About this file

This is a Contract Amendment Summary from the Arizona Department of Transportation (ADOT) for a contract with Tony's Lawn Service, specifically Amendment Number One (1) for Contract Number CTR077316 covering Landscaping and Tree Trimming Services. The amendment is dated with a revision of 03/2025 and is located in Phoenix, Arizona.

The amendment primarily focuses on replacing Section 21 of the original contract, which details comprehensive insurance requirements for the contractor. The new section outlines specific insurance coverage mandates, including Commercial General Liability with aggregate limits of $2,000,000, Business Automobile Liability with a $1,000,000 combined single limit, and Workers' Compensation with statutory coverage. The amendment requires Tony's Lawn Service to provide certificates of insurance within 10 calendar days of contract award, maintain insurance policies throughout the contract period, and ensure that all subcontractors are also covered under the specified insurance requirements. The amendment does not appear to modify the scope of services, pricing, or contract duration, but instead updates the insurance provisions to ensure adequate protection for ADOT.
