About this file

This document is a Contract Amendment (#1) for Contract #CTR077252 issued by the Arizona Department of Child Safety's Office of Procurement & Contracts, located in Phoenix, Arizona. The amendment modifies an existing contract related to Parenting Time Services.

The amendment clarifies several key operational aspects of the contract, including procedures for contact attempts, cancellations, and missed visits. Specifically, it defines a "no-call/no-show" as a failure to arrive at a designated Parenting Time location within fifteen minutes of the scheduled start time without prior notice. The amendment also provides guidelines for make-up sessions, indicating that such sessions are not automatically required unless directed by the ADCS Specialist. Additionally, the document adjusts the timeline for price adjustment requests, stipulating that contractors must initiate written requests ninety days before the contract's annual extension date. The changes become effective on August 18, 2025, with all other contract terms remaining unchanged.
