APPENDIX A
Limited Data Set: A limited data set is not completely deidentified data, because it contains some of the identifiers listed above. While a limited data set is considered to be PHI under the HIPAA
Privacy Rule, it may be shared with you for research purposes provided a data use agreement is entered into with Collaborative and the appropriate approvals are obtained. Limited data sets may also be disclosed under HIPAA for public health purposes and for health care operations in accordance with 45 CFR § 164.514(e).
A limited data set is PHI that excludes the following direct identifiers of the individual (or the individual's relatives, employers or household members):
• Names;
• Address information, other than city, State and zip code;
• Telephone numbers;
• Fax numbers;
• E-mail addresses;
• Social Security numbers;
• Medical record numbers;
• Health plan beneficiary numbers;
• Account numbers;
• Certificate/license numbers;
• Vehicle identifiers and serial numbers, including license plate numbers;
• Device identifiers and serial numbers;
• Web Universal Resource Locators (URLs);
• Internet Protocol (IP) addresses;
• Biometric identifiers, including finger and voice prints;
• Full-face photographs and any comparable images.
Note that limited data sets can contain city, county and zip code (5 digit only), other information about an individual (such as age) as long as it is not one of the identifiers listed above, and dates that are directly related to an individual, including birth date, dates of service, admission date, and discharge date.
Before you will be furnished with a limited data set from Collaborative, you must agree to protect the information from being disclosed or used except for these research purposes. In addition, you must agree not to try to identify individuals whose information is included in the limited data set by linking that information with other identifiable information in other databases.

APPENDIX B
Postpartum Care Disparities in Women with Gestational Diabetes:
Current Care Patterns and Opportunities for Improving Outcomes
Requestor(s) : Katilyn Crosby, PhD, MHA, BS
University of Nebraska Medical Center 984388 Nebraska Medical Center | Omaha, NE 68198-4355
What are the projects objectives?
This study aims to describe and identify the predictors of 1) the transition from postpartum obstetric care to postpartum primary care and 2) adherence to postpartum glucose testing among women with GDM in Nebraska and Iowa. Access to and utilization of healthcare services is limited by environmental factors related to rurality as they face more burdens due to travel distance and lack of providers. We hypothesize that location of residence in rural areas, greater distance to obstetric/ primary care providers, and lack of obstetric/primary care providers in a given area are associated with lower odds of transitioning to primary care and completing guideline recommended diabetic testing in the postpartum period.
What is the project methodology?
Through the Nebraska Health Collaborative data, a retrospective, cross-sectional cohort design, merged with AHRQ social determinants data, will be deployed and guided by the Andersen theoretical model. Multivariable logistic regression will be estimated to assess the associations and sensitivity analyses.
Who will access this data?
RWJF and UNMC Research Team.
What are the data points requested:
Table 1 below shows the CPT Codes needed for data production for this project.
Where and how long will data be stored; how will it be destroyed?
5 years
Identify expected data flow?
Limited Data Set provided via secure Box file.

How you must inform us of your decision to receive future notices and disclosure in paper format and withdraw your consent to receive notices and disclosures electronically is described below.
Consequences of changing your mind
If you elect to receive required notices and disclosures only in paper format, it will slow the speed at which we can complete certain steps in transactions with you and delivering services to you because we will need first to send the required notices or disclosures to you in paper format, and then wait until we receive back from you your acknowledgment of your receipt of such paper notices or disclosures. Further, you will no longer be able to use the DocuSign system to receive required notices and consents electronically from us or to sign electronically documents from us.
All notices and disclosures will be sent to you electronically
Electronic Record and Signature Disclosure created on: 5/16/2023 6:53:28 PM Parties agreed to: Larra Petersen-Lukenda, Michael Hrncirik, Pam Ehmke
Unless you tell us otherwise in accordance with the procedures described herein, we will provide electronically to you through the DocuSign system all required notices, disclosures, authorizations, acknowledgements, and other documents that are required to be provided or made available to you during the course of our relationship with you. To reduce the chance of you inadvertently not receiving any notice or disclosure, we prefer to provide all of the required notices and disclosures to you by the same method and to the same address that you have given
us. Thus, you can receive all the disclosures and notices electronically or in paper format through the paper mail delivery system. If you do not agree with this process, please let us know as described below. Please also see the paragraph immediately above that describes the consequences of your electing not to receive delivery of the notices and disclosures electronically from us.
