Assumption of the Risk and Waiver of Liability
Relating to Coronavirus/COVID-19
The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health
Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people.
Mid Plains Food & Lodging (“the Caterer") and affiliated companies have put in place preventative measures to reduce the spread of COVID-19; however, the Caterer cannot guarantee that you or your guests will not become infected with COVID-19. Further, attending an event could increase your risk and your guest's risk of contracting COVID-19.
By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my guests and I may be exposed to or infected by COVID-19 by attending an event and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the
7/15/2025 - 10:36:55 AM Page 3 of 4 risk of becoming exposed to or infected by COVID-19 at the event may result from the actions, omissions, or negligence of myself and others, including, but not limited to, MPFL employees, location staff, volunteers, and program participants and their families.
I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my guests or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my guests may experience or incur in connection with my guest's attendance at the event (“Claims”). On my behalf, and on behalf of my guests, I hereby release, covenant not to sue, discharge, and hold harmless Mid Plains Food & Lodging, its employees, agents, and representatives, and parent company, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of the Caterer, its employees, agents and other representatives, whether a COVID-19 infection occurs before, during, or after attendance at an event.

From: Shelby Lynn Rosasco To: Angie Mohr Cc: Rick Funston Subject: Re: ARSBC Leadership Team Meeting Space Date: Tuesday, August 12, 2025 4:50:33 PM
Hi Angie, Based on the catering/food expenses from the last 2 meetings I would estimate around $800-$1000.
The catering at the Best Western is a little less expensive than the venue in Georgia and Wyoming, so we may end up less than that. As I mentioned in my email to Rick our BRTF group will cover all of the expenses (either reimburse or pay the invoice).
Do you need a copy of the contract with UNL information filled in?
Thank you for all of your help with this!
Best, Shelby

From: Shelby Lynn Rosasco <srosasco@uwyo.edu> Sent: Thursday, July 24, 2025 5:55 PM To: Rick Funston <rick.funston@unl.edu>; Randy Saner <randy.saner@unl.edu> Cc: Sandy Johnson <sandyj@ksu.edu> Subject: ARSBC Leadership Team Meeting Space
Good afternoon, During yesterday's meeting we briefly discussed the BRTF Leadership Team Meeting/Dinner that will be held on 9/16. The space at the Best Western is available that evening and would cost $250.00, which includes use of A/V equipment and coffee, tea, and water. They do have catering available, or mailto:srosasco@uwyo.edu mailto:rick.funston@unl.edu mailto:randy.saner@unl.edu mailto:sandyj@ksu.edu we can use an outside caterer, however, I believe we would need to use them for the bar/alcohol. I have attached the menu that they forwarded to me.
We all agreed this space would be the easiest from a travel and logistics standpoint and are fine with the $250 charge for renting the space. We will need to sign a contract to rent the space, would UNL be willing to sign the contract? BRTF can cover the costs associated with the meeting/dinner (either reimburse UNL or pay the invoice directly), but in our discussion yesterday it seemed like it would be best for the contract to be handled through UNL. They sent me an initial contract, but it is filled out with UW information. I have gone ahead and attached the contract so you can see what the contract looks like. If UNL is willing to sign the contract I can have them update it with the correct information.
Let me know if you have any questions.
Thanks, Shelby

$0.00 per-page fee. You may request delivery of such paper copies from us by following the procedure described below.
Withdrawing your consent
If you decide to receive notices and disclosures from us electronically, you may at any time change your mind and tell us that thereafter you want to receive required notices and disclosures only in paper format. How you must inform us of your decision to receive future notices and disclosure in paper format and withdraw your consent to receive notices and disclosures electronically is described below.
Consequences of changing your mind
If you elect to receive required notices and disclosures only in paper format, it will slow the speed at which we can complete certain steps in transactions with you and delivering services to you because we will need first to send the required notices or disclosures to you in paper format, and then wait until we receive back from you your acknowledgment of your receipt of such paper notices or disclosures. Further, you will no longer be able to use the DocuSign system to receive required notices and consents electronically from us or to sign electronically documents from us.
All notices and disclosures will be sent to you electronically
Electronic Record and Signature Disclosure created on: 5/16/2023 6:53:28 PM Parties agreed to: Michael J Zeleny, Melody Stark
Unless you tell us otherwise in accordance with the procedures described herein, we will provide electronically to you through the DocuSign system all required notices, disclosures, authorizations, acknowledgements, and other documents that are required to be provided or made available to you during the course of our relationship with you. To reduce the chance of you inadvertently not receiving any notice or disclosure, we prefer to provide all of the required notices and disclosures to you by the same method and to the same address that you have given
us. Thus, you can receive all the disclosures and notices electronically or in paper format through the paper mail delivery system. If you do not agree with this process, please let us know as described below. Please also see the paragraph immediately above that describes the consequences of your electing not to receive delivery of the notices and disclosures electronically from us.
