About this file

This is a Disqualification Disclosure form required by the State of California under Public Contract Code Section 10162, applicable to the City of Pasadena's Mobile Substation Project. The form requires prospective bidders to disclose, under penalty of perjury, whether the bidder, any officer, or any employee with a proprietary interest has been disqualified, removed, or otherwise prevented from bidding on or completing a federal, state, or local government project due to violations of law or safety regulations. Bidders must indicate whether their answer is yes or no, and if affirmative, provide a detailed explanation of the circumstances with supporting documentation attached.

The Mobile Substation Project involves the City of Pasadena seeking proposals from qualified manufacturers to provide two mobile substations for Pasadena Water and Power to support emergency power needs and construction initiatives, with an in-service date targeted for June 2029. This disclosure requirement ensures compliance with California contracting law and serves as a prerequisite qualification mechanism for all bidding entities on this public works project.
