About this file

This document is a memorandum from the Arizona Department of Health Services, Office of Procurement, dated November 18, 2025. The memo involves a Request for Quotes (RFQ) No. BMP007097 for Specimen Testing, with RecoveryTrek LLC as the awarded vendor. The document was prepared by Emily Ngo, Procurement Specialist, and approved by Gina Corwin, Chief Procurement Officer.

The memorandum addresses a procedural oversight in the procurement process, specifically that the procurement file was not made publicly available on the Arizona Procurement Portal (APP) on the same day as the award, as required by the Arizona Procurement Code. The award for this RFQ was finalized on November 17, 2025, but the required documents were not immediately uploaded. Once the oversight was identified, the correct documents were prepared and uploaded, making the procurement file publicly accessible. No specific details about the specimen testing services, contract value, or contract terms are provided in this administrative memorandum.

Memorandum | Office of Procurement

Date: November 18, 2025

To: Memo to Procurement File

From: Emily Ngo, Procurement Specialist

Regarding: BPM007097, Specimen Testing – RecoveryTrek LLC.

Subject: Request for Quotes (RFQ) No. BMP007097 Specimen Testing

In accordance with the Arizona Procurement Code, Rules and Regulations, §41-2533 and R2-7-D303(D), Public Access to Procurement Information, the procurement file was not made available to the public on the same day as award as required. This delay occurred due to a Procurement Officer’s oversight in preparing and uploading the award documentation in the Arizona Procurement Portal (APP).

The award for this RFQ was finalized on November 17, 2025; however, the required documents were not completed and made available in APP on the same day as required by the Arizona Procurement Code.

Once identified, the correct documents were prepared and uploaded into the APP solicitation, and the procurement file was made publicly accessible.
