Screening Information Request/Request for Offeror DTFAAC‐15‐R‐01685 Amendment 0004
The purpose of this amendment is to make changes to the solicitation, incorporate a revised Performance Work Statement, and extend the closing date.
FIRST: The Performance Work Statement (Solicitation Attachment 1) is hereby replaced with the attached Performance Work Statement (Rev 1) dated 17 Jun 2015.
SECOND: Paragraph 3.2.4 (Factor Three – Key Personnel) of Provision L.1 is hereby replaced with the following:
3.2.4 Factor Three: Staffing Plan
The offeror must submit an organizational chart, along with resumes and certifications of all key personnel, which clearly demonstrate their capability to adequately execute all tasks outlined in the Performance Work Statement. Resume information to be provided shall be limited to no more than one (1) page per person and shall include the following information as a minimum:
Name and Title Project Assignments Name of Firms with which Associated Years of Experience with Offeror and with Other Vendors Training (including year completed) and Specialization (If Applicable) Any Associations to which Individual Maintains an Active Membership (Include
Year First Registered) Other Experience and Qualifications Relevant to Same/Similar Work Required
Under the Performance Work Statement
Particular expertise required by the offeror and/or subcontractor(s) includes knowledge of the
National Environmental Policy Act (NEPA) Endangered Species Act (ESA) National Historic Preservation Act (NHPA) Coastal Zone Management Act (CZMA) Clean Air Act (CAA) Clean Water Act (CWA)
THIRD: Paragraph 2.1.1 (Evaluation Factors used to evaluate each proposal) of Provision M.1 is hereby replaced with the following:
2.1.1 Evaluation Factors used to evaluate each proposal
Based on an integrated assessment of the evaluation factors described below, award will be made to the offeror proposing the lowest Total Evaluated Price meeting acceptability standards for all non-cost factors.
Factor One (1): Offeror Experience Factor Two (2): Past Performance Factor Three (3): Staffing Plan Factor Four (4): Understanding the Requirements Factor Five (5): Cost/Price

2.4 Factor Three – Staffing Plan
The Government will assess the organizational chart, resumes, certifications, and other documentation (submitted in accordance with L.1, Instructions to Offerors, Section 3.2.4) to
Screening Information Request/Request for Offeror DTFAAC‐15‐R‐01685 Amendment 0004 evaluate the Offeror’s proposed staffing. The documentation must clearly demonstrate the offeror staffing’s capability to adequately execute all tasks outlined in the Performance Work Statement.
Acceptable: The standard is met when the offeror provides an organizational chart, resumes, and certifications for all key personnel which demonstrate the following minimum requirements:
Resumes include the following information:
o Name and Title o Project Assignments o Name of Firms with which Associated o Years of Experience with Offeror and with Other Vendors o Training (including year completed) and Specialization (If
Applicable) o Any Associations to which Individual Maintains an Active
Membership (Include Year First Registered) o Other Experience and Qualifications Relevant to Same/Similar
Work Required Under the Performance Work Statement
Offeror demonstrated sufficient knowledge (including subcontractors, if applicable) of the following:
o National Environmental Policy Act (NEPA) o Endangered Species Act (ESA) o National Historic Preservation Act (NHPA) o Coastal Zone Management Act (CZMA) o Clean Air Act (CAA) o Clean Water Act (CWA)
Unacceptable: The Offeror did not submit the required documentation (organizational chart and resumes) or the information submitted did not meet the minimum requirements of this Factor.
FIFTH: Clause G.3 (Warranty – Commercial Products and Services (Jun 2006) CLA.4529) is hereby deleted and replaced with the following:
G.3 Professional Standard of Care
The contractor shall perform its services in accordance with the generally accepted practices of professionals performing similar services in the same geographic region at the time the services are performed for the Government. In the event the services are not in conformance with the generally accepted standard of care, and the Government promptly notifies the Contractor within twelve (12) months of the completion of the services of such non-conformance, the Contractor agrees to re-perform the original services to correct the non-conformance.
SIXTH: AMS Clause 3.8.2-17, Key Personnel and Facilities (May 1997), located in Section H of the solicitation is hereby deleted.
SEVENTH: The closing date for receipt of offers is hereby changed as follows:
From: 22 Jun 2015 at 5:00 pm (CDT) To: 26 Jun 2015 at 5:00 pm (CDT)
EIGHTH: All other terms and conditions remain unchanged.
