About this file
This is a Disqualification Disclosure form required by the City of Pasadena Department of Transportation in accordance with California Public Contract Code Section 10162 for the Preparation of Technical Transportation Studies: On-Call Services solicitation. The form requires prospective bidders to certify under penalty of perjury whether the bidder, any officer of the bidder, or any employee with proprietary interest in the bidder has ever been disqualified, removed, or otherwise prevented from bidding on or completing a federal, state, or local government project due to a violation of law or safety regulation. Bidders must indicate either "Yes" or "No" in response to this question, and those answering affirmatively must provide written explanation of the circumstances along with supporting documentation.
This disclosure requirement is a mandatory compliance document that establishes bidder eligibility and integrity for the on-call transportation consultant services contract. The form serves as a screening mechanism to ensure that only qualified firms without disqualifying violations participate in the selection process, thereby protecting the City's interests and maintaining the quality and reliability of the consultant team that will be assigned project-specific task orders for transportation impact analyses throughout Pasadena's development review process.
