About this file

This document is Amendment No. 4 to a Mandatory Use Contract issued by the State of Ohio's Department of Administrative Services (DAS) for the Ohio Department of Mental Health and Addiction Services. The contract is with Cardinal Health Pharmacy Services, LLC, located in Chicago, Illinois, and pertains to a contract originally opened through Request for Proposal (RFP) No. CSP904122 on 08/13/21.

The contract covers Supplemental Remote Pharmacy Review Services and is being renewed for an additional 24 months, extending from 11/01/24 through 10/31/26. A key update in this amendment includes modifications to the Prohibition of the Expenditure of Public Fund for Offshore Services clause, which restricts services and data management outside the United States. Additionally, the amendment includes a provision for Cardinal Health to cover daily operational needs during an Epic conversion at a rate of $130.00 per hour, specifically during the hours of 7 AM through 4 PM, as instructed by the Department of Mental Health. The contract remains a requirements contract, meaning the Ohio Department of Mental Health and Addiction Services can make purchases at any time and in any amount, with no guaranteed volume of services.
