually for OSHA standards compliance.
Deficiency: Boom Heel Pin- the pin at the articulating portion of the base boom. No presence of lubrication presented. The pin retaining washers are corroded beyond standards for simple washers. This condition as presented may require further investigation to determine why the pin lubrication points will not accept grease.
March 17, 2015 Deficiency: The unit turret is fixed to the rotary bearing to allow for slew action to the right and left. The upper structure and boom assembly rely on the structural connection fasteners for load capacity and safety. The fixing fasteners that are exposed to the starboard side of the ship (when unit is in storage) present exposure to the environment and exhibit corrosion. The level of corrosion presented has caused the fasteners to delaminate. This effect may drastically affect the stability of the crane when in use. It would be recommended to renew all the rotary bearing fasteners.
Deficiency: Boom base and boom inner slide pad- the forward lower assembly consist of two slide pads and a retainer bracket. This unit has one missing slide pad and the retainer bracket If this condition is left unattended condition will degrade creating a possible unsafe condition.
Deficiency: Crane load chart- Illegible; unit does have a variable load chart. Standards require a clearly visible load chart to be presented to the operator. Unit has a Hydraulic control to further effect chart.
Deficiency: Crane controls- controls presented to the operator are not clearly indicated. Standards require all controls to be clearly marked when present to the operator.
Deficiency: Crane controls- This unit has two hydraulic controls on the operator panel. One, for the heated oil circulation; and the other for hoist capacity/speed. The operator’s panel has been upgraded to a stainless steel unit. The two hydraulic controls were not installed into the new panel, and hang down below the panel. Thus, these controls are not able to be operated properly. This condition presents an unnecessary operation condition that may at some point adversely affect the operator. If condition left unattended it may degrade to affect crane performance and safety.
Deficiency: Crane control- operators panel, the operators control for the on/off of the crane has exposed conductors from a failed cord grip at the control station. The ‘on’ light is also inoperative. Standards and the environment required single conductors to be protected by electrical conduit or similar protection method, this condition could present an unsafe electrical hazard to the operator.
Deficiency: Crane Boom Tip- lower boom tip sheave has excessive TIR at the cross shaft. The excessive TIR is most notably transverse to the axle. Indicating that the bushing / bearing has excessive wear. This may create further hazards in the case of a bearing/bushing failure.7, 15
Deficiency: Hoist winch- the hoist winch has developed an oil leak at the support plate on the hydraulic motor drive side of the unit. This can only further degrade to a larger discharge of hydraulic fluid. Hoisting standards require all hydraulic components to be maintained and no oil leaks in the system.
Deficiency: Boom Hoist upper connection point- the rod end of the boom hoist cylinder is retained by a fixing pin. The boom structure has additional backing half shells. These half shells have excessive pack rust development. Recommend, measure be taken to reduce the effect of the corrosion and possible re-coating of the components.
Deficiency: maintenance- attempts were made to lubricate the existing grease points. Many points will not receive lubrication. The unit presents no recent maintenance.
General Information:
The inspection conducted was during ships stores operation. The starboard crane was in use by ship staff. Area’s not visually inspected include the base stanchion of the crane units, the interior of the inner boom section, and hydraulic swivel below the turret. Both units were operated after ship stores operations had completed. The wire rope has been “slushed” adequately. However the coating precluded the inspection of the wire rope in its entirety.
It should be noted that the lower base section of the boom has received welding repair previously. Generally, the crane units have not received periodic maintenance lubrication and are beginning to suffer corrosion in critical areas of the structure. With attention to the fixing hardware, and steel structural components. Attention to the functions of the crane control system may require further retraining of the ship operation staff.
At this time a fully function “anti two block system” is installed on the unit. this system has lock out incorporated to allow only limited operation of the when the ATB is activated. The base components have the ability to add additional devices to provide a system that would disallow any other misuse of the units (i.e. overload control). Also, actual load measurement could be included
