Mold Project Design and Oversight Management:
Once the data has been collected from the initial inspection, Dominion will provide the client with a report of findings with recommendations. The depth of the recommendations are based on the needs of the client and can range from general recommendations to an in-depth project specification. All aspects of the project design will follow the latest edition of the IICRC S520.
During some projects, past clients have requested that Dominion conduct project oversight. While a project oversight is not necessary for all mold projects, it may be helpful for larger, more complex projects. Projects that occur in large commercial buildings with more involved building systems can benefit most from project oversight. An oversight can allow the project to continue to move smoothly when unexpected issues arise, such as when mold is identified on a fire rated ventilation chase or in an elevator shaft. With the consultant on site, recommendations can be made on the spot without having to stop work and wit for an additional visit. An oversight can also be beneficial for allowing post-remediation verification to occur as soon as an area is complete. Again, large projects with multiple work areas undergoing remediation simultaneously would benefit the most from project oversight.
Question 3- Provide a narrative response that describes your Organization’s overall method of conducting building inspections.
Asbestos Survey – General Procedures:
Prior to performing the building inspection, an EPA-accredited Asbestos Hazard Emergency Response Act (AHERA) certified inspector will review any existing drawings or reports, to familiarize the inspector with any previously identified asbestos-containing materials (ACM) that may be present. The Inspector will also

Department of Environmental Quality
25 Solicitation Attachments conduct interviews with the building engineers or any other knowledgeable personnel that may have information concerning renovations, additions or demolition scheduled for the building or space in question.
On completion of the document review and interviews, the inspector will conduct an initial building walk-through, to identify all homogenous suspect materials and record the presence of the materials on an inventory list. At this time, each building material will also be assessed for friability, material type, condition, and location by functional space. In addition, each individual homogenous building material will be quantified by square, linear, or cubic footage as appropriate. Once this is established, a sampling scheme for each individual building material can be created in order to meet all applicable regulatory guidelines (i.e.
ASTM E2356-09, NESHAP 40 CFR 61, AHERA 40 CFR 763, and OSHA 29 CFR 1926.1101) along with any local regulatory guidelines.
Bulk samples will then be collected pursuant to the above stated regulatory sampling guidelines. On completion of the bulk sampling, a written sample log will be completed, identifying each sample with a unique sample number along with the exact location of each sample. The sample locations will also be plotted on internally generated CADD drawings, for future reference, as needed. At the conclusion of the bulk sampling, the samples, along with a laboratory chain-of-custody, will be sent to a competitively priced third-party accredited laboratory for Polarized Light Microscopy (PLM) analysis.
Once a copy of the PLM bulk sample analysis report from the laboratory is received, the results will be reviewed. Any friable materials containing five percent (5%) or less asbestos content under PLM analysis will be resubmitted for point count analysis. In addition, a Transmission Electron Microscopy (TEM) analysis will be performed on one (1) negative or less than or equal to one percent (≤1%) asbestos detected PLM analysis of any non-friable organically bound materials, if requested by the client. Once all the sample analysis results have been obtained from the laboratory, an asbestos inspection report will be generated.
The survey report will include, but will not be limited to: date of inspection, physical description and use of the facility, inventory of sampled suspect materials, a sample description log, a sample location map, manner of sampling, NESHAP classifications, OSHA class of work, AHERA category, accessibility and condition, estimated quantity of ACM, map(s) identifying the location of the ACM, risk assessment, recommendations, copy of the laboratory PLM bulk sample analysis report including the laboratory chain of custody, current copy of the inspectors valid certification, copy of the laboratory accreditation, and a NESHAP notification summary sheet. An electronic copy of the report will be submitted to the client.
If ACMs are revealed as a result of the asbestos survey, two (2) abatement cost estimates will be submitted under separate cover. The first cost estimate will include a unit cost for each ACM, along with a total cost estimate for the removal of all 0ACM. The second cost estimate will include the Company project oversight and sampling fees.
Lead Survey – General Procedures:
All surveys will be performed by an EPA certified Lead Inspector or Risk Assessor in accordance with EPA regulations, Lead: Requirements for Lead-Based Paint (LBP) Activities Final Rule, Title 40, Part 745, Vol.
61 No. 169, pg. 45813, Section 227, Subpart L, of the Code of Federal Registrar and any other regulatory guidelines that apply. Also, if the survey’s intent is for the disturbance by workers, the Company will comply with the OSHA 1926.62 regulations, which states that if detectable amounts of lead are identified in a painted substrate, the outlined OSHA guidelines apply.
Prior to performing the lead paint survey, an accredited inspector will review any existing drawings or reports, in reference to any identified lead paints. The Inspector will conduct interviews with the building engineers or any other personnel that may have information concerning remodels or additions to the building or space in question. The objective of the survey is to determine if lead paint is present in the

26 Solicitation Attachments building(s). If lead paint is present, each individual lead paint building component will be identified by location inside the building, by lead content, assessed condition, and by total quantification.
Paint Chip sampling analyzed by Atomic Absorption Spectroscope (AAS) will be utilized to sample the components of the building(s) for the identification of lead paint. The paint chip samples will be sent, along with a written chain of custody, to an accredited AAS testing laboratory. At the request of the client, onsite Portable X-Ray Fluorescence (XRF) testing can also be utilized.
As required by EPA 40 CFR Part 260, Toxic Characteristic Leaching Procedure (TCLP) sampling will be performed on all lead paint waste prior to its disposal into an EPA landfill. If the lead paint waste is above the EPA standard of five parts per million (5 ppm), the waste will be disposed as hazardous waste, and if the waste is below that EPA standard it can be disposed as general waste.
The survey report will include but will not be limited to: date of inspection, address and physical description of facility, accreditation of the Inspector and signature, laboratory analytical results (AAS analysis) including the written chain of custody, complete documentation of all XRF readings or paint chips collected, CADD drawings or blueprints identifying the areas of lead paint and associated quantities, classification of all surfaces of lead-containing or non-lead-containing categories based on the XRF or AAS laboratory analysis if lead paint is identified during the survey, the description of the lead paint categories would be as follows:
interior and exterior surface areas including type of substrate (i.e. wall system, ceilings, moldings and trim, floors and concrete), risk assessment and recommendations, technical information on the XRF equipment including the Performance Characteristic Sheet, accredited Inspector/Risk Assessor and laboratory certifications.
If lead paint is revealed during the survey, two (2) cost estimates under separate cover from the lead paint survey report will be submitted. The first cost estimate will include a cost for abatement at each lead paint location and a total cost estimate for the removal of all lead paint. The second cost estimate will include project oversight and sampling fees.
