PAST PERFORMANCE EVALUATION SURVEY

FORM INSTRUCTIONS:

OFFERORS:

Section A (Part I and Part II) shall be completed by the Offeror (the company requesting the reference) prior to issuing the surveys in accordance with Volume II, Section L.6 of the SIR.

INSTRUCTIONS TO CUSTOMER:

The purpose of this survey is to obtain past performance information with respect to the Offeror identified in Part I, Section A below.

The Federal Aviation Administration (FAA) is conducting a competitive procurement for sustainment, management, and further development of the Agency’s Traffic Flow Management System under SIR number DTFAWA-16-R-00010 known as TFM2. The TFM2 procurement is a major FAA acquisition and your input into this survey is greatly appreciated.

You have been identified as a customer for work performed on the contract cited in Part I, Section B below, and your candid response will assist in the evaluation process and will be kept confidential.

Please review the information provided by the Offeror in Part I, and identify and correct any information that is not accurate. Please complete Part II of this survey as thoroughly as possible and submit directly to the FAA Contracting Officer Sharonda.Holmes@faa.gov no later than 3:00pm, Eastern Time on the 42nd calendar day after the day of SIR issuance.

The information provided on this survey is considered confidential and will not be released to the Offeror.

The subject line in the submission e-mail must clearly indicate: TFM2 Past Performance Survey Submission for (Offeror name cited in Part I, Section A) and the message shall originate from the customer’s e-mail system.
