About this file
This document is a Disqualification Disclosure form aligned with California Public Contract Code §10162, applicable to the City of Pasadena's commercial appraisal services contract opportunity. The form requires prospective bidders to disclose under penalty of perjury whether the bidder, any officer, or employee with proprietary interest has ever been disqualified, removed, or prevented from bidding on or completing a federal, state, or local government project due to a violation of law or safety regulation.
The disclosure form provides a simple yes/no checkbox mechanism for bidders to declare their past compliance history, with an option to explain and attach documentation if the answer is affirmative. This document serves as a transparency and accountability mechanism to ensure contractor eligibility for the City of Pasadena's commercial and vacant land appraisal services contract, helping to maintain ethical standards in government procurement processes.
