SCOPE OF SERVICES
ROUTINE MAINTENANCE SERVICES FOR SOUTH DADE MAINTENANCE
1.0 DESCRIPTION OF WORK
The purpose of this contract is to provide staff to perform routine maintenance services on non-interstate roadways for the Florida Department of Transportation (FDOT) District Six, located at: South Dade Maintenance, 14655 SW 122 Avenue, Miami, Fl 33186. The field crew(s) will report to the South Dade Maintenance Facility. Routine Maintenance Services will be performed in Miami Dade County.
The scope of work will include furnishing all labor necessary to perform the work.
The awarded Contractor will be tasked with hiring, training, and employing qualified, skilled personnel that will provide services described within the Proposed Maintenance Activities Section 5.0. A Field Crew will consist of one (1) Supervisor and four (4) Maintenance Workers, as described in the Key Personnel Requirement Section 6.0. The supervisor of each crew must be able to speak and read English proficiently.
A Crew Cab will be needed each workday and will be incidental to each Field Crew.
The Crew Cab will transport the Field Crew to the Maintenance Facility and to the work site. The Crew Cab must also be capable of hauling a minimum of (8) eight cubic yards of debris and pulling a trailer provided by FDOT with up to a 10,000 lb.
GVRW. Trailer specifications will be specified by the Project Manager.
Routine maintenance services may include, but are not limited to, maintenance and repair activities associated with roadways (to include off-system associated infrastructure), drainage infrastructure, roadside vegetation and aesthetics, and traffic services. All traffic control installations associated in connection with performing a maintenance activity are included as part of the maintenance activity itself. Travel from the Maintenance Facility to the work site is included in the work activity. The Vendor will receive work assignments from the Contract Manager.
Vendor’s staff is expected to work under minimal supervision from the Department or delegate. If the Vendor expects to complete the work prior to the end of the anticipated time of completion, coordinate with the Contract Manager for additional work assignments.
2.0 POLICIES, MANUALS PROCEDURES, AND GUIDELINES
All services shall be performed in accordance with the established standard procedures and practices of the Department (see list below). The Vendor shall comply with all current versions as they are updated throughout the life of the

3.0 SAFETY PERSONAL PROTECTIVE EQUIPMENT AND TRAINING
The Vendor is to provide a safe working environment for the field staff ensuring that each worker has access to Vendor’s personal protective equipment (PPE) compliant
DOT-ITB-26-6192EG Exhibit “A” Scope of Services Page 4 of 10 to Occupational Safety and Health Administration (OSHA) standards and appropriate for task being performed.
PPE shall include, but are not limited to:
Safety vests conforming to American National Standards Institute (ANSI) Safety glasses Ear protection Safety/steel-toed boots Gloves
The Vendor shall provide training to staff, including but not limited to:
Work Zone Traffic Control Certification Intermediate Method of Traffic (MOT)/Advanced MOT
Flagger training by a national recognized organization Personal safety using equipment. This will include specific training on motorized equipment such as pole saws, chainsaws, woodchippers, and power tools.
FDOT online Illicit Discharge Detection and Elimination Training.

4.0 RESPONSIBILITIES
FDOT shall furnish the Vendor with all materials and equipment necessary to perform assigned work. When a specific piece of equipment is required, FDOT will furnish equipment to perform the required work. FDOT will require the Vendor to operate tools/equipment such as concrete mixers, woodchippers, weed eaters, curb edger’s, chainsaws, and other equipment when the operation of such equipment is a routine part of the assigned activity.
The Vendor shall accept responsibility for the use of any tools, equipment, supplies and materials that are furnished by FDOT. Operating costs on Department owned equipment, such as fuel, maintenance, and scheduled repairs shall be the responsibility of FDOT. If FDOT property is damaged directly or indirectly by the Vendor, its agents’, and/or any employee negligence, act of omission, the Vendor shall replace or repair FDOT’s property to its previous condition. In the event of any accident or damage to the equipment occurring during the use, the contractor must adhere to the Departments protocols. The Vendor must promptly submit an incident report detailing the accident or incident to our safety officer and the Roadside supervisor.
The Vendor should act to prevent or minimize damage, injury, or loss in an emergency affecting the safety of persons or property. The vendor shall promptly notify FDOT’s Contract Manager, which notice may be oral followed by written confirmation, within 24 hours of the occurrence of such an emergency. Describe in
DOT-ITB-26-6192EG Exhibit “A” Scope of Services Page 5 of 10 full what occurred and provide the names of those involved with their contact phone numbers, and extent of injury and damage. Provide photos of damage to include time, date, and location.
5.0 PROPOSED MAINTENANCE ACTIVITIES
The activities listed below are proposed work activities that the Vendor may be assigned to perform and complete.

– Manual
Repair of roadway pavement depressions, edge raveling, potholes, and leveling of pavement irregularities with hot or cold plant mix material by hand labor method. Other areas of asphalt repairs which are to be reported to this standard are as follows: Paved shoulders, paved turnouts, edge widening, roadway ditches, under guardrail, constructing or maintaining drainage flumes, etc.

Repairing Non-paved Shoulders, Front Slopes, and Roadside Ditches - Manual
Repairing non-paved shoulders by adding suitable material, or by lowering high areas to include minor work on slopes, ditches, and turnouts. This activity should only be performed in situations requiring attention which are not practical to be corrected by mechanical means. Includes small areas matched by hand. This activity includes hauling of necessary/ unnecessary materials.

Placing sod in areas along the roadside associated with reworking non-paved shoulders, slopes, ditches, median islands, utility strips and repairing washouts. Sod should be used in areas where seeding, fertilizing, and mulching operations have failed to provide desirable turf. This activity would also include the cutting of sod.

Drainage Structures drains, french drains, manholes, side drains, cross drains, inlets, piped outfalls, box culverts and other miscellaneous drain structures. Not to include bridge drains.

Concrete repair on items such as catch basins, median inlets, headwalls, curb inlets, pedestrian underpasses, seawalls, retention walls, barrier walls, and box culverts (under 20-foot span), curb and gutters, paved ditches, paved slopes, flumes or spillways, and rip rap.

Concrete Sidewalk Repair
Repair or replacement of existing sections of concrete sidewalk. This includes the repair or replacement of truncated domes and concrete driveways.

Mowing the roadside with small hand or riding mowers having a cutting width of 40 inches or less.

Weed Control - Manual
Brush, weed, and grass cutting (4 inches in diameter or less) performed with hand tools, including weed eaters.

The trimming of the height and sides of trees and removal of undesirable trees over 4 inches in diameter. Work includes the chipping and/or removal of all debris from work site.

This activity is for the installation of traffic control devices for non- maintenance activities for units such as laboratories, traffic signals, construction, local government approved requests, utilities, railroads, etc. Placing and/or repairing manning barricades due to road closure.
This activity also includes traffic control for traffic incidents, fires, etc. after the initial response.

Charges to this activity represent work that is not routine maintenance or included under another activity.

6.0 KEY PERSONNEL REQUIREMENT SECTION
The Vendor shall train and hire employees to provide routine maintenance services to the Department. The following are job descriptions and qualifications for the staffing to be provided by the Vendor under this contract.
6.1 GENERAL REQUIREMENTS FOR ALL STAFF
a. All employees who operate vehicles within the State Right-of-Way must maintain, at a minimum, a valid and current State of Florida issued Driver’s License, Class “E” and always maintain the license on their person while performing work.
b. All personnel must have lawful status to work in the United States.
c. All personnel must not have any outstanding warrants.
6.2 FIELD CREWS
The Vendor shall provide staffing of two field crews to perform routine maintenance services. In the event, a third field crew should be needed, a 14-calendar day notice will be given. Each crew will consist of one supervisor and four maintenance workers.
6.2.1 SUPERVISOR RESPONSIBILITIES
a. Responsible for planning and directing day-to-day activities of a field crew as directed by the FDOT and defined in the scope of services.
b. Transport field crew to and from the work site each day.
c. Responsible for overseeing operation of all equipment supplied by the FDOT.
d. Responsible for inventory and management of any equipment, materials, stockpiles, etc. provided by the FDOT.

e. Operate tools and small equipment and perform day-to-day maintenance activities as directed by the Department and defined in the Scope of Work.
f. Complete Daily Maintenance Crew Report (Form 325-010-01) in the format acceptable to the Department. Reports will be turned in at the end of each workday.
g. Supervisor must have a smart phone available and be capable of receiving telephone calls, emails, text messages and pictures.
6.2.2 SUPERVISOR QUALIFICATIONS
a. Five years of infrastructure maintenance and/or construction experience.
b. Valid Class “E” Driver’s License, at a minimum.
c. Demonstrated experience planning and monitoring the work activities of field personnel providing highway construction and/or maintenance activities.

d. Thorough knowledge of practices, materials, methods, and equipment used in highway routine maintenance activities.
e. Ability to interpret and adhere to Section 2.0, Policies, Manuals, Procedures, and Guidelines of the scope of services.
f. Working knowledge of operation, repair and maintenance of equipment supplied by FDOT utilized in performing the activities defined in the scope of services.
g. Ability to clearly communicate orally and written in English.
h. Completion of an FDOT approved Temporary Traffic Control Intermediate
Course.
6.2.3 MAINTENANCE WORKER MINIMUM QUALIFICATIONS
a. Working knowledge of typical roadway maintenance activities including operation of equipment utilized in performing the activities defined in the scope of work.
b. Working knowledge of operating tools and small equipment and performing day-to-day activities as directed by the Vendor’s Supervisor and defined in the Scope of Work and Maintenance Activities
6.3 DEPARTMENT REQUEST TO REPLACE EMPLOYEE
a. Any person employed by the Vendor who does not perform in a proper and skillful manner or displays any conduct which would present a negative impact on the FDOT, or facility will at the request of the FDOT, be removed from further work on the contract by the Vendor.
b. Such employees shall not return to work on this contract without the FDOT’s prior written approval.
c. The FDOT reserves the right for the final decision to accept or reject any individual being considered for assignment by Vendor.

7.0 VENDOR PROVIDED EQUIPMENT
The Vendor shall provide a crew cab truck with a truck mounted arrow board and capable of transporting a field crew, materials, and debris as well as hauling an equipment trailer. A crew cab will be needed each workday. The crew cab will transport the field crew to the Maintenance Facility and to the work site. The crew cab must also be capable of hauling an FDOT equipment trailer up to 10,000 lb.

8.0 PROJECT SCHEDULE AND WORK HOURS
The Vendor will provide labor coverage for a 40-hour concurrent work week, typically consisting of four 10-hour days. The start of the work week will be defined as 7:00 A.M. on Monday. Typical workdays will be defined as 7:00 AM to 5:30 PM, Monday through Thursday. The Vendor should not permit any worker to labor more than 40 hours during one calendar week. The Department’s official workweek runs from 12:00 AM Friday to 11:59 PM on Thursday. This contract will match the Department’s official work week. Holiday time is not official work time and will not be paid by the Department.
The vendor shall keep an accurate record showing the actual hours worked each day and each calendar week by each worker employed on the contract and shall be kept open at all reasonable hours to the inspection of the Department. Time sheets must be submitted with billing to verify hours worked by each employee.
9.0 SERVICE PERIOD
The performance period of this contract will begin upon execution and be in effect for Two Years.
10.0 TERMINATION ACTION AND DEFAULT
Reference is made for Section 6, of the Standard Written Agreement. Any necessary default action will be processed in accordance with Department of General Services Rule 60A-1.006(3).
